بررسی مقایسه ای تأثیر سیاه دانه با مفنامیک اسید بر خونریزی قاعدگی در دانشجویان مبتلا به دیسمنوره اولیه

چکیده

مقدمه: سیاه دانه از جمله گیاهانی است که به طور سنتی در درمان قاعدگی دردناک کاربرد دارد، ولی تأثیر این گیاه بر میزان خونریزی قاعدگی گزارش نشده است، لذا مطالعه حاضر با هدف مقایسه تأثیر سیاه دانه و مفنامیک اسید بر میزان خونریزی قاعدگی انجام شد.
روش کار: این مطالعه کارآزمایی بالینی دوسوکور در سال 1393 بر روی 70 نفر از دانشجویان مبتلا به دیسمنوره اولیه ساکن در خوابگاه های دانشگاه علوم پزشکی مشهد انجام شد. افراد به طور تصادفی در گروه مفنامیک اسید و سیاه دانه قرار گرفتند. تحقیق طی دو مرحله صورت گرفت. ابتدا تمامی دانشجویان برای دو سیکل از نظر میزان خونریزی پیگیری شدند. سپس به هرکدام از واحدهای پژوهش به طور تصادفی برای دو سیکل متوالی کپسول مفنامیک اسید و یا کپسول سیاه دانه داده شد. به منظور سنجش میزان خونریزی قاعدگی از چارت هیگام استفاده شد. داده ها با استفاده از نرم افزار آماری SPSS (نسخه 16) و آزمون های تی‌مستقل و فریدمن مورد تجزیه و تحلیل قرار گرفتند. میزان p کمتر از 05/0 معنی دار در نظر گرفته شد.
یافته ها: میانگین میزان خونریزی در گروه سیاه دانه قبل از مداخله 85/37±20/85 سی‌سی بود که در سیکل اول درمان به 2/34±62/85 و در سیکل دوم درمان به 81/31±77/85 افزایش یافت. در گروه مفنامیک اسید میانگین میزان خونریزی قبل از مداخله 31/38±77/83 بود که در سیکل اول درمان به 66/32±05/78 و در سیکل دوم درمان به 71/29±85/76 کاهش یافت. به طور کلی دو گروه از نظر میزان خونریزی در سیکل اول و دوم درمان تفاوت آماری معنی داری نداشتند (05/0>p).
نتیجه‌گیری: مصرف سیاه دانه می تواند باعث افزایش میزان خونریزی قاعدگی شود، اما این افزایش تفاوت آماری معنی داری با مصرف مفنامیک اسید ندارد.

چکیده [English]

Introduction: Nigella sativa is one of the plants traditionally used in the treatment of dysmenorrhea, but its effect on menstrual hemorrhage has not been reported. Therefore, this study was performed with aim to compare the effects of Nigella sativa and mefenamic acid on menstrual hemorrhage.
Methods:This double-blind clinical trial was performed on 70 students with primary dysmenorrhea who were resident dormitories of in Mashhad University of Medical Sciences in 2014. The patients were randomly assigned to two groups of mefenamic acid and Nigella sativa. The research was conducted in two stages. At first, all the students were followed for two cycles in terms of bleeding, and then each of the patients randomly received Nigella sativa or mefenamic acid capsules for two consecutive cycles. Data were analyzed using SPSS software (version 16), and independent t-test and Friedman test. P< 0.05 was considered significant.
Results: Mean bleeding before the intervention in nigella sativa group was 85.20 ±37.85 cc that increased to 85.62 ± 34.2 in the first and to 85.77 ± 31.81 in the second cycle of treatment. Mean bleeding before the intervention in mefenamic acid group was 83.77 ±38.31 and decreased to 78.05 ±32.66 in the first and to 76.85 ± 29.71 in the second cycle of treatment. Overall, two groups were not statistically significant in terms of bleeding in the first and second cycle of treatment (P<0.05).
Conclusion: Use of Nigella sativa can increase the amount of menstrual bleeding, but this increase was not significantly different with mefenamic acid.
