تروما در بارداری؛ معرفی چند مورد و مروری بر مطالعات پیشین

چکیده

مقدمه: تروما در طول حاملگی، یک علت مهم بستری و مرگ‌و‌میر مادر و جنین می‌باشد. برخی از این عوارض شامل: مرگ مادر یا مرگ جنین داخل رحمی، شوک، پارگی احشاء داخل شکمی و حتی پارگی رحم، جداشدگی جفت و آسیب مستقیم جنین می­باشد. مطالعه حاضر با هدف معرفی چند مورد تروما در زنان باردار و بررسی برخی مطالعات منتشر شده قبلی در زمینه ترومای بارداری انجام شد.
معرفی بیمار: در این مطالعه هفت زن باردار در سنین مختلف بارداری که به تعدادی از مراکز درمانی آکادمیک شهر مشهد مراجعه کرده بودند، شامل ترومای بلانت شکم، ترومای ناشی از سوختگی و ترومای متعدد و آسیب مغزی معرفی می­شوند.
نتیجه‌گیری:اولویت در مدیریت بیمار باردار دچار تروما، پایدارسازی وضعیت مادر می­باشد و در بسیاری موارد، نتیجه نهایی وضعیت جنین با احیاء کامل و سریع مادر مرتبط است. در صورتی‌که جنین قابلیت حیات دارد، انجام پایش جنین امری ضروری است. از سوی دیگر با توجه به آمار بالای حوادث در دوران بارداری و عوارض آن، با آموزش به مادران و خانواده‌های ایشان و آگاه ساختن از عوارض جسمی و نیز جلوگیری از قرارگیری مادران در شرایط پرخطری همچون تردد با موتورسیکلت یا عدم استفاده از کمربند ایمنی مناسب می‌تواند تا حدی از وقوع سوانح و بالطبع عوارض ناشی از آن به‌خصوص مرگ مادر پیشگیری نمود.

Introduction: Trauma in pregnancy is a major cause of hospital admission and maternal and fetal mortality. Some of the main complications of trauma in pregnant women are intrauterine death, shock, placental abruption, intraperitoneal hemorrhage, and direct fetal injury. The present study aimed to report some of the cases of trauma in pregnancy and review the previous studies in this regard.
Case Presentation: In this case series, we presented the case of four pregnant women with trauma, who referred to various teaching hospitals in Mashhad, Iran. The subjects had blunt abdominal trauma, burn injuries, multiple trauma, and traumatic brain injury.
Conclusion: Stabilizing the mother is the primary goal in the management of traumatized, pregnant patients. In many cases, fetal outcome is directly correlated with the rapid, thorough maternal resuscitation. In viable fetuses, fetal monitoring is crucial. On the other hand, due to the high rate of complications during pregnancy, educational interventions should be considered for pregnant women and their families. Furthermore, pregnant women must be aware of risky conditions, such as motorcycle riding and not using seatbelts.
