گزارش چند مورد نادر حاملگی خارج رحمی محل اسکار سزارین و درمان موفق آن

چکیده
مقدمه: یکی از انواع نادر حاملگی خارج رحمی، حاملگی در محل اسکار سزارین (CSP) می­باشد که تشخیص آن مشکل بوده و دارای عوارض مهم از جمله پارگی رحم، خونریزی شدید و حتی مرگ مادر می‌باشد. مطالعه حاضر با هدف معرفی چند مورد حاملگی محل اسکار سزارین و درمان موفقیت‌آمیز آنها انجام شد.
معرفی بیمار: بیمار اول خانمی با دو بار سزارین قبلی و سن حاملگی حدود 8 هفته بود که در سونوگرافی شکمی و سونوگرافی ترانس واژینال انجام شده توسط متخصص رادیولوژی، ساک حاملگی دارای یک جنین با ضربان قلب در محل اسکار سزارین گزارش شده بود. بیمار تحت لاپاراتومی و وج رزکشن محل CSP قرار گرفت. بیماران دوم و سوم بیمارانی بودند که با علائم لکه‌بینی و تأخیر قاعدگی تحت سونوگرافی ترانس واژینال قرار گرفته بودند و برای آنها حاملگی 5/6 و 7 هفته در محل اسکار سزارین تشخیص داده شد و با تزریق متوتروکسات داخل ساک تحت هدایت سونوگرافی ترانس واژینال و سپس ساکشن کورتاژ تحت درمان قرار گرفتند.
نتیجه‌گیری: حاملگی اسکار سزارین از بیماری‌های نادر و دارای عوارض خطرناک می­باشد. تشخیص به موقع نیازمند وجود ظن بالینی بالا در بیماران با سابقه سزارین می­باشد و درمان مداخله‌ای مناسب موجب جلوگیری از عوارض جدی و کاهش موربیدیته و مورتالیته مادر می­شود.

Introduction: One of the types of EP is Cesarean Scar Pregnancy (CSP) that its diagnosis is difficult and can cause serious complications such as uterine rupture, profuse hemorrhage and even maternal death. This study was performed with aim to report some rare cases of CSP and the successful treatment.
Case presentation: The first case was a woman with history of 2 previous C/S with gestational age of 8 weeks. In TVS which was performed by radiologist, she had a gestational sac with a fetal pole having heart rate in cesarean scar. The patient underwent laparotomy and wedge resection of CSP. The second and third patients were detected with spotting and menstrual retard, and transvaginal sonography detected gestational sac with 6.5 & 7 weeks in the cesarean scar and were treated with elective intragestational sac methotrexate injections by ultrasound guidance and then curettage.
Conclusion: CSP is a rare condition with serious complications. For early diagnosis, one must consider high clinical suspicion for detecting it in patients with previous C/S, and appropriate interventional treatment leads to decreased maternal morbidity and mortality.
