بررسی تأثیر رایحه اسطوخودوس با و بدون موسیقی بر زمان واکنشی شدن آزمون بدون استرس و ضربان پایه قلب جنین در زنان باردار نخست‌زا: یک کارآزمایی بالینی تصادفی شده

چکیده

مقدمه: زمان واکنشی شدن در آزمون بدون استرس کمتر در مطالعات بررسی شده است. مطالعه حاضر با هدف بررسی تأثیر رایحه اسطوخودوس با و بدون موسیقی بر زمان واکنشی شدن آزمون بدون استرس و ضربان پایه قلب جنین در زنان نخست‌زا انجام شد.
روشکار: این مطالعه کارآزمایی بالینی تصادفی شده سه‌گروهه در سال 1400-1399 بر روی 195 زن باردار نخست‌زا که به درمانگاه پره‌ناتال مرکز آموزشی درمانی شهید اکبرآبادی تهران مراجعه ­کردند، انجام شد. افراد به‌ طور تصادفی در سه گروه 65 نفره رایحه اسطوخودوس، رایحه اسطوخودوس- موسیقی و کنترل قرار گرفتند. در گروه رایحه اسطوخودوس 4 قطره اسانس اسطوخودوس 10% استنشاق می­ شد. در گروه رایحه اسطوخودوس- موسیقی شرکت‌کنندگان به موسیقی (صداهای طبیعت) نیز گوش می­ دادند. پس از مداخله، آزمون بدون استرس در هر سه گروه به‌مدت 20 دقیقه انجام شد. زمان واکنشی شدن آزمون بدون استرس و تعداد ضربان پایه قلب جنین به‌عنوان پیامد اولیه در نظر گرفته شد. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار آماری SPSS (نسخه 16) و آزمون‌های آنالیز واریانس یک‌طرفه، تعقیبی توکی، تی زوجی، کای ­اسکوئر و آزمون دقیق فیشر انجام شد. میزان p کمتر از 05/0 معنی‌دار در نظر گرفته شد.
یافته­ها: میانگین زمان شروع آزمون تا واکنشی شدن آزمون بدون استرس جنین در دو گروه مداخله ی رایحه اسطوخودوس (11/4±05/7 دقیقه) و ترکیب رایحه اسطوخودوس- موسیقی (59/3±04/6 دقیقه) کمتر از گروه کنترل (32/5±43/10 دقیقه) بود. نتایج آزمون در 64 نفر (5/98%) در هر یک از گروه­های مداخله و 52 نفر (80%) در گروه کنترل واکنشی بود (001/0p<). دو گروه مداخله از نظر زمان واکنشی شدن تفاوت معنی‌داری نداشتند (144/0=p). در طی انجام آزمون، سه گروه از نظر میانگین ضربان پایه قلب جنین تفاوت آماری معنی‌داری نداشتند (729/0=p).
نتیجه ­گیری: ترکیب رایحه اسطوخودوس و موسیقی بر کوتاه شدن زمان واکنشی شدن آزمون در مقایسه با رایحه اسطوخودوس به‌تنهایی تأثیر بیشتری نداشت، لذا استفاده از رایحه اسطوخودوس برای کاهش زمان واکنشی شدن آزمون پیشنهاد می­ گردد.

Introduction: The reactivity time of the non-stress test (NST) has been less investigated in the studies. The present study was performed with aim to investigate the effect of lavender aromatherapy with and without music on the reactivity time of NST and basal fetal heart rate in primiparous women.
Methods: This three-group randomized clinical trial study was conducted between 2020-2021 on 195 primiparous women who referred to the prenatal clinic of Shahid Akbar Abadi educational and medical center in Tehran. The participants were randomly allocated into three groups: lavender aromatherapy, lavender aromatherapy-music, and control (n=65 in each group). In the lavender aromatherapy group, four drops of 10% lavender essence was inhaled. In the lavender aromatherapy-music group, participants also listened to music (nature sounds). After the intervention, NST was performed in all three groups for 20 minutes. The reactivity time of the NST and the number of basal fetal heart rate were considered as primary outcomes. The data were analyzed by statistical software (version 16), and one-way analysis of variance, Tukey's post hoc, paired t-test, chi-square, and Fisher's exact tests. P<0.05 was considered statistically significant.
Results: The mean reactivity time of the NST in the two intervention groups: lavender aromatherapy (7.05±4.11 minutes) and the combination of lavender aromatherapy and music therapy (6.04±3.59 minutes) was lower than the control group (10.43±5.32 minutes). The NST results were reactive in 64 (98.5%) of the cases in each of the intervention groups and 52 (80%) in the control group (P<0.001). There was no significant statistical difference between the two intervention groups in reactivity time of the NST (p= 0.144). During the NST, the three groups did not have a statistically significant difference in the mean basal fetal heart (p= 0.729).
Conclusion: The combination of lavender aromatherapy and music did not have a more effect on shortening the reactivity time of the NST when compared to lavender oil use alone. Therefore, the use of lavender aromatherapy is suggested to reduce the reactivity time of the NST.
