تأثیر محلول جنتامایسین موضعی بر میزان ترمیم اپیزیوتومی: یک کارآزمایی بالینی تصادفی شاهد دار

چکیده

مقدمه: اپیزیوتومی، شایع ترین عمل جراحی در مامایی است که مانند هر زخم دیگری می تواند دچار عفونت یا تأخیر در ترمیم شود. تأخیر در ترمیم زخم اپیزیوتومی باعث افزایش خطر عفونت و نتایج بد آناتومیک شده و این عفونت می‌تواند منجر به بروز عوارض خطرناک و حتی مرگ مادر شود. هدف از مطالعه حاضر تعیین تأثیر محلول جنتامایسین موضعی بر میزان ترمیم زخم اپیزیوتومی بود.
روش‌کار: این مطالعه کارآزمایی بالینی تصادفی در سال 1392 بر روی 80 زن نخست زا در بیمارستان سینا شهر اهواز انجام شد. محل اپیزیوتومی قبل از ترمیم شدن، در گروه مورد (40 نفر) با محلول جنتامایسین رقیق شده و در گروه شاهد (40 نفر) با نرمال سالین شستشو داده شد. میزان التیام برش اپیزیوتومی در ساعت 12، روزهای 3 و 10 بعد از ترمیم با استفاده از مقیاس REEDA ارزیابی شد. در این مقیاس نمرات کمتر نشان دهنده ترمیم بهتر زخم می باشند. داده ها با استفاده از نرم افزار آماری SPSS (نسخه 5/11) و آزمون تی مستقل تجزیه و تحلیل شدند. میزانp کمتر از 05/0 معنی دار در نظر گرفته شد.
یافته‌ها: میانگین امتیاز ترمیم اپیزیوتومی در ساعت 12 (002/0=p) و روزهای 3 (019/0=p) و 10 (034/0=p) بعد از ترمیم اپیزیوتومی، در گروه مورد به طور معناداری کمتر از گروه شاهد بود که نشان دهنده ترمیم بهتر زخم در گروه مورد می باشد.
نتیجه‌گیری: محلول جنتامایسین موضعی در بهبود بهتر و سریع تر زخم اپیزیوتومی مؤثر است؛ بنابراین می توان از آن به عنوان تسریع کننده بهبود زخم اپیزیوتومی استفاده کرد.

Introduction: Episiotomy is the most common obstetrical surgery that like any other wound can be infected or cause delayed healing. Delay in episiotomy wound healing increases the risk of infection and poor anatomic results and this infection can lead to serious complications and even maternal death. The aim of this study was to determine the effect of local gentamicin solution on episiotomy healing.
Methods: This randomized clinical trial was carried out on 80 primiparous mothers in Ahvaz Sina hospital, Iran in 2013. Before repair, episiotomy site was irrigated with diluted gentamicin solution in case group (n=40) and normal saline in control group (n=40). Episiotomy incision healing was assessed using REEDA scale at 12 hour, 3 and 10 days after the repair. In this scale, lower scores indicate better wounds healing. Data were analyzed using SPSS software version 11.5 and independent t-test. P value less than 0.05 was considered significant.
Results: The average score of wound healing in case group was significantly lower than control group at 12 hour (p=0.002), 3 and 10 days (p=0.019, P=0.03, respectively) after delivery  that showed better wound healing in case group.
Conclusion: Gentamicin solution has a significant effect on better and faster healing of episiotomy wound; Hence, it can be used to accelerate episiotomy wound healing.
