تأثیر تمرین هوازی و مصرف کورکومین بر علائم بالینی و سطوح سرمی هورمون‌های جنسی در زنان 35-18 سال مبتلا به سندرم پیش از قاعدگی: کارآزمایی بالینی تصادفی شده

چکیده

مقدمه: سندرم پیش از قاعدگی (PMS)در فاز لوتئال سیکل قاعدگی رخ داده و از شدت کافی برای تداخل با جنبه‌های معمول زندگی برخوردار می‌باشد. استفاده از تمرین هوازی و داروهای گیاهی، از راه‌های مؤثر برای درمان PMS می‌باشند. مطالعه حاضر با هدف بررسی تأثیر ورزش هوازی و مصرف کورکومین بر ادم و سطوح هورمون‌های استروژن و پروژسترون در افراد مبتلا به PMS انجام شد.
روش‌کار: این مطالعه کارآزمایی بالینی تصادفی شده دوسوکور در سال 1395 بر روی 40 نفر از زنان مبتلا به سندرم پیش از قاعدگی در تبریز انجام شد. افراد به 4 گروه تمرین، کورکومین، تمرین + کورکومین و دارونما تقسیم شدند. گروه‌های تمرینی به مدت 8 هفته و هفته‌ای 3 جلسه به انجام تمرینات ایروبیک پرداختند و سایر گروه‌ها از 7 روز مانده به عادت ماهانه تا 3 روز اول، روزانه 2 عدد کپسول حاوی 100 میلی‌گرم کورکومین و یا دارونما مصرف کردند. تشخیص سندرم به‌صورت آینده‌نگر و با استفاده از پرسشنامه دیکرسون صورت گرفت. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار آماری SPSS‌ (نسخه 21) و آزمون­های آماریتوصیفیواستنباطی انجام شد. میزان p کمتر از 05‏/0 معنی‌دار در نظر گرفته شد.
یافته‌ها: تمرین، کورکومین و ترکیب آن دو، اثر معنی‌داری بر ادم ناشی از سندرم پیش از قاعدگی نداشتند (05‏/0<p)، اما موجب کاهش معنی‌دار در علائم جسمانی ناشی از سندرم پیش از قاعدگی شدند (05‏/0>p). بهبود علائم رفتاری ناشی از سندرم پیش از قاعدگی، تنها در گروه تمرین و گروه تمرین به همراه کورکومین مشاهده شد (2/0=p). علائم خلقی سندرم پیش از قاعدگی در هر سه گروه مورد مطالعه نسبت به قبل و پس از مداخله بهبود نشان داد (05‏/0>p). همچنین هیچ‌کدام از مداخلات تغییر معنی‌داری را در میزان پروژسترون سرم ایجاد نکردند(05‏/0<p)، ولی میزان استروژن سرم در هر 3 گروه کاهش معنی‌داری یافت(05‏/0>p).
نتیجه‌گیری: تمرین هوازی و مکمل کورکومین هر دو بر علائم و هورمون‌های سندرم پیش از قاعدگی تأثیر‌گذار می‌باشد، ولی نشانگان سندرم پیش از قاعدگی به یک اندازه تحت تأثیر این دو مداخله قرار نمی‌گیرند. لذا توصیه می­شود جهت کسب نتیجه بهتر از ترکیب تمرین هوازی و کورکومین در زنان مبتلا به سندرم پیش از قاعدگی استفاده گردد.

Introduction: Premenstrual syndrome (PMS) occurs in the luteal phase of menstrual cycle and is severe enough to interfere with the usual aspects of the life. Aerobic exercise and herbal drugs are effective to treat PMS. This study was performed with aim to evaluate the effect of aerobic exercise and consumption of curcumin on edema, estrogen and progesterone levels in patients with PMS.
Methods: This double-blind randomized clinical trial was performed on 40 women with PMS at Tabriz in 2016. The subjects were divided into 4 groups: exercise, curcumin, training + curcumin and placebo. The training groups performed aerobic exercise for 8 weeks, 3 times in a week, and the other groups daily consumed 2 capsules containing 100 mg of curcumin or placebo from 7 days before the menstrual period up to 3 first days. Diagnosis of the syndrome was done prospectively by using the Dickerson questionnaire. Data were analyzed using SPSS software (version 21) and descriptive and inferential statistics. P<0.05 was considered significant.
Results: Training, curcumin and their combination had no significant effect on the edema caused by PMS, but significantly reduced physical symptoms caused by PMS (P<0.05). Improvement of behavioral symptoms of PMS was seen only in exercise group and exercise+ curcumin group (P = 0.2). PMS mood symptoms in all three groups improved compared to before and after the intervention (P<0.05). Also, none of the interventions caused significant changes in serum levels of progesterone (P>0.05). However, estrogen levels significantly decreased in all 3 groups (P<0.05).
Conclusion: Both aerobic exercise and curcumin are effective in improving the symptoms and hormones of PMS, but PMS syndrome is not equally affected by these two interventions. So, it is recommended that for obtaining best results, a combination of aerobic exercise and curcumin be used in women with PMS.
