A case report of surgery for a rare, asymptomatic giant serous ovarian adenofibroma in a pregnant woman

Abstract

Background: Ovarian serous cystadenofibroma is a very uncommon tumor, especially in pregnancy. In this study, a pregnant woman with asymptomatic giant ovarian serous cystadenofibroma was introduced who underwent successful surgery.

Case presentation: The patient was a 31-year-old woman in her first pregnancy (14 weeks), without symptoms, and a multilocular cyst with dimensions of 259 x 173 mm and originating from the right ovary was reported in the routine pregnancy ultrasound. The levels of CA-125, CA 19-9 and HE4 were 72, 8.5 and 27.3 units per milliliter, respectively, and the RoMa index was 2.7 units per milliliter. The patient underwent open abdominal surgery and after being sent to pathology, the diagnosis of ovarian serous cystadenofibroma was confirmed. Finally, the patient underwent caesarean section at 39 weeks and two days of gestation due to macrosomia and a full-term baby weighing 4300 grams was born.

Conclusion: In pregnant women with ovarian serous cystadenofibroma, mass surgery, considering the tumor size and the levels of tumor markers such as CA-125, can be an effective treatment with minimal complications, especially in young nulliparous women who interested in preserving the uterus.
