اثر گیاه ختمی بر عملکرد جنسی زنان یائسه: کارآزمایی بالینی شاهددار تصادفی شده

چکیده

مقدمه: یکی از شکایت‌های دوران یائسگی، مشکلات جنسی و درد حین مقاربت است. بر اساس طب ایرانی، گیاه ختمی دارای اثر رطوبت‌بخشی و نرم‌کنندگی است. مطالعه حاضر با هدف تعیین اثر فرآورده گیاهی ختمی بر عملکرد جنسی زنان یائسه انجام شد.
روش‌کار: این مطالعه کارآزمایی بالینی دوسوکور در سال 1398 بر روی 60 زن یائسه مراجعه‌کننده به بیمارستان ام‌البنین (س) مشهد و دارای اختلال عملکرد جنسی انجام شد. افراد به‌طور تصادفی در دو گروه ۳۰ نفره درمان و دارونما تقسیم شدند. شیاف واژینال ختمی 5% یا دارونما به‌مدت 2 هفته هر شب و سپس 6 هفته شب در میان تجویز شد. پرسشنامه فعالیت جنسی زنان (FSFI) در ابتدا و انتهای مداخله تکمیل شد. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار آماری SPSS (نسخه 18) و آزمون تی مستقل، من‌ویتنی، کای دو و ویلکاکسون انجام شد. میزان p کمتر از 05/0 معنی‌دار در نظر گرفته شد.
یافته­ها: میانگین امتیاز تمامی حیطه‌های عملکرد جنسی پس از 8 هفته مداخله در گروه درمان افزایش پیدا کرد که این تغییرات نسبت به دارونما به‌طور معنی‌داری بالاتر بود. میانگین نمره کل پرسشنامه قبل از مداخله در گروه درمان از 25/5±45/17 به 01/4±21/26 (0001/0=p) و در گروه دارونما از 65/5±93/21 به 06/5±89/23 رسید (001/0=p).
نتیجه­گیری: فرآورده گیاهی ختمی اثر مفیدی بر میزان عملکرد جنسی زنان یائسه دارد، لذا می‏تواند به‌عنوان درمان جایگزین پیشنهاد گردد.

چکیده [English]

Introduction: One of the complaints of menopausal period is sexual problems and dyspareunia. According to Persian Medicine, Alcea has a moisturizing and softening effect. This study was performed with aim to determine the effect of Alcea herbal product on sexual function in postmenopausal women.
Methods: This double-blind clinical trial was performed in 2019 on 60 postmenopausal women with sexual dysfunction referred to Omolbanin Hospital in Mashhad. The subjects were randomly divided into two groups of treatment and placebo (n=30 in each group). Alcea vaginal suppository 5% or placebo was administered every night for 2 weeks and then every other night for 6 weeks. The FSFI (Female Sexual Function Index) questionnaire was completed at the beginning and end of the intervention. Data were analyzed by SPSS software (version 18) and Independent T-test, Man Whitney, Chi-square and Wilcoxon. P<0.05 was considered statistically significant.
Results: Mean score of all domains of sexual function increased after 8 weeks of intervention in the treatment group, which was significantly higher than placebo group. The mean total score of the questionnaire before the intervention in the treatment group reached from 17.45±5.25 to 26.21±4.01 (P=0.0001) and in the placebo group from 21.93±5.65 to 23.89±5.06 (P =0.001).
Conclusion: Alcea herbal product has a beneficial effect on sexual function in postmenopausal women, so it can be suggested as an alternative treatment.
