تأثیر طب فشاری نقطه H7 دست بر الگوی غیرواکنشی تست بدون استرس جنین: یک کارآزمایی بالینی تصادفی‌سازی شده

چکیده

مقدمه: تست بدون استرس، پرکاربردترین روش اولیه برای ارزیابی سلامت جنین است. شرایط روحی و روانی مادر بر الگوی ضربان قلب جنین مؤثر است. در هنگام افزایش اضطراب مادر، حرکات جنین و تغییرپذیری ضربه‌به‌ضربه جنین کاهش می‌یابد. طب فشاری موجب ترشح انکفالین‌ها مانند سروتونین و اندورفین شده و باعث کاهش سطح استرس و اضطراب مادر می‌شود، لذا مطالعه حاضر با هدف تعیین تأثیر طب فشاری نقطه H7 دست بر الگوی ضربان قلب جنین در تست بدون استرس انجام شد.
روش‌کار: این مطالعه کارآزمایی بالینی تصادفی شده در سال 1400-1399 با مشارکت 60 مادر نخست‌زا با سن حاملگی 42-37 هفته در بیمارستان ولیعصر (عج) بیرجند انجام گرفت. افراد به‌صورت تصادفی در دو گروه مداخله و طب فشاری کاذب قرار گرفتند. در گروه مداخله، طب فشاری نقطه H7 در هر دست به‌مدت 5/2 دقیقه و در گروه کنترل نیز نقطه‌ای کاذب در دست لمس می‌شد. تست بدون استرس به‌مدت 20 دقیقه قبل و بعد از مداخله انجام گرفت. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار آماری SPSS (نسخه 16) و آزمون‌های آماری کای اسکوئر، تی مستقل، تی زوجی، من‌ویتنی و ویلکاکسون انجام شد. میزان p‌ کمتر از 05/0 معنی‌دار در نظر گرفته شد.
یافته‌ها: بعد از مداخله متغیرهای تعداد موارد واکنشی تست بدون استرس، تعداد افزایش ضربان قلب جنین، تعداد حرکات جنین و تغییر‌پذیری ضربه‌به‌ضربه در گروه مداخله نسبت به گروه کنترل و همچنین نسبت به قبل از مداخله در گروه مداخله اختلاف معنی‌داری داشت (001/0p<)، اما میانگین متغیر ضربان پایه قلب جنین در دو گروه بعد از مداخله نسبت به قبل اختلاف معنی‌داری نداشت (732/0=p).
نتیجه‌گیری: طب فشاری می‌تواند تست بدون استرس جنین و پارامترهای آن را بهبود بخشد.

چکیده [English]

Introduction: Non-stress test is the most widely used primary method for assessing fetal health. The mother's mental state affects the fetal heart rate pattern. As maternal anxiety increases, fetal movements and beat to beat variability decrease. Acupressure releases enkephalins such as serotonin and endorphins and reduces maternal stress and anxiety levels. Therefore, this study was performed aimed to determine the effect of H7 point acupressure on fetal heart rate pattern in stress-free test.
Methods: This randomized clinical trial study was performed in 2020-2021 on 60 nulliparous mothers with gestational age of 37-42 weeks in Birjand Valiasr Hospital. The subjects were randomly assigned to two groups of intervention and pseudoacupressure. In the intervention group, point H7 acupressure was touched in each hand for 2.5 minutes and in the control group, a false point in the hand was touched. Non-stress test was performed for 20 minutes before and after the intervention. Data were analyzed using SPSS software (version 16) and Chi-square, Independent t-test, paired t-test, Mann-Whitney and Wilcoxon tests. P<0.05 was considered statistically significant.
Results: After the intervention, the variables of number of non-stress test cases, number of fetal heart rate increase, number of fetal movements, and beat to beat variability were significantly different in the intervention group compared to the control group and also compared to before the intervention (p<0.001). But the mean variable of fetal heart rate in the two groups after the intervention was not significantly different than before the intervention (P = 0.732).
Conclusion: Acupressure can improve fetal non-stress test and its parameters.
