عملکرد جنسی زنان مبتلا به پرولاپس دستگاه تناسلی قبل و پس از اعمال جراحی کولپورافی قدامی خلفی

چکیده

مقدمه: اختلال عملکرد جنسی یک مشکل شایع در زنانی است که پرولاپس اعضای لگنی دارند که کیفیت زندگی و رابطه زناشویی آن‌ها را تحت تأثیر قرار می‌دهد. لذا مطالعه حاضر با هدف ارزیابی عملکرد جنسی زنان مبتلا به پرولاپس دستگاه تناسلی قبل و پس از اعمال جراحی کولپورافی قدامی خلفی انجام شد.
روش‌کار: این مطالعه توصیفی مقایسه‌ای از آذر 1393 تا آذر 1394 بر روی 105 زن مبتلا به پرولاپس اعضاء لگن مراجعه‌کننده به درمانگاه زنان شهید مفتح شهر یاسوج انجام شد. داده‌ها به‌وسیله پرسشنامه فردی و پرسشنامه استاندارد FSFI قبل و سه ماه پس از جراحی گردآوری شد. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار آماری SPSS (نسخه 18) و آزمون‌های کولموگروف اسمیروف، تی زوجی، کای دو و آزمون رگرسیون انجام شد. میزان p‌ کمتر از 05/0 معنی‌دار در نظر گرفته شد.
یافته‌ها: میانگین امتیاز عملکرد جنسی 3 ماه پس از عمل جراحی کولپورافی با قبل از جراحی اختلاف معنی‌داری نداشت (05/0p>)، ولی میانگین امتیاز رضایت جنسی 3 ماه پس از عمل جراحی کاهش معنی‌داری نشان داد (003/0=p). میانگین امتیاز حیطه‌های برانگیختگی جنس، ارگاسم و درد تفاوت معنی‌داری با قبل از عمل جراحی نداشت (05/0p>) و حیطه‌های میل جنسی و مرطوب شدن واژن به طور معنی‌داری کاهش یافت (05/0p>).
نتیجه‌گیری: اگرچه عمل جراحی از جمله کولپورافی، درمان انتخابی و مناسب جهت درمان پرولاپس اعضاء لگنی می‌باشد و می‌تواند باعث بهبود علائم پرولاپس شود، اما بر عملکرد جنسی تأثیری ندارد و رضایت جنسی را کاهش می‌دهد.

Introduction: Sexual dysfunction is a common problem in women with pelvic organ prolapse that impacts their quality of life and sexual relations. Therefore, this study was performed with aim to assess the sexual function in women with genital prolapse before and after anterior- posterior colporrhaphy.
Methods: This descriptive-comparative study was performed on 105 women with pelvic organ prolapse referred to Shahid Mofateh gynecology clinic of Yasouj in 2014-2015. Data were collected through a demographic questionnaire and Female Sexual Function Index (FSFI) before and three months after the surgery. Data were analyzed by SPSS software (version 18) and …., paired T-test, Chi-square and regression test. PResults: The mean score of sexual function before and 3 months after colporrhaphy had no significant difference with before the surgery (P>0.05), but mean score of sexual satisfaction showed significant decrease 3 months after colporrhaphy (P=0.003. Mean score of arousal, orgasm and pain didn’t have significant difference with before the surgery (P>0.05), and the score of desire and lubrication significantly decreased (P<0.05)
Conclusion: Although surgical treatment such as colporrhaphy is a chosen treatment for pelvic prolaps and can improve the symptoms of prolapse, but has no impact on sexual function and decrease sexual satisfaction.
