Background: Uterine fibroids are masses that are mainly composed of uterine tissue. Their presence causes dysfunction of the reproductive system, which in many cases requires surgery. In this study, we compared the outcomes during and after myomectomy in women with and without uterine artery ligation.
Materials and Methods: Thirty patients aged 29 to 48 years with symptomatic uterine fibroids (intramural myomas) were evaluated and randomly assigned to either uterine artery ligation or non-ligation groups. The primary outcome measures were mean hemoglobin levels, mean blood loss at the time of surgery, as well as FSH indices, serum LH estradiol levels, and uterine perfusion parameters (pulsatility index and resistance index).
Results: The mean bleeding and hemoglobin drop in patients with uterine artery ligation was significantly lower than in women without ligation (p = 0.034 for hemoglobin and p = 0.001 for bleeding). The difference in serum levels of FSH, LH, and estradiol in the two groups before and after surgery and between the two groups was not significant (p>0.05). The pulse and resistance index of the two groups increased significantly after surgery, the difference between the two groups in terms of resistance index was significant three months after surgery (p<0.05), but the resistance and pulse index between the two groups did not show a significant difference at other times (p>0.05).
Conclusion: It was found that myomectomy ligation in women with fibroids reduces blood loss during surgery and reduces hemoglobin levels after surgery.
