اثربخشی اوریکولوتراپی بر درد پس از زایمان واژینال و سزارین: مروری سیستماتیک بر کارآزمایی بالینی تصادفی شده همراه با گروه کنترل و مکانیسم‌های ضد درد

چکیده

مقدمه: در روزهای اولیه پس از زایمان، کنترل مناسب درد سبب نگرش مثبت به زایمان، مراقبت از فرزند و انجام فعالیت­ های روزمره می­شود. از آنجایی که درمان­ های شیمیایی عوارضی را برای مادر و نوزاد به همراه دارند، لذا مطالعه حاضر با هدف مروری بر کارآزمایی ­های بالینی انجام شده در زمینه تأثیر اوریکولوتراپی بر درد پس از زایمان انجام شد.
روش‌کار: تمامی مطالعات کارآزمایی بالینی انگلیسی زبان به‌منظور بررسی تأثیر اوریکولوتراپی بر درد پس از زایمان از سال 2000 تا آگوست 2024 در پایگاه­های اطلاعاتی PubMed، Web of Science، Scopus، Embase، Cochran Library و Google Scholar مورد جستجو قرار گرفتند. کلمات کلیدی شامل: درد پس از زایمان واژینال، درد پس از زایمان سزارین، تحریک گوش خارجی، اوریکولوتراپی، طب فشاری گوش و طب سوزنی گوش بود که با عملگرهای بولین OR و AND با هم ترکیب شدند. ارزیابی کیفیت مطالعات توسط ابزار معتبر و پایای مطالعات کارآزمایی بالینی مؤسسه جونا بریگز انجام شد و نتایج به‌صورت کیفی گزارش گردید.
یافته­ ها: بر اساس جستجوهای انجام شده 415 مطالعه یافته شد و 11 مطالعه با حجم نمونه 1362 شرکت کننده از ایران، چین، هند، تایلند، آمریکا و آلمان مطابق با هدف مطالعه انتخاب شدند. اکثر مطالعات کاهش معنادار شدت درد و مصرف مسکن را در گروه اوریکولوتراپی در مقایسه با گروه کنترل نشان داده بودند. همچنین در اکثر مطالعات، عارضه جانبی خاصی در این زمینه گزارش نشده بود.
نتیجه ­گیری: اوریکولوتراپی می‌تواند به‌عنوان جایگزین درمان­ های شیمیایی به‌صورت مؤثر، بی‌عارضه و دردسترس در جهت افزایش کیفیت زندگی در روزهای اول بعد از زایمان استفاده گردد.

چکیده [English]

Introduction: In the first days of postpartum period, effective pain management has been shown to positively impact mother's positive attitude toward delivery, child care, and daily activities. Since chemical treatments have side effects for mother and baby, the resent study was conducted with aim to review clinical trials conducted on the effect of auriculotherapy on postpartum pain.
Methods: To investigate the effect of auriculotherapy on postpartum pain, all English-language clinical trial studies were searched in PubMed, Web of Science, Scopus, Embase, Cochran Library and Google Scholar databases from 2000 to August 2024. The search terms encompassed the following keywords: vaginal postpartum pain, cesarean section postpartum pain, external ear stimulation, auriculotherapy, ear acupressure, and ear acupuncture, which were combined with the Boolean operators OR and AND. The quality of the studies was assessed using the Jonah Briggs Institute Clinical Trials Study Validation Tool, and results were reported qualitatively.
Results: According to the searches, 415 studies were found and 11 studies with a sample size of 1362 participants from Iran, China, India, Thailand, USA and Germany were selected according to the study objective. Most studies showed a significant reduction in pain intensity and analgesic consumption in the auriculotherapy group compared to the control group. Also, no specific side effects were reported in most studies.
Conclusion: Auriculotherapy can be used as an effective, uncomplicated, and accessible alternative to chemical treatments to enhance the quality of life in the initial days following delivery.
