Introduction: Blood pressure disorders during pregnancy and gestational diabetes are among the common problems during pregnancy with negative effects on maternal and fetal health. The acidic load of the diet can play an important role in the occurrence of these complications. The present study was conducted to determine the effect of an alkaline diet on blood pressure disorders during pregnancy and gestational diabetes.
Methods: In this systematic review study, the keywords "Dietary Acid Load", "Acidity", "Alkaline diet", "Gestational Diabetes" and "Hypertensive disorders" were searched in international databases: PubMed, Web of Science, Scopus, Embase and national databases: SID and Magiran, in Persian and English until July 2025. The selection of articles was based on the inclusion and exclusion criteria and the quality of the articles was assessed using the Newcastle-Ottawa scale. 5 relevant studies were selected and reviewed with a sample size of 7869 pregnant and non-pregnant women. The data were mainly extracted and calculated from the semi-quantitative food frequency questionnaire and acid load indices such as NEAP and PRAL.
Findings: The results of most studies showed that high dietary acid load is associated with a significant increase in the risk of hypertensive disorders and gestational diabetes.
Conclusion: Despite limited studies, adjusting the acid load of the diet by increasing the consumption of fruits and vegetables and reducing the consumption of animal protein can help reduce blood pressure disorders and gestational diabetes. In this regard, prospective studies and clinical trials are recommended to more accurately confirm this relationship.
