Introduction: Ovarian serous cystadenofibroma is an uncommon tumor in pregnancy. In this study, a pregnant woman with asymptomatic giant ovarian serous cystadenofibroma who underwent successful surgery was introduced.
Case presentation: The patient was a nulliparous 31-year-old woman (14 weeks), asymptomatic that a multilocular cyst with dimensions of 259×173 mm originating from the right ovary was reported in the routine pregnancy ultrasound. The levels of CA-125, CA 19-9 and HE4 were 72, 8.5 and 27.3 U/mL, respectively, and the Roma index was 2.7 U/mL. The patient underwent open abdominal surgery and after being sent to pathology, the diagnosis of ovarian serous cystadenofibroma was confirmed. Finally, the patient underwent caesarean section at 39 weeks and two days of gestation due to macrosomia and a full-term baby weighing 4300 gr was born.
Conclusion: In pregnant women with ovarian serous cystadenofibroma, mass surgery, considering the tumor size and the levels of tumor markers such as CA-125 can be an effective treatment with minimal complications, especially in young nulliparous women who are interested in preserving the uterus.
