بررسی ارتباط بستن لوله‌های رحمی با اختلالات قاعدگی

چکیده

مقدمه: اختلالات قاعدگی یکی از شایع ترین مشکلات زنان است. وجود یک سندرم پس از بستن لوله همیشه مورد تردید بوده و هنوز ابهامات درباره وجود یا عدم وجود اختلالات قاعدگی مرتبط با بستن لوله وجود دارد. این تحقیق باهدف بررسی تفاوت احتمالی فراوانی بستن لوله‌ در بین زنان دارای خونریزی غیرطبیعی رحمی با افراد بدون این اختلالات انجام شد.
 
روش‌کار: در این مطالعه مورد- شاهدی روی مراجعه کنندگان به درمانگاه زنان بیمارستان بوعلی تهران از فروردین 1388 تا خرداد 1389، 300 خانم با شکایت خونریزی غیرطبیعی رحمی درگروه مورد و 313 نفر بدون خونریزی غیرطبیعی رحمی در گروه شاهد قرار گرفتند. نمونه گیری به روش در دسترس انجام شد. برای افراد پرسشنامه ای شامل اطلاعات فردی، ویژگی سیکل های قاعدگی و روش پیشگیری از بارداری تکمیل شد. داده ها با نرم افزار SPSS (نسخه 5/11) و با استفاده از آزمون های کای دو، آنووای یک طرفه، پیرسون و مانتل- هنزل تحلیل شد.
 
یافته‌ها: بستن لوله ها در 3/22% از زنان دارای خونریزی غیرطبیعی و 4/13% افراد گروه شاهد انجام شده بود (p=0/004، OR:1/85، 95% CI:1/21-2/8). پس از حذف اثر سن و پاریتی نیز بستن لوله درگروه مورد بیش از گروه شاهد بود (با حذف اثر سن p= 0/02 OR:1/69 و با حذف اثر پاریتی p= 0/028 OR:1/66. با در نظر گرفتن یک علامت غیرطبیعی از پنج معیار به عنوان خونریزی غیرطبیعی رحمی، باز هم فراوانی بستن لوله در زنان دارای خونریزی غیرطبیعی رحمی 3/27% و در گروه شاهد 2/14% بود (3/27% در برابر22/14% ، (p=0/03، OR:1/91، 95% CI:1/22-3/01).
 
نتیجه‌گیری: بین انجام بستن لوله های رحمی و بروز اختلالات قاعدگی ارتباط وجود دارد.

Introduction: Abnormal uterine bleeding (AUB) is one of the most common gynecologic problems. The existence of a post tubal ligation syndrome has been debated for decades and there are still discussions on the presence or absence of menstrual abnormalities related to tubal ligation. This study was performed with the aim of evaluating the probable difference of tubal ligation frequency between women with AUB and individuals who doesn’t have such abnormalities.
Methods: In this case-control study conducted in Bu-Ali hospital gynecology clinic in Tehran during April 2009- June 2001, 300 women with the chief complaint of AUB were compared to 313 women without AUB. Subjects were selected by available sampling method. A questionnaire containing the demographic data, the characteristics of menstrual cycles and the contraception method was completed for each subject. Data was analyzed by using SPSS 11.5 software and Chi-square, one-way ANOVA, Pearson and Mantel-Haenszel tests.
Results: The incidence of previous tubal ligation was higher in women with AUB than the control group (22.3% versus 13.4%. OR: 1.85, 95% CI: 1.21- 2.8, P=0.004). This difference was also significant after adjustment for age and parity (for parity: OR=1.66, P= 0.028 and for age OR: 1.69, P=0.02). The incidence of previous tubal ligation was higher in women with one abnormal menstrual characteristic out of five criteria (27.3% versus 14.22%. OR: 1.91, 95% CI: 1.22- 3.01, P= 0.03).
 
Conclusion: Results show that there is relationship between tubal ligation and menstrual problems. Further prospective cohort studies with long term follow up are suggested.
