بررسی تأثیر داروهای گیاهی بر درمان گرگرفتگی در زنان یائسه: یک مطالعه مروری نظاممند و متاآنالیز

چکیده

مقدمه: علائم وازوموتور دوران یائسگی به‍‌ طور میانگین در 75% زنان رخ می‌دهد. گرگرفتگی شایع‌ترین این علائم است. درمان‌های هورمونی، مؤثرترین روش درمانی برای درمان این علائم می‌باشند که با توجه به خطرات شناخته شده هورمون درمانی، بسیاری از پژوهشگران به بررسی تأثیر داروهای گیاهی برای بهبود علا‌ئم وازوموتور به‌ویژه گرگرفتگی پرداخته‌اند، لذا مطالعه حاضر با هدف بررسی مطالعات کارآزمایی بالینی انجام شده بر روی اثر‌بخشی انواع داروهای گیاهی بر گرگرفتگی انجام شد.
روشکار: در این مطالعه جهت یافتن مقالات مرتبط، جستجو در پایگاه‌های Google scholar، Scopus، PubMed، web of science، science direct و پایگاه‌های داده‌های ملی همچون بانک اطلاعات نشریات کشور (MagIran)، پایگاه اطلاعات علمی جهاد دانشگاهی کشور (SID)، پژوهشگاه علوم و فناوری اطلاعات ایران (IranDoc) و سامانه دانش گستر برکت (IranMedex) از ابتدای ژانویه 2005 تا ژانویه 2020 انجام شد. داده‌ها با استفاده از نرم‌افزار STATA (نسخه 2/12) و با استفاده از روش متاآنالیز، مدل اثرات تصادفی با هم ترکیب شدند.
یافته ­ها: در مطالعه حاضر 11 کارآزمایی بالینی با توجه به معیارهای ورود به متاآنالیز وارد شدند. در گروه گیاهان دارویی میانگین کاهش تعداد گرگرفتگی به‌میزان 41/2 دفعه (56/1- تا 27/3- :CI 95%) کمتر از گروه کنترل بود که این میزان کاهش از نظر آماری معنی‌‌‌دار بود (05/0>p). علاوه بر این در گروه گیاهان دارویی، شدت گرگرفتگی به‌میزان 28/20 درجه (70/71- تا 87/22- :CI 95%) نسبت به گروه کنترل کمتر بود و این تفاوت از نظر آماری معنادار بود (05/0>p).
نتیجه ­گیری: گیاهان دارویی حاوی فیتواستروژن‌ها، تأثیرات خوبی بر کاهش تعداد دفعات و شدت گرگرفتگی زنان یائسه دارند و می‌توانند درمان مناسبی برای درمان گرگرفتگی زنان یائسه باشند.

Introduction: Vasomotor symptoms during menopause occur on average in 75% of women. Hot flashes are the most common of these symptoms. Hormonal drugs  are the most effective method of treatment. Due to the known risks of hormone therapy, many researchers have investigated the effect of herbal medicines to improve vasomotor symptoms, especially hot flashes. This study was performed with aim to investigate the clinical trial studies conducted on the effectiveness of various herbal medicines on hot flashes.
 Methods: In this study, to find the related articles, databases of Google scholar, Scopus, PubMed, Web of science, Science direct and national databases such as (MagIran), (SID), (IranDoc) and (IranMedex) were searched from the beginning of January 2005 to January 2020. Data were combined using STATA software (version 12.2) and using meta-analysis method, random effects model.
Results: In the present study, 11 clinical trials were included in the meta-analysis according to the inclusion criteria.In the medicinal plants group, mean reduction in the number of hot flushes was 2.41 times (95% CI: -1.56 to -3.27%) less than the control group, which was statistically significant (p<0.05).In addition, in the medicinal plants group, the intensity of hot flashes was 20.28 degrees (95% CI: -71.70 to -22.87) compared to the control group, and this difference was statistically significant (p<0.05).
Conclusion: Medicinal plants containing phytoestrogens have good effects on reducing the frequency and intensity of hot flashes in menopausal women and can be a suitable treatment for hot flashes in menopausal women.
