تأثیر بازتاب‌شناسی دست بر پس‌درد زایمانی در زنان چندزا: کارآزمایی بالینی تصادفی شده

چکیده

مقدمه: پس­ درد زایمانی، یکی از عوارض شایع دوران نفاس است. امروزه تمایل ویژه ­ای نسبت به استفاده از درمان مکمل مشاهده می­ شود. مطالعه حاضر با هدف تعیین تأثیر بازتاب­ شناسی دست بر پس ­درد انجام شد.
روشکار: این مطالعه کارآزمایی بالینی تصادفی یک‌سوکور در سال 1403-1402 بر روی 99 واحد پژوهش پس از زایمان طبیعی در بخش بعد از زایمان بیمارستان­ های بنت‌الهدی بجنورد و جواد الائمه (ع) جاجرم انجام شد. افراد به‌طور تصادفی در سه گروه 33 نفره مداخله حقیقی، مداخله شم و مراقبت روتین قرار گرفتند. در دو گروه مداخله حقیقی و مداخله شم، مداخله­ توسط پژوهشگر در نواحی بازتابی قلبی، شبکه خورشیدی، هیپوفیز و رحمی دست­ ها انجام شد. گروه مراقبت روتین تنها مراقبت­ های روتین بعد از زایمان را دریافت کردند. گردآوری داده ­ها با استفاده از پرسشنامه اطلاعات دموگرافیک و مامایی، مقیاس اندازه ­گیری عددی درد و زمان‌سنج انجام شد. تجزیه و تحلیل داده­ ها با استفاده از نرم‌افزار آماری SPSS (نسخه 24) و آزمون­ های آماری آنالیز واریانس، کروسکال والیس، کای اسکوئر، دقیق فیشر و مدل برآورد تعمیم یافته انجام گرفت. میزان p کمتر از 05/0 معنادار در نظر گرفته شد.
یافته ­ها: در مقایسه درون‌گروهی، میانگین شدت پس­ درد در سه گروه روند کاهشی داشت (001/0p<). میانگین شدت پس­ درد در مداخله حقیقی نسبت به مراقبت روتین 58/3 واحد کمتر و در مداخله شم نسبت به مراقبت روتین 06/2 واحد کمتر (001/0p<) و در مداخله حقیقی نسبت به مداخله شم 52/1 واحد کمتر بود (001/0p<). مصرف مسکن در مداخله حقیقی نسبت به دو گروه دیگر کمتر بود (001/0=p).
نتیجه ­گیری: بازتاب­ شناسی دست با توجه به کاهش پس ­درد و عدم عارضه می­ تواند در کنار درمان­ های دارویی توصیه شود.

Introduction: Postpartum pain is one of the most common complications of the postpartum period. Today there is a special tendency towards the use of complementary therapy. The present study was conducted with aim to determine the effect of hand reflexology on postpartum pain.
Methods: This single-blind randomized clinical trial study was conducted in 2023 on 99 research units after natural childbirth in the postpartum ward of Bent Al-Hoda Hospital in Bojnourd and Javad Al-Aemmeh Hospital in Jajarm. The subjects were randomly assigned to  three groups of 33 people: real intervention, sham intervention, and routine care. In the real intervention and sham intervention groups, the intervention was performed by the researcher in the cardiac, solar plexus, pituitary, and uterine reflex areas of the hands. The routine care group only received routine postpartum care. Data was collected using a demographic and obstetric information questionnaire, a numerical numerical rating scale, and a timer. Data were analyzed by SPSS software (version 24) and statistical tests of analysis of variance, Kruskal-Wallis, Chi-square, Fisher's exact, and generalized estimating model. P<0.05 was considered statistically significant.
Results: In the intra-group comparison, the mean intensity of post-partum pain decreased in the three groups  (p<0.001). The mean intensity of post-partum pain in the real intervention was 3.58 units lower than in the routine care and in the sham intervention was 2.06 units lower than in the routine care (P<0.001) and in the real intervention was 1.52 units lower than in the sham intervention (P<0.001). The use of analgesics in the real intervention was lower than the other two groups (P=0.001).
Conclusion: Hand reflexology can be recommended alongside drug treatments due to reducing post-partum pain and absence of complications.
