بررسی ارتباط درد بعد از سزارین با اضطراب قبل و بعد از جراحی

چکیده

مقدمه: شدت درد و اضطراب در همه افراد یکسان نیست، به عبارتی تجربه درد در افراد تحت تأثیر تجارب گذشته درد، اضطراب و انتظارات بیمار از تسکین درد می‌باشد. لذا مطالعه حاضر با هدف بررسی ارتباط بین درد بعد از سزارین با اضطراب قبل و بعد از جراحی انجام شد.
روش‌کار: این مطالعه همبستگی در سال 1394 بر 101 زن مراجعه‌کننده به بیمارستان ام‌البنین (س) مشهد که تحت سزارین انتخابی قرار گرفتند، انجام شد. افراد با روش نمونه‌گیری آسان انتخاب شدند. جهت سنجش اضطراب قبل از عمل، 3-2 ساعت قبل از جراحی، بیماران پرسشنامه اضطراب اشپیل‌برگر را تکمیل کردند. 5/2-5/1 ساعت پس از جراحی، اضطراب با پرسشنامه اشپیل‌برگر و درد، با پرسشنامه کوتاه شده درد مک‌گیل و مقیاس دیداری درد ارزیابی شد. داده‌ها پس از گردآوری با استفاده از نرم‌افزار آماری SPSS (نسخه 16) و آزمون‌های کای‌اسکوئر و همبستگی اسپیرمن و پیرسون با کنترل زمان، مورد تجزیه‌ و تحلیل قرار گرفت. میزان p کمتر از 05/0 معنی‌دار در نظر گرفته شد.
یافته‌ها: از 101 نفر مورد مطالعه، 67 نفر (4/66%) دارای اضطراب آشکار قبل از عمل متوسط و 64 نفر (4/63%) دارای اضطراب پنهان متوسط بودند و پس از عمل 53 نفر (5/52%) از بیماران درد متوسط و 78 نفر (2/77%) اضطراب آشکار متوسط داشتند. بین اضطراب آشکار (91/0=p، 01/0-=r) و پنهان (86/0=p، 018/0=r) قبل از عمل با درد پس از عمل ارتباط معناداری مشاهده نشد؛ اما بین اضطراب آشکار بعد از عمل و شدت درد ارتباط آماری معناداری وجود داشت (001/0=p، 32/0=r).
نتیجه‌گیری: بین اضطراب پس از سزارین با درد ارتباط وجود دارد؛ بنابراین برای کاهش درد بیمار بهتر است راهکارهای کاهش اضطراب نیز مورد توجه قرار گیرد.

چکیده [English]

Introduction: The intensity of anxiety and pain is not the same in all people. In other words, pain experience in patients is affected by past experiences of pain, anxiety and expectation of pain relief. So, this study was performed with aim to investigate the relationship between postoperative pain after cesarean section with pre and postoperative anxiety.
Methods: This correlation study was conducted on 101 pregnant women referred to Mashhad Omolbanin hospital who underwent elective caesarean section in 2015. Sampling was performed by accessible method. To assess preoperative anxiety, the patients completed Spielberger questionnaire2-3 hours before surgery. 1.5-2.5 hours after surgery, anxiety was evaluated by Spielberger questionnaire and pain was measured by short form of Mcgill pain questionnaire and visual analogue scale. Data were analyzed by SPSS software (version 16), Chi-square, Spearman and Pearson correlation tests by controlling the time. PResults: from 101 studied cases, 67 patients)66.4%( had moderate state preoperative anxiety and 64 (63.4%) moderate trait preoperative anxiety and 78 (77.2%) moderate  state postoperative anxiety and 53 (52.5%) had moderate postoperative pain. There was no significant correlation between preoperative state anxiety (r=-0.01 ,P=0.91) and trait anxiety (r=0.018 , P=0.86) with postoperative pain. But, significant relationship was observed between postoperative state anxiety and postoperative pain (r=0.32 ,P=0.001).
Conclusion: There is a significant relationship between postoperative anxiety and postoperative pain in cesarean section. So, it is better to consider the methods of reducing anxiety to decrease the patient's pain.
