تأثیر تجویز وریدی اندوانسترون در پیشگیری از لرز ناشی از بیهوشی در جراحیهای زنان: کارآزمایی بالینی تصادفی شده

چکیده

مقدمه: با توجه به ایمنی اندوانسترون در مقایسه با سایر داروهای ضد لرز، عوارض کمتر آن و همچنین نتایج متناقض استفاده از این دارو در پیشگیری از لرز پس از جراحی، مطالعه حاضر با هدف بررسی تأثیر تجویز وریدی اندوانسترون قبل از عمل در پیشگیری از لرز ناشی از بیهوشی در اعمال جراحی زنان انجام شد.
روش‌کار: این مطالعه کارآزمایی بالینی تصادفی شده در سال 1398 با مشارکت 100 زن در بیمارستان الزهرا (تبریز) انجام شد. بیماران به‌صورت تصادفی در دو گروه تقسیم شدند و به گروه مداخله 2 میلی‌لیتر (8 میلی‌گرم) اندوانسترون و به گروه کنترل 2 میلی‌لیتر نرمال سالین تزریق شد. شدت لرز هر بیمار بر اساس معیار تعیین شدت لرز (LAS) ثبت شد. تجزیه و تحلیل داده­ها با استفاده از نرم‌افزار آماری SPSS (نسخه 22) و آزمون خی دو انجام شد. میزان p کمتر از 05/0 معنی‌دار در نظر گرفته شد.
یافته‌ها: در مقایسه تعداد افراد مبتلا به شدت لرز پس از بیهوشی، 6 نفر (12%) از گروه انداونسترون و 21 نفر (42%) از گروه کنترل دارای شدت لرز بیشتر از 3 بودند که دو گروه اختلاف آماری معناداری داشتند (001/0=p).
نتیجه‌گیری: اندوانسترون به‌عنوان یکی از آنتاگونیست‌های 5-HT3 می­تواند در پیشگیری از لرز پس از بیهوشی عمومی در جراحی­های زنان مؤثر واقع شود، اما نمی‌تواند لرز را به‌صورت کامل مهار نماید.

Introduction: Regarding the safety of Ondansetronecompared to other anti-shivering drugs, its lower side effects and the contradictory results of its use in the prevention of postoperative shivering, this study was performed with aim to evaluate the effect of preoperative intravenous administration of Ondansetroneon prevention of anesthesia-induced shivering in gynecological surgeries.
Methods: This randomized clinical trial study was conducted in 2019 with the participation of 100 women in Alzahra Hospital-Tabriz. Patients were randomly divided into two groups. The intervention group received 2 ml (8 mg) of ondansetroneand the control group received 2 ml of normal saline. The shivering intensity in each patient was recorded according to the shiver severity criterion (LAS). Data were analyzed by SPSS software (version 22) and Chi-square test. P <0.05 was considered statistically significant.
Results: Comparison of the number of cases with shivering after anesthesia indicated that 6 (12%) of the ondansetronegroup and 21 (42%) of the control group had shivering intensity >3 that two groups had statistically significant difference (p= 0.001)
Conclusion: Ondansetrone, as one of the 5-HT3 antagonists, can be effective in preventing shivering after general anesthesia in gynecological surgeries, but it cannot completely inhibit shivering.
