تجربه زیسته مادران از انتخاب زایمان سزارین: یک مطالعه پدیدار شناسی

چکیده

مقدمه: زایمان سزارین با درخواست مادر، به سزارین انتخابی برای بارداری ترم طبق درخواست مادر و علی رغم فقدان اندیکاسیون های مادری یا جنینی اطلاق می شود. میزان شیوع زایمان طبیعی و سزارین در زنان باردار یک کشور، یکی از شاخص های بررسی عملکرد برنامه های سلامتی مادران به شمار می رود و افزایش سزارین های غیر ضروری و کاهش زایمان طبیعی در بین زنان، حاکی از عملکرد نامناسب نظام سلامتی می باشد. مطالعه حاضر با هدف تبیین تجربه زیسته مادران نخست زا از انتخاب زایمان سزارین انجام شد.
روش کار: در این مطالعه کیفی با رویکرد پدیدارشناسی، 12 زن نولی پار با روش مصاحبه عمیق و نیمه سازمان یافته مورد بررسی قرار گرفتند. مصاحبه ها با توافق شرکت کنندگان در مراکز درمانی یا منزل آنان صورت گرفت و شرکت کنندگان دلایل انتخاب روش سزارین را 6 هفته پس از زایمان به بحث گذاشتند. تمام مصاحبه ها ضبط و کلمه به کلمه پیاده شد و با استفاده از روش کولیزی مورد تجزیه و تحلیل قرار گرفت.
یافته ها: در این مطالعه 3 درونمایه اصلی و چندین درون مایه فرعی استخراج شد. درونمایه های اصلی شامل: ترس، تجربه نامطلوب و تشویق دیگران بود.
نتیجه گیری: با توجه به نتایج این مطالعه و عوامل مؤثر در انتخاب سزارین الکتیو در مادران نخست زا، به نظر می رسد که بهترین راه کاهش میزان سزارین، کاهش سزارین در خانم های نخست زا است. بنابراین باید زنان نخست زا با ایجاد درک صحیح نسبت به مزایا، خطرات و عوارض احتمالی روش های زایمان، به سمت زایمان طبیعی سوق داده شوند.

چکیده [English]

Introduction: Cesarean delivery on maternal request is defined as an elective cesarean for term pregnancy in the absence of any maternal or fetal indication. Rate of vaginal delivery and cesarean section is one of the mother health program indexes. Increase of cesarean section and decrease of vaginal birth is representative of poor performance of the health program. The purpose of this study was explanation of nulliparous women's lived experience of cesarean delivery as a birth choice.
Methods: This qualitative study with phenomenology approach was conducted on 12 nulliparous women using in-depth interview method and semi-structured. Interviews were done in health care centers or women's home. The participants discussed reasons for the request for cesarean six weeks after childbirth. All interviews were recorded and analyzed by Colaizzi method.
Results: Three main themes and many sub-themes were extracted during this study. The main themes were: fear, unpleasant experience and others encouragement.
Conclusion: According to the results of this study and effective factors in choice of elective cesarean in nulliparous women, it seems the best way to reduce cesarean section rate is decreasing the rate of cesarean in nulliparous women. Therefore nulliparous women with knowledge about the benefits, risks and possible complications of delivery methods should encourage selecting vaginal delivery.
