بررسی نظام‌مند اثرات رژیم غذایی DASH بر پیامدهای متابولیک، استرس اکسیداتیو، فاکتورهای التهابی و هورمون‌های جنسی در بیماران مبتلا به سندرم تخمدان پلی‌کیستیک

چکیده

مقدمه: سندرم تخمدان پلی­ کیستیک، یک اختلال شایع غدد درون‌ریز است که بسیاری از زنان در سنین باروری را تحت تأثیر قرار می ­دهد. این مرور نظام­ مند با هدف تعیین اثرات رژیم غذایی Dietary Approaches to Stop Hypertension (DASH) بر پیامدهای متابولیک، استرس اکسیداتیو، فاکتورهای التهابی و هورمون­ های جنسی مبتلایان به این بیماری انجام شد.
روشکار: در این مرور نظام­مند، کلید واژه ­های مرتبط با سندرم تخمدان پلی­کیستیک، پروفایل لیپیدی، وضعیت گلیسمیک، فاکتورهای تن­سنجی، استرس اکسیداتیو، فاکتورهای التهابی، هورمون­ های جنسی و رویکردهای غذایی برای توقف فشار خون بالا، در پایگاه­ های اطلاعاتی Medline، Scopus، Embase، Science Direct، Magiran، ISD، ISC و Cochrane به زبان­ های فارسی و انگلیسی تا ژانویه 2024 مورد جستجو قرار گرفت.
یافته ­ها: چهار مقاله اصیل پژوهشی با طراحی کارآزمایی بالینی با یک دوره مداخله 12-8 هفته‌ای با رژیم DASH در مبتلایان به سندرم تخمدان پلی­کیستیک مورد بررسی قرار گرفت. یافته­ های این مطالعات نشان داد پیروی از رژیم DASH منجر به کاهش وزن، شاخص توده بدنی، دور کمر، دور باسن، سطوح سرمی تری‌گلیسیرید، لیپوپروتئین‌های بسیار کم‌چگال، مقاومت به انسولین، آندروستندیون، آنتی‌مولرین هورمون، شاخص آندروژن آزاد، مالون دی آلدئید و پروتئین واکنشی C با حساسیت بالا شده و منجر به افزایش حساسیت به انسولین، ظرفیت آنتی‌اکسیدانی کل پلاسما و سطوح سرمی گلوتاتیون تام، گلوبولین متصل به هورمون جنسی و اکسید نیتریک می­ گردد.
نتیجه­ گیری: پیروی از رژیم غذایی DASH با مزایای سلامتی مختلفی از جمله کاهش وزن، بهبود پروفایل لیپیدی، وضعیت آنتی‌اکسیدانی، هورمون ­های جنسی، حساسیت به انسولین و نشانگرهای استرس اکسیداتیو و التهاب در مبتلایان سندرم تخمدان پلی­کیستیک مرتبط است.

Introduction: Polycystic ovary syndrome (PCOS) is a common endocrine disorder affecting many women of reproductive age. This systematic review was performed with aim to determine the effects of the Dietary Approaches to Stop Hypertension (DASH) on metabolic outcomes, oxidative stress, inflammatory factors and sex hormones in patients with PCOS.
Methods: In this systematic review, the keywords related to PCOS, lipid profile, glycemic status, anthropometric factors, oxidative stress, inflammatory factors, sex hormones, and DASH were searched in databases including Medline, Scopus, Embase, Science Direct, Magiran, ISD, ISC, and Cochrane in both Persian and English languages until January 2024.
Results: Four original research articles with a clinical trial design and an intervention period of 8-12 weeks using the DASH in PCOS patients were analyzed. The findings of these studies showed that following the DASH led to weight loss, reduced body mass index, waist circumference, and hip circumference. It also resulted in lower serum levels of triglycerides, very low density lipoproteins, insulin resistance, androstenedione, antimullerin hormone, free androgen index, malondialdehyde, and high-sensitivity C-reactive protein. Additionally, it was associated with improved sensitivity to insulin, increased total plasma antioxidant capacity, higher levels of total glutathione, sex hormone-binding globulin, and nitric oxide in the blood.
Conclusion: Following the DASH is associated with various health benefits, including weight loss, improved lipid profile, antioxidant status, sex hormones, insulin sensitivity, and markers of oxidative stress and inflammation in PCOS patients.
