Introduction: Breast hamartoma is a rare benign lesion often seen in women with a mean age of 41.8±10.8 years and a mean size of 3.9±2.7 cm. In this study, a case of giant breast hamartoma in a young woman is presented.
Case presentation: A 23-year-old female patient presented to a gynecologist with symptoms of a palpable mass, pain, and deformity in the left breast that had gradually increased in size over the past few years. The patient had no family history of the disease, no history of trauma, surgery, or radiation. Ultrasound examination revealed a distinct hypoechoic mass measuring 97×88×25 mm with lobular margins, and pathological examination of a needle biopsy performed at a non-university center revealed a preliminary diagnosis of fibroadenoma. Following surgery, a solid, cream-colored, encapsulated mass with well-defined borders and lobular margins without adhesion to adjacent structures, weighing 245 g and measuring 5×10×10 cm was completely removed. Histopathological examination revealed breast tissue with disorganized lobules and ducts in a PASH-like fibrotic stroma, consisting of intra-anastomosing spaces in a collagenized stroma. Immunohistochemical examination revealed that the cells lining the spaces were positive for CD34 and negative for CD31.
Conclusion: Given the nature of breast hamartoma, a definitive diagnosis may not be made initially following ultrasonography and pathological examination of a needle biopsy specimen. Therefore, a definitive diagnosis of breast hamartoma can be reached by histopathological examination of the resected tumor.
