بررسی مشکلات جنسی پس از زایمان در زنان شیرده مراجعه‌کننده به مراکز بهداشتی درمانی، بندرعباس

چکیده

مقدمه: فعالیت جنسی و رضایت حاصل از آن به ‌عنوان یکی از مهم‌ترین ابعاد زندگی انسان شناخته ‌شده است که تحت تأثیر عوامل متعددی همچون بارداری و زایمان قرار می‌گیرد. با توجه به این که آگاهی از مشکلات جنسی زوجین در دوران پس از زایمان می‌تواند در ارتقاء کیفیت مراقبت‌های این دوران مؤثر باشد، لذا مطالعه حاضر با هدف بررسی مشکلات جنسی زنان شیرده در بندرعباس انجام‌ شد.
روش‌کار: این مطالعه‌ توصیفی و مقطعی در سال 1394 بر روی 130 زن نخست‌زا و شیردهی که به مرکز بهداشتی درمانی سیدالشهداء بندرعباس مراجعه کرده بودند، انجام شد. ابزار گردآوری داده‌ها شامل پرسشنامه‌ مشخصات فردی مامایی و چک لیست کوتاه نشانه‌های اختلال عملکرد جنسی به صورت محقق ساخته بود. داده‌ها پس از گردآوری با استفاده از نرم‌افزار آماری SPSS (نسخه 16) و آزمون های آماری توصیفی (میانگین و فراوانی) و تحلیلی (لوجستیک) مورد تجزیه و تحلیل قرار گرفت. میزان p کمتر از 05/0 معنی‌دار در نظر گرفته شد.
یافته‌ها: متوسط تعداد مقاربت در دوران پس از زایمان 09/1±33/1 بار در هفته و متوسط زمان شروع اولین مقاربت 12/2±81/6 هفته پس از زایمان بود. بیشترین مشکل جنسی گزارش شده زنان در دوران پس از زایمان به ترتیب مشکل درد حین مقاربت (4/85%)، مشکل مربوط به ارگاسم (6/84%)، مشکل کاهش رطوبت جنسی (3/82%)، مشکل کمبود میل جنسی (80%) و مشکل کاهش برانگیختگی جنسی (8/73%) بود. تمامی اختلالات جنسی با شیوع بیشتری در دو ماه پس از زایمان وجود داشت و شیوع آن در 4 ماه و 6 ماه پس از زایمان کاهش یافت (001/0>p).
نتیجه‌گیری: به طور کلیمشکلات جنسی با شیوع بیشتری در دو ماه پس از زایمان وجود دارد و شیوع آن در 4 ماه و 6 ماه پس از زایمان کاهش می‌یابد. به دلیل شیوع بالای مشکلات جنسی در این دوران،برگزاری دوره‌های آموزشی در جهت توانمندسازی ماماها برای مواجهه با مشکلات جنسی مراجعین خود، می‌تواند گام مهمی در جهت فعال نمودن ماماها در زمینه‌ ایفای نقش در این حیطه‌ مهم از وظایفشان باشد.

Introduction: Sexual activity and satisfaction from it is known as one of the most important aspects of human life that is affected by multiple factors such as pregnancy and childbirth. Given the fact that awareness of sexual problems in the postpartum period can be effective in improving quality of care in this period, this study was performed to determine sexual problems of breastfeeding women in Bandar Abbas.
Methods: This descriptive, cross-sectional study was carried out on 130 primipara breastfeeding women referred to Seyed-Alshohada health center of Bandar Abbas, Iran, in 2015. Data collection tools were a form of demographic and obstetric information and Brief Sexual Symptom Checklist (BSSC). To analyze the data, descriptive (frequency and mean) and analytical (logistic) statistics were used in SPSS, version 16. P-value less than 0.05 was considered statistically significant.
Results: The mean number of sexual intercourse during the postpartum period was 1.33±1.09 in a week and the mean time to resumption of the first intercourse was 6.81±2.12 postpartum weeks. Most sexual problems reported by women during the postpartum period were dyspareunia 111 (85.4%), orgasm problem 110 (84.6%), lack of sexual lubrication 107(82.3%), lack of sexual desire 104 (80%), and sexual arousal problem 96 (73.8%). All the sexual problems were more prevalent in the first two months after delivery, which diminished four and six months after delivery (P<0.001).
Conclusion: In general, sexual problems are more frequent in the first two postpartum months, which reduces four and six months after delivery. Because of high prevalence of sexual problems in this period, training programs for empowering midwives to deal with sexual problems of their clients can be an important step towards activating midwives regarding their role in this important portion of their duties.
