بررسی اثرات لپتین بر وزن گیری مادران باردار: مروری سیستماتیک

چکیده

مقدمه: بارداری وضعیتی است همراه با کاهش گرمازایی که منجر به افزایش توده چربی بدنی می شود. در ابتدای بارداری مسیرهای لیپوژنز غالب شده که به نظر می رسد این مسیر توسط هورمون هایی مانند لپتین به راه می افتند؛ در حالی که در انتهای آن، مسیرهای مولکولی لیپولیز فعال ترند. نقش کلیدی لپتین در حیوانات و انسان ها، تنظیم چربی بدنی است که سطح آن با افزایش سن بارداری بالا می رود، در سه ماهه دوم به بالاترین حد رسیده و تا زمان ترم، ثابت می ماند. این مطالعه مروری با هدف خلاصه بندی مطالعات مرتبط با لپتین و وزن گیری دوران بارداری انجام شد.
روش کار: در این مطالعه، 56 مقاله خلاصه و کامل از طریق جستجوی الکترونیکی با وارد کردن کلید واژه های مورد نظر در بانک های اطلاعاتی PubMed، Science Direct، Iran Medex، Yahoo، Google Scholar، Google و SID از بازه زمانی 1995 تا 2013 به دست آمد که در نهایت 22 مقاله کامل شامل 4 مقاله فارسی و 18 مقاله انگلیسی مورد نقد و بررسی قرار گرفتند.
یافته ها: مرور مقالات مورد بررسی، حاکی از ارتباط احتمالی قوی بین سطوح سرمی لپتین مادری و وزن گیری دوران بارداری آنان بود و این ارتباط در اکثر مطالعات، مستقل از شاخص توده بدنی، ارتباطی معنادار بود.
نتیجه گیری: اگرچه نتایج این مطالعه مروری، مؤید ارتباط قوی بین لپتین سرمی مادران و افزایش وزن ایشان در دوران بارداری بود اما به دلیل مطالعات متناقض در این زمینه، برای قضاوت قطعی تر، مطالعات بیشتری در آینده نیاز است

Introduction: Pregnancy is a condition associated with decreased thermogenesis leading to increased body fat mass. In early pregnancy, lypogenesis routes are dominant that seems it is by hormones such as Leptin, while at the end, lipolysis molecular routes are more active. The key role of Leptin in animals and humans is regulating body fat which its levels increases with increased gestational age, peaks in second trimester and stays platue untill term. This systematic review was performed with the aim to summarize the studies related to leptin and weight gain during pregnancy.
Methods: in this study, 56 abstract and full text articles were obtained from electronic research by entering the desired keywords in Sciencedirect, Pubmed, Googlescholar,Yahoo, Iranmedex and SID databases since 1995 to 2013 that finally 22 full text articles including 4 Persian and 18 English were reviewed and evaluated.
Results: The literature review suggests a strong possible association between maternal serum leptin levels and maternal weight gain during pregnancy, and this relation was a significant correlation in most studies independent of body mass index.
Conclusion: Although the results of this review article indicate a strong association between maternal serum leptin and their weight gain during pregnancy, but due to controversial studies in this field, further studies are needed for definite judgment
