مطالعه موردی بیهوشی هیپنوتیزمی‌ در جراحی توده پستان

چکیده

مقدمه: سرطان پستان، مهم‌ترین و رایج‌ترین بیماری زنان و دومین میزان مرگ‌و‌میر بعد از سرطان ریه می‌باشد. با توجه به اینکه توده پستان، درمان دارویی نداشته و درمان اصلی و قطعی آن جراحی است و عوارض ناشی از بیهوشی دارویی در آنها بالا بوده، لذا استفاده از بیهوشی هیپنوتیزمی، راه مناسبی برای کاهش عوارض ناشی از بیهوشی، بهبود سریع‌تر جراحت و کاهش مدت زمان اقامت در بیمارستان است. با توجه به فواید استفاده از هیپنوتیزم در جراحی پستان، این پژوهش با هدف بررسی موردی بیهوشی هیپنوتیزمی‌ در جراحی توده پستان انجام گرفت.
معرفی بیمار: در این مطالعه موردی که از درمان غیردارویی (روان‌درمانی) برای جلوگیری از عوارض بیهوشی دارویی از بیهوشی هیپنوتیزمی ‌استفاده شد، نمونه مورد بررسی زن 45 ساله بود که با تشخیص توده پستان در بیمارستانی در شهر ساری بستری و چند ساعت پس از عمل جراحی پارشیل ماستکتومی با حال مساعد از بیمارستان ترخیص گردید. هیپنوتیزم توسط روان‌پزشک و فلوشیپ روان درمانی که در انجمن هیپنوتیزم بالینی ایران دوره هیپنوتیزم درمانی را گذرانده بودند، انجام شد. بیمار تحت جراحی مستقیم با بیهوشی هیپنوتیزمی قرار گرفت و پس از اتمام این جراحی که حدود 30 دقیقه زمان برد، با استفاده از القای مستقیم هیپنوتیزم، بیمار هوشیاری کامل خود را به‌دست آورد و چند ساعت پس از عمل با حال مساعد بیمارستان را ترک کرد.
نتیجه­ گیری: با توجه به عمل‌های متعدد جراحی و فواید هیپنوتیزم، توصیه می­ شود شرایط بکارگیری از این روش با توجه به ماهیت درمان فراهم شود.

Introduction: Breast cancer is the most important and common disease among women, which has the second highest death rate after lung cancer. Considering that the breast mass has no medical treatment and its definitive treatment is surgery and the side effects caused by drug anesthesia are high, so using hypnotic anesthesia is a good way to reduce the effects caused by anesthesia, faster healing the injury and reducing the length of hospital stay. Therefore, considering the benefits of using hypnosis in breast surgery, this research was conducted as a case study of hypnotic anesthesia in breast lump surgery.
Case presentation: In this case study, which used non-drug treatment (psychotherapy) i.e. hypnotic anesthesia to prevent the side effects of medical anesthesia, the investigated sample was a 45-year-old woman who was admitted to a hospital in Sari after diagnosed with a breast mass and was discharged from the hospital in a good condition a few hours after the partial mastectomy. Hypnosis was performed by an experienced psychiatrist and psychotherapy fellowship in the Scientific Association of Clinical Hypnosis of Iran. The patient underwent direct surgery by hypnotic anesthesia and after the completion of the surgery, which took about 30 minutes, using direct induction of hypnosis, the patient regained full consciousness and left the hospital a few hours after the operation in a good mood.
Conclusion: Considering the many surgical operations and the benefits of hypnosis, it is recommended to provide the conditions for using this method according to the nature of the treatment.
