اثر گاباپنتین بر درد پس از عمل جراحی هیسترکتومی شکمی

چکیده
مقدمه: یکی از مشکلات عمده در اعمال جراحی، درد پس از عمل جراحی می باشد که می تواند حال عمومی بیمار را پس از عمل به شدت تحت تأثیر قرار دهد. مطالعات نشان داده اند که گاباپنتین می تواند در کاهش درد پس از عمل جراحی و کاهش مصرف مخدر تأثیر داشته باشد. هدف از این مطالعه بررسی اثربخشی تجویز گاباپنتین به عنوان یک پیش دارو در کاهش درد پس از عمل جراحی هیسترکتومی شکمی و بهبود اثر آرامبخشی آن در مقایسه با دارونما است.
روش کار: این مطالعه کارآزمایی بالینی دو سوکور در سال 1390 در زایشگاه الزهرا (س) رشت انجام شد. 60 بیمار کاندید هیسترکتومی شکمی غیر اورژانس، به طور تصادفی به دو گروه گاباپنتین (1200میلی گرم) و پلاسبو (کپسولی با ظاهر شبیه گاباپنتین) تقسیم شدند. بیمار بلافاصله پس از به هوش آمدن و 2 و 4 ساعت بعد از عمل در اتاق ریکاوری ویزیت و از نظر مقیاس دیداری- تصویری درد بررسی و نتایج حاصل یادداشت شد. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار آماری SPSS (نسخه 16) و آزمون‌های کای دو و من ویتنی انجام شد.
یافته ها: دو گروه از لحاظ سن و مدت زمان جراحی همسان بودند. میانگین درجات درد (VAS) در دو گروه در هر 3 مرحله ارزیابی تفاوت معنی‌داری داشت (0001/0=p). همچنین میزان پتیدین دریافتی در گروه شاهد به طور معناداری بیشتر از گروه گاباپنتین بود (0001/0=p).
نتیجه گیری: تجویز گاباپنتین باعث کاهش شدت درد پس از عمل جراحی و بهبود کیفیت آرامبخشی در حین و پس از عمل جراحی می شود. از طرفی دیگر تجویز گاباپنتین به عنوان پیش دارو، نیاز به تجویز مخدر را کاهش می دهد.

Introduction: Postoperative pain is a major problem in general surgery which can severely effect on patient. Studies have shown that gabapantin can produce analgesia and reduce the need for opioids in postoperative patients. In this study the efficacy of gabapantin were studied as a premedication for reduce pain and need for opioids in abdominal hysterectomy.
Methods: This double-blind clinical trial was performed in Alzahra maternity of Rasht in 2011. 60 patients who undergo elective abdominal hysterectomy randomly were divided into gabapantin group (1200mg) and placebo group (capsule like gabapantin). Patients visited immediately after recovery and on second and forth hours after surgery in recovery room and VAS was recorded. Data were analyzed with SPSS version 16 by t-student and Mann-Whitney tests.
Results: Age and surgery duration were matched in both groups. VAS was significantly difference in two groups for each of 3 visiting times (p=0.0001). Also the need for petidine was significantly more in control group than case group (p=0.0001).
Conclusion: Administration of gabapantin can reduce postoperative pain and improve sedation after surgery. Gabapantin as a premedication can also reduce need for opioids prescription.
