چکیده
مقدمه: سندروم تخمدان پلیکیستیک یکی از شایعترین اختلالات غدد درونریز زنان است که با اختلال تخمکگذاری و تغییرات فولیکولی همراه بوده و کیفیت اووسیت و نتایج ART را تحت تأثیر قرار میدهد. شواهد نشان میدهد التهاب مزمن نقش مهمی در پاتوفیزیولوژی PCOS دارد و افزایش نشانگرهایی مانند CRP میتواند پیامدهای باروری را کاهش دهد. مطالعه حاضر با هدف ارزیابی تأثیر مکمل اینوزیتول بر سطح CRP و پیامدهای ART در زنان مبتلا به PCOS انجام شد.
روش کار: این کارآزمایی بالینی تصادفی در سالهای ۱۴۰۲-۱۴۰۳ روی ۱۱۴ زن مبتلا به PCOS کاندید IVF انجام شد. شرکتکنندگان واجد شرایط بر اساس معیار روتردام، سن ۱۸ تا ۴۰ سال و عدم وجود بیماریهای مزمن به دو گروه تقسیم شدند. گروه کنترل متفورمین و گروه مداخله متفورمین همراه با اینوزیتول را حداقل دو ماه پیش از IVF دریافت کردند. تخمکگیری، ارزیابی بلوغ اووسیتها، لقاح و سنجش کیفیت جنین طبق استاندارد انجام شد. سطح CRP قبل و روز برداشت تخمک اندازهگیری و دادهها با آزمونهای آماری مناسب تحلیل شدند. میزان p<0/05 معنی دار در نظر گرفته شد.
یافته ها: هر دو گروه کاهش معنادار CRP نشان دادند، اما کاهش در گروه اینوزیتول بیشتر بود. تعداد تخمکها تفاوتی نداشت، اما کیفیت آنها و نیز تعداد و کیفیت جنینها در گروه مداخله بهطور معناداری بالاتر بود. رگرسیون اثر مستقل اینوزیتول و کاهش CRP بر بهبود جنین را تأیید کرد.
نتیجه گیری: یافتهها نشان میدهد که ترکیب اینوزیتول و متفورمین یک استراتژی درمانی امیدوارکننده برای بهبود پارامترهای تولید مثلی در بیماران PCOS تحت IVF است.

Introduction: Polycystic ovary syndrome is one of the most common endocrine disorders in women, characterized by ovulatory dysfunction and follicular changes. It impacts oocyte quality and ART outcomes. Evidence suggests that chronic inflammation plays a significant role in PCOS pathophysiology, and elevated markers like CRP can reduce fertility outcomes. This study aimed to evaluate the effect of inositol supplementation on CRP levels and ART outcomes in women with PCOS.
Methods: A randomized clinical trial was conducted in 2023-2024 with 114 women diagnosed with PCOS who were candidates for IVF. Participants met the Rotterdam criteria, were aged 18-40, and had no chronic diseases. They were divided into two groups. The control group received metformin, while the intervention group received metformin plus inositol for at least two months prior to IVF. Oocyte retrieval, assessment of oocyte maturation, fertilization, and embryo quality were performed according to standard protocols. CRP levels were measured before the procedure and on the day of oocyte retrieval. Data were analyzed using appropriate statistical tests, with p< 0.05 considered significant.
Results: Both groups showed significant reduction in CRP, but the reduction was greater in the inositol group. The number of oocytes did not differ, but their quality, as well as the number and quality of embryos, were significantly higher in the intervention group. Regression confirmed the independent effect of inositol and CRP reduction on embryo improvement.
Conclusion: Findings indicate that the combination of inositol and metformin is a promising strategy for improving reproductive parameters in PCOS patients undergoing IVF.
