چکیده
مقدمه: تومور تروفوبلاستیک جایگاه جفت زیرگروهی از تومورهای تروفوبلاستیک جفت میباشد و بروز آن یک در 100 هزار و بسیار نادر میباشد. این تومور از جایگاه جفت منشأ میگیرد و علائم آن به صورت خونریزی واژینال، علائم متاستاتیک و ... میباشد. در این مقاله یک تظاهر نادر PSTT گزارش میشود.
معرفی بیمار: بیمار خانمی 24 ساله P1L1 با سابقه زایمان طبیعی یکسال قبل و خونریزی نامنظم واژینال از 6 ماه قبل مراجعه کرده بود که در سونوگرافی لگن، توده 45×29 میلیمتر به نفع میوم در رحم گزارش شده بود که بیمار به دلیل خونریزی غیر طبیعی مقاوم به درمان، تحت میومکتومی قرار گرفته بود و در گزارش پاتولوژی لیومیوسارکوم مطرح شده بود که پس از ارجاع به تومور کلینیک بیمارستان قائم، بازبینی مجدد لامها انجام شد که PSTT گزارش شد. سپس سطح Bhcg سرم چک شد که 119 میلی یونیت در میلی لیتر بود و با توجه به عدم تمایل بیمار به حفظ باروری، داوطلب هیسترکتومی شد و در بررسی سطح Bhcg سرم یک هفته پس از عمل نرمال بود. بیمار با گذشت 8 ماه از عمل زنده و سالم میباشد.
نتیجه گیری:PSTT تومور نادری است که در بیمار با سابقه حاملگی ترم که با خونریزی غیرطبیعی و سونوگرافی که توده میومتریال گزارش شده است، باید مدنظر باشد.

Introduction: Placental site trophoblastic tumor (PSTT) is a subgroup of gestational trophoblastic tumor and its incidence is very rare and 1 in 10000. The origin of this tumor is placental site and its symptoms are vaginal bleeding, metastatic symptoms, and …. Here, a rare presentation of PSTT is reported.
Case Report: A 24-years old woman, P1L1 with history of vaginal delivery one year ago and abnormal vaginal bleeding from 6 months ago had referred. Pelvic sonography reported a fibroma sized 29×45 mm as a myoma in the uterus. Due to abnormal vaginal bleeding which was refractory to medical therapy, myomectomy was done. The pathologic diagnosis was leiomyosarcoma. Then, the patient was referred to Ghaem Hospital Tumor Clinic. The diagnosis of the second pathology review of the same specimen was placental site trophoblastic tumor. Serum hCG level was 119 milliunit in milliliter. The patient had no desire to preserve fertility, so hysterectomy was done. One week after surgery, hCG level was normal and now after 8 months, the patient is disease free.
Conclusion: Placental site trophoblastic tumor is a rare tumor which should be suspected in a patient who presents with term pregnancy and abnormal vaginal bleeding and a myoma in sonography.
