اثرات مکمل اسیدهای چرب امگا-3 بر سطوح لیپیدهای سرم و شاخص های استرس اکسیداتیو در زنان مبتلا به سندرم تخمدان پلی کیستیک

چکیده

مقدمه: سندرم تخمدان پلی کیستیک شایع ترین بیماری غدد درون ریز در زنان سنین باروری می باشد که این افراد در معرض خطر بروز زودرس بیماری های قلبی عروقی قرار دارند. مطالعه حاضر با هدف بررسی اثرات مکمل اسیدهای چرب امگا-3 بر سطوح لیپیدهای سرم و شاخص های استرس اکسیداتیو شامل مالون دی آلدئید و ظرفیت تام آنتی اکسیدانی سرم در مبتلایان به سندرم تخمدان پلی کیستیک انجام شد.
روش کار: این مطالعه کارآزمایی بالینی دو سوکور در سال 1390 بر روی 64 بیمار مبتلا به سندوم تخمدان پلی کیستیک انجام شد. افراد به طور تصادفی در دو گروه دریافت کننده اسیدهای چرب امگا-3 (32 نفر) و دارونما (32 نفر) قرار گرفتند. افراد گروه اسیدهای چرب امگا-3 به مدت 8 هفته روزانه 4 گرم اسیدهای چرب امگا-3 و گروه دارونما به مدت مشابه، دارونما دریافت کردند. اندازه گیری های تن سنجی و بیوشیمیایی و ارزیابی دریافت غذایی از طریق پرسشنامه یادآمد غذایی 24 ساعته و نرم افزار Nutritionist (نسخه 4) در ابتدا و انتهای مطالعه صورت گرفت. داده ها پس از گردآوری با استفاده از نرم افزار آماری SPSS (نسخه 5/11) و تست های آماری تی مستقل، تی زوجی و آنالیز کوواریانس مورد تجزیه و تحلیل قرار گرفت. مقادیر p کمتر از 05/0 معنی دار در نظر گرفته شد.
یافته ها: اسیدهای چرب امگا-3 باعث کاهش معنی داری در سطح سرمی کلسترول تام (001/0>p)، تری گلیسیرید (024/0=p)، لیپو پروتئین با چگالی پایین (001/0>p) و مالون دی آلدئید (01/0=p) و افزایش معنی داری در سطح لیپو پروتئین با چگالی بالا (018/0=p) در انتهای مطالعه نسبت به مقادیر پایه شد. کاهش در سطح سرمی کلسترول تام، لیپو پروتئین با چگالی پایین و مالون دی آلدئید در گروه اسیدهای چرب امگا-3 نسبت به گروه دارونما معنی دار بود (به ترتیب 002/0=p=0/007، p=0/003 ،p). در هیچ یک از گروه ها تغییر معنی داری در ظرفیت تام آنتی اکسیدانی سرم مشاهده نشد (05/0<p).
نتیجه گیری: اسیدهای چرب امگا-3 اثرات مطلوبی بر سطوح لیپیدهای سرم و پراکسیداسیون لیپیدی در مبتلایان به سندرم تخمدان پلی کیستیک دارد.

Introduction: Polycystic ovary syndrome is the most common endocrine disorder among women in reproductive age. These women are high risk for early onset of cardiovascular diseases.The aim of this study was investigating the effects of omega-3 fatty acids supplement on serum lipid profile and markers of oxidative stress including malondialdehyde and serum total antioxidant capacity in polycystic ovary syndrome patients.
Methods: This double-blind randomized controlled clinical trial was conducted on 64 PCOS patients in year 2011. Subjects were divided randomly into two groups of omega-3 fatty acids (n=32) and placebo group (n=32). Case group were given 4 g omega-3 fatty acids daily for 8 weeks and placebo group took placebo for the same period. Anthropometric and biochemical measurements and dietary intake assessment were performed using 24-hour dietary recalls and Nutritionist software version 4 at onset and end of the study. Data were analyzed by independent T-test, paired T-test, analysis of covariance and SPSS software version 11.5. P value less than 0.05 was considered significant.
 
Results: Omega-3 fatty acids significantly decreased serum levels of total cholesterol (P<0.001), triglyceride (P=0.024), low density lipoprotein cholesterol (P<0.001) and malondialdehyde (P=0.01) and increased high density lipoprotein cholesterol (P=0.018) at the end of the study compared with baseline values. Decrease in serum levels of total cholesterol, low density lipoprotein cholesterol and malondialdehyde in omega-3 fatty acids group in comparison with placebo group was significant (P=0.002, P=0.003 and P=0.007, respectively). No significant differences were seen in serum total antioxidant capacity levels in two groups (p>0.05).
 
Conclusion: Omega-3 fatty acids had beneficial effects on serum lipid profile and lipid peroxidation in women with polycystic ovary syndrome.
