A Case Report of Cervical Pregnancy at Term

Abstract

Introduction: Ectopic cervical pregnancy is a rare clinical condition and the most common symptom is vaginal bleeding that has differential diagnosis with placenta previa. Transvaginal sonography is helpful in diagnosis and three-dimensional ultrasound and Magnetic resonance imaging (MRI) help in confirming the diagnosis.
Case report: A 25 year old primigravid woman with painless vaginal bleeding during the first trimester and an ultrasound assessment in 14th week of pregnancy that showed complete placenta previa and repeated vaginal bleeding in second and third trimester underwent cesarean delivery in 37th week of pregnancy due to beginning of labor pain. During cesarean section it was perceived that placenta was completely located in lower segment of uterus from anterior to posterior wall and fundal height was about 10th week of pregnancy. Hysterectomy was performed because of profuse and uncontrollable bleeding. After two weeks, the patient was discharged without any complications. The pathology report revealed complete implantation of placenta into the cervical canal (cervical pregnancy).
Conclusion: The most gestational age of cervical pregnancy which was reported was 14 weeks, while our patient was the first case that her pregnancy continued to 37 weeks. According to the history and performed sonographies, placenta previa was diagnosed for our patient. So, it is strongly recommended that all pregnant women who experience vaginal bleeding and placenta previa is reported in their ultrasound, they should be evaluated regarding cervical pregnancy through three-dimensional sonography and MRI.
