اثرات رژیم غذایی کتوژنیک بر پیامدهای سندرم تخمدان پلی‌کیستیک: یک مرور نظام‌مند بر کارآزمایی‌های بالینی

چکیده

مقدمه: سندرم تخمدان پلی‌کیستیک، یکی از مهم‌ترین علل عدم تخمک‌گذاری و ناباروری در زنان بوده که با بیماری‌های مزمن از جمله چاقی، مقاومت به انسولین، دیابت، بیماری‌های قلبی - عروقی و کبد چرب در ارتباط است. مطالعه مرور نظام‌مند حاضر با هدف تعیین اثرات رژیم غذایی کتوژنیک بر پیامدهای سندرم تخمدان پلی‌کیستیک انجام شد.
روشکار: در این مرور نظام‌مند، پس از جستجوی کلمات کلیدی "سندرم تخمدان پلی‌کیستیک"، "رژیم غذایی کتوژنیک"، "رژیم غذایی کم کربوهیدرات" و "کارآزمایی‌های بالینی" به زبان­های فارسی و انگلیسی بدون محدودیت زمانی در پایگاه‌های اطلاعاتی Medline، PubMed، Science Direct، Magiran، ISD، ISC و Cochrane، مقالات مرتبط با هدف مطالعه مورد بررسی قرار گرفت.
یافتهها: در این مطالعه، 4 مقاله مورد بررسی قرار گرفت. نتایج نشان داد پیروی از رژیم غذایی کتوژنیک در افراد مبتلا به سندرم تخمدان پلی‌کیستیک می‌تواند با کاهش وزن و شاخص توده بدنی، کاهش فشار خون، کاهش کلسترول تام و تری‌گلیسیرید سرم منجر به کاهش خطر بیماری‌های قلبی - عروقی در این بیماران شود.
نتیجهگیری: پیروی از رژیم غذایی کتوژنیک در طولانی‌مدت می‌تواند سبب کاهش وزن، بهبود مقاومت به انسولین و اختلالات پروفایل چربی سرم در مبتلایان به سندرم تخمدان پلی‌کیستیک گردد. به‌منظور بررسی اثرات متابولیسم و اندوکرین رژیم غذایی کتوژنیک در بیماران مبتلا به سندرم تخمدان پلی‌کیستیک، انجام کارآزمایی‌های بالینی توصیه می‌شود.

Introduction: Polycystic ovary syndrome is a major cause of anovulation and infertility in women that is associated with chronic diseases such as obesity, insulin resistance, diabetes, cardiovascular disease, and fatty liver disease. This systematic review was performed aimed to determine the effects of ketogenic diet on the outcomes of polycystic ovary syndrome.
Methods: In this systematic review, the keywords of “polycystic ovary syndrome”, “ketogenic diet”, “low carbohydrate diet”, and “clinical trials” were searched in databases of Medline, PubMed, Science Direct, Magiran, ISD, ISC and Cochrane in English and Persian language without time restrictions. Then, the articles relevant to the study's aim were reviewed.
Results: In this research, 4 studies were examined. The results showed that following a ketogenic diet in patients with polycystic ovary syndrome may reduce the risk of cardiovascular diseases in these patients by lowering weight and body mass index, blood pressure, serum total cholesterol, and triglycerides.
Conclusion: Long-term adherence to a ketogenic diet may result in weight loss, improved insulin resistance, and serum lipid profile disorders in patients with polycystic ovary syndrome. Clinical trials are suggested to evaluate the effects of ketogenic diet on these metabolic and endocrine factors in PCOS women.
