Introduction: Pre-sacra Epidermoid cyst is a rare diagnosis in middle-aged women which is often asymptomatic and discovered incidentally. Pre-operative biopsy is not recommended and surgical resection is the best diagnostic and therapeutic modality. This article presents a case of pre-sacral epidermoid cyst which was discovered during the staging of cervical cancer.
Case presentation: The patient was a 47-year-old woman complaining of menometrorrhagia in May 2015. After gynecological examination and biopsy, squamous cell carcinoma of the cervix was diagnosed for her. CT scan and ultrasound for ruling out cervical cancer metastasis revealed a large cystic mass in pre-sacral area. The patient initially underwent hysterectomy and bilateral salpingo-oophorectomy. Then, laparotomy was performed for resection of pre-sacral lesion in Mashhad Omid oncology hospital. Histopathological examination showed the diagnosis of pre-sacral epidermoid cyst.
Conclusion: Epidermoid cyst should be considered in the differential diagnosis of any asymptomatic pre-sacral cystic lesion in middle-aged women. Surgery is the optimal diagnostic and therapeutic method. The selection of surgery route depends on the size of the lesion and the surgeon's experience.
