بررسی آخرین گایدلاین ها و مطالعات در مورد بهترین درمان با آنتی کواگولانت طی بارداری در زنان دارای دریچه قلب مصنوعی

چکیده
مقدمه: تمام زنان دارای دریچه مصنوعی باید به صورت طولانی مدت تحت درمان با داروهای آنتی کواگولانت قرار گیرند. درمان آنتی کواگولانتی مناسب قابل استفاده در دوران بارداری برای زنان دارای دریچه مصنوعی قلب با اختلاف نظرهای زیادی روبرو است. با توجه به اطلاعات اندک پیرامون استراتژی های قابل استفاده در برخورد با زنان باردار دارای دریچه مصنوعی، مطالعه حاضر با هدف بررسی مزایا و معایب استفاده از انواع آنتی کواگولانت ها در زنان باردار دارای دریچه مصنوعی جهت دستیابی به نکات کلیدی قابل استفاده برای پزشکان انجام شد.
روش کار: برای دسترسی به مطالعات انجام شده با موضوع آنتی کواگولانت تراپی در افراد باردار دارای دریچه مصنوعی از پایگاه های اطلاعاتی PUBMED، MEDLINE، SCOPUS و EMBASE و DARE و همچنین از کلید واژه های مرتبط با موضوع مطالعه استفاده شد. همچنین آخرین گایدلاین های مرتبط با آنتی کواگولانت تراپی پس از جراحی دریچه مصنوعی قلب مورد بررسی قرار گرفتند.
نتیجه گیری: بهترین رژیم آنتی کواگولاسیون در خانم های باردار با دریچه مکانیکال، استفاده از وارفارین با دوز زیر 5 میلی گرم است.

چکیده English

Introduction: All women with prosthetic heart valve should undergo long-term treatment with anticoagulants drugs. Anticoagulants suitable for use in the treatment of pregnant women with prosthetic heart valves has met with controversy. Due to limited information on the strategies used in the treatment of pregnant women with prosthetic heart valve, this study was performed with aim to evaluate the advantages and disadvantages of the use of anticoagulants in pregnant women with prosthetic valve to achieve key points for medical use.
Methods: To access the performed studies about anticoagulant therapy in pregnant women with prosthetic valve, databases of PUBMED، MEDLINE، SCOPUS و EMBASE و DARE and also, related keywords were used. Last guidelines related to the coagulant therapy after surgery of prosthetic heart valve were studied.
Conclusion: best coagulation regimen in pregnant women with mechanical valve is using warfarin with dose <5 mg.
