بررسی تأثیر گیاهان دارویی در درمان گرگرفتگی دوران یائسگی به روش مرور سیستماتیک و متاآنالیز (مطالعات کارآزمایی بالینی) در ایران

چکیده

مقدمه: شایع ترین عارضه دوران یائسگی گرگرفتگی می باشد. امروزه استفاده از داروهای گیاهی در درمان عوارض یائسگی از جمله گرگرفتگی مورد توجه قرار گرفته است. مطالعه حاضر با هدف بررسی تأثیر گیاهان دارویی مورد استفاده در درمان گرگرفتگی انجام شد.
روش کار: مطالعه حاضر یک مطالعه مرور سیستماتیک و فرا تحلیل است. اطلاعات در مورد تأثیر داروهای گیاهی بر گرگرفتگی دوران یائسگی از پایگاه ‌های اطلاعاتی Google scholar، IranMedex، Magiran، MedLib، PubMed، SID و IranDoc  جستجو شد. در نهایت 16 مقاله در فاصله سال‌ های 1379تا 1391 در ایران انتخاب شد. داده ها با نرم افزارSTATA  (نسخه 12/2) و با استفاده از روش متاآنالیز، مدل اثرات تصادفی با هم ترکیب شدند.
یافته‌ها: از بین 16 مقاله، 15 مطالعه کاهش تعداد گرگرفتگی را مورد بررسی قرار داده بودند. در گروه گیاهان دارویی میانگین کاهش تعداد گرگرفتگی، /085- (فاصله اطمینان 95%: از 1/83- تا 0/12) کمتر از گروه شاهد بود که این میزان کاهش از نظر آماری معنی دار بود (0/05>p). شدت گرگرفتگی ها نیز کاهش چشمگیری داشته است؛ به گونه ای که شدت گرگرفتگی شدید 0/81- (فاصله اطمینان 95%: از 1/86- تا 0/23)( 0/126=p)و گرگرفتگی متوسط 0/85- (فاصله اطمینان 95%: از 1/83- تا 0/12) کمتر از گروه شاهد بود که از نظر آماری معنی دار بود (0/087=p).
نتیجه‌گیری: گیاهان دارویی حاوی فیتواستروژن ها، تأثیر بسیار خوبی بر کاهش تعداد دفعات و شدت گرگرفتگی زنان یائسه دارند و انتخاب مناسبی برای درمان گرگرفتگی زنان یائسه، به جای هورمون درمانی جایگزینی می باشند.

Introduction: The most common phenomenon of menopause period is hot Flashes. nowadays using herbaceous medicines in hot Flashes treatment has gained much attention. The aim of this study is to investigate the effect of herbaceous medicines in hot Flashes treatment.
Methods: Current study is a systematic and Meta-analysis review. Information about the effect of herbaceous medicines on hot Flashes in menopause period collected from Google scholar, IranMedex, Magiran, MedLib, PubMed, SID, IranDoc databases. 16 articles published between 2000 and 2009 worldwide were selected. Data were analysed by STATA software (version12.2) using Meta-analysis method (random selection model).
Results: of 16 articles, 15 one were evaluated the reduction of hot Flashes. In herbaceous medicines group, the average of reduction of hot Flashes was 0.85 (CI 95%: 1.83-0.12) lower than the control group which was significant, statistically. The intensity of hot Flashes also significantly reduced. The severe and medium hot Flashes was 0.81( CI 95%: 1.86-0.23) and 0.85 ( CI 95%: 1.83-0.12) lower than control group, respectively which was statistically significant.
Conclusion: Herbaceous Medicines contain phytoestrogens, have a positive effect on reducing the frequency and severity of hot Flashes and They are the best choise for the treatment of hot Flashes in menopausal women and in hormone replacement therapy.
