Background and Aim: The Pro12Ala polymorphism of the PPARγ gene influences insulin sensitivity; however, its distribution in Iranian patients with Polycystic Ovary Syndrome (PCOS) based on diabetic status has been less explored. This study aimed to determine the frequency of Pro12Ala genotypes in diabetic and non-diabetic PCOS patients in Mashhad.
Materials and Methods: This cross-sectional study was conducted on 200 women with PCOS (based on Rotterdam criteria). Demographic data, including age and BMI, were recorded for all participants. Patients were divided into two groups: diabetic (n=100) and non-diabetic (n=100). Genotyping was performed using the PCR-RFLP method, and biochemical markers including FBG, HbA1c, and HOMA-IR were evaluated.
Results: The mean age in the diabetic group was significantly higher than in the non-diabetic group (31.4 ± 5.2 vs. 28.7 ± 4.8 years). Additionally, the mean BMI was significantly higher in the diabetic group (30.2 ± 3.5 vs. 26.8 ± 4.1 kg/m²). The frequency of the Ala allele in the diabetic group (4.5%) was significantly lower than in the non-diabetic group (11%) (P < 0.05). Ala allele carriers exhibited lower HOMA-IR scores compared to Pro/Pro homozygotes.
Conclusion: The Pro12Ala polymorphism has a lower prevalence in diabetic PCOS patients. It appears that the Ala allele exerts a protective effect against type 2 diabetes in women with PCOS.
