Comparison effect of Mixture of oxygen and N2O and Dexamethasone versus Remifentanil and Dexamethasone on the control of painless delivery

Abstract

Background and Objective: Labor pain is often severe, and unrelieved pain can negatively impact women's physiological status. It is associated with fear, which can slow labor progress. Effective pain management is crucial for medical and nursing staff. Various options for pain relief during labor come with their own risks and benefits. This study aimed to compare the effects of Entonox and Dexamethasone versus Remifentanil and Dexamethasone on painless delivery control in Alavi Hospital, Ardebil.

Methods: In this clinical trial, 100 women with term pregnancies were observed. All participants experienced spontaneous labor pains and had indications for normal vaginal delivery. They were randomly divided into two groups: one received Entonox and Dexamethasone, while the other received Remifentanil and Dexamethasone. Data on labor pain and delivery outcomes were collected using questionnaires.

Results: Pain severity, measured by VAS scores, was significantly lower in the Remifentanil and Dexamethasone group at 30, 60, 120, and 180 minutes post-intervention compared to the Entonox group. No significant differences were found in the duration of the active phase of labor, uterine contractions, Apgar scores, or fetal heart rates between the groups. Dry mouth was less common in the Remifentanil group. Other maternal complications, mean diastolic blood pressure, and respiratory rates showed no significant differences.

Conclusions: Concomitant use of remifentanil and dexamethasone is recommended compared with Entonox and Dexamethasone because it is safe for the mother and child and provides more favorable analgesia during labor.
