مقایسه میزان باروری با سه روش درمانی درسندرم تخمدان پلی کیستیک

چکیده

مقدمه: ناباروری مشکلی است که10-15% زنان مبتلا به آن می باشند و شایعترین منشا زنانه آن اختلالات تخمک گذاری است (35-40%)، از میان اختلالات تخمک گذاری تخمدانهای پلی کیستیک شایعترین علت می باشد. از آنجا که این افراد دچار مقاومت به انسولین و چاقی و افزایش آندروژنها هستند، مطالعات بسیاری دال بر تاثیر داروهای حساس کننده به انسولین مانند متفورمین در بیماران PCOS انجام شده است. این مطالعه با هدف مقایسه تاثیر سه روش درمان با کلومیفن سیترات (CC)، متفورمین و کلومیفن سیترات به علاوه متفورمین، در بیماران نابارور مبتلا به PCOS طراحی شده است.
روش کار: مطالعه حاضر یک مطالعه کارآزمایی بالینی است که بر 60 نفر بیمار مبتلا به PCOS مراجعه کننده به مرکز ناباروری منتصریه وابسته به دانشگاه مشهد در سال 1383 به مدت یکسال انجام شده است. زنان مورد مطالعه به طور تصادفی به 3 گروه 20 نفره تقسیم شدند.
گروه اول فقط کلومیفن سیترات، گروه دوم فقط متفورمین و گروه سوم کلومیفن سیترات به علاوه متفورمین، در یک دوره درمانی سه ماهه دریافت کردند. مشخصات فردی و نتایج درمانی در پرسشنامه جمع آوری گردید. اطلاعات به دست آمده با استفاده از نرم افزارکامپیوتری SPSS و آزمون های آماری T و کای دو مورد تجزیه وتحلیل آماری قرار گرفت و آمار توصیفی 05/0 £ p به عنوان سطح معنی داری قابل ملاحظه در نظر گرفته شد.
نتایج: از تعداد60 زن نابارور مورد مطالعه در این بررسی 29نفر (3/48%) باردار شده اند. بیشترین تعداد بارداری در گروه درمان توام کلومیفن سیترات و متفورمین بوده است (70%) که با دو روش دیگر تفاوت معنی داری دارد (028/0 =p). در بیماران با تیتر تستوسترون بالای سرم و هیرسوتیسم این نتیجه بارزتر می باشد.
نتیجه گیری: ترکیب متفورمین + کلومیفن سیترات به طور قابل ملاحظه ای میزان تخمک گذاری وحاملگی را در بیماران مبتلا به PCOS افزایش می دهد.

چکیده [English]

Introduction: About 10-15% of women in the reproductive ages suffer from infertility. The most common cause of infertility with female factor is ovulation disorders (35-40%) and among them polycystic ovary syndrome (PCOS) is the most common. These patients suffer from insulin resistance, obesity and increase in androgens; many recent studies have focused on the effects of insulin sensitizing agents like metformine on the patients with PCOS. The aim of this study was to evaluate the efficacy of the three therapeutic methods of clomiphen citrate (CC), metformine and clomiphen citrate plus metformine on infertile women with PCOS.
 
Materials and Methods: This clinical trial study performed in the year 2004 at Montaserie Hospital of Mashhad University of Medical Sciences, on three groups of 20 PCOS Patients. This study was approved by the local committee of medical ethics. The women in the first group were treated with clomiphen citrate, the second group with metformine and the third group with clomiphen plus metformine for 3 months. Data were analyzed by SPSS software, by using t-student and c2. p£0.05 was statistically significant.
 
Results:Over all twenty nine (48.3%) of women conceived, with the highest rate (%70) belonging to the group treated with clomiphen citrate plus metformine (group three). There was a significant difference comparing to the other two groups (p=0.028). This finding was even more significant in the cases with high levels of serum testosterone and hirsutism.
Conclusion: Combination of metfomine plus clomiphen citrate significantly improves ovulation and pregnancy rates in patients with PCOS.
