اثرات میواینوزیتول خوراکی بر کیفیت اسپرم در مردان مبتلا به اولیگواستنوتراتوزوسپرمیا: متاآنالیز

چکیده

مقدمه: اولیگواستنوتراتوزواسپرمی (OAT) یکی از شایع‌ترین عوامل ناباروری مردان است که باعث تغییرات قابل توجه در پارامترهای اسپرم می‌شود. افزایش تولید رادیکال‌های آزاد اکسیژن (ROS) می‌تواند منجر به آسیب سلولی و کاهش باروری گردد. میواینوزیتول (MYO) می‌تواند بهبود بخشی از پارامترهای اسپرم را فراهم کرده و اثرات منفی ROS را کاهش دهد، بنابراین مطالعه حاضر با هدف بررسی اثربخشی ترکیبات حاوی میواینوزیتول بر پارامترهای اسپرم در مردان مبتلا به OAT انجام شد.
روشکار: جستجوی سیستماتیک در پایگاه‌های اطلاعاتی PubMed، Scopus، Web of Science، Cochrane، SID و Google Scholar با استفاده از کلمات کلیدی فارسی (ناباروری مردان، میواینوزیتول، کیفیت اسپرم، تحرک اسپرم، کارآزمایی تصادفی کنترل‌شده، متاآنالیز) و انگلیسی شامل Male Infertility، Myo-Inositol،Sperm Quality، Sperm Motility، Randomized Controlled Trial، Meta-Analysis با به‌کارگیری عملگرهای بولی AND و OR طراحی شد. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار Egger انجام شد. برای بررسی همگنی مطالعات از آزمون Cochran و شاخص I² استفاده گردید.
یافته ­ها: از مجموع 23،250 مقاله شناسایی شده، ۱۰ مقاله وارد مطالعه شدند که از این میان، ۴ مقاله قابلیت ورود به متاآنالیز را داشتند. تحلیل‌ها با استفاده از مدل اثر ثابت نشان داد که مصرف میواینوزیتول با افزایش معنی‌دار پارامترهای اسپرم همراه است؛ به‌طوری‌که تحرک اسپرم (اندازه اثر: 66/2 فاصله اطمینان 95%: 58/0 تا 73/4)، میزان تحرک پیش‌رونده (اندازه اثر: 20/3 فاصله اطمینان 95%: 44/1 تا 96/4)، غلظت اسپرم (اندازه اثر: 93/3 فاصله اطمینان 95%: 64/2 تا 41/5) و مورفولوژی اسپرم (اندازه اثر: 19/6 فاصله اطمینان 95%: 63/5 تا 75/6) بود.
نتیجه ­گیری: یافته‌ها نشان می‌دهند که مصرف میواینوزیتول می‌تواند بهبود قابل توجهی در پارامترهای اسپرم و باروری مردان مبتلا به OAT ایجاد کند و به‌عنوان یک مکمل مؤثر در درمان ناباروری مردان OAT مورد استفاده قرار گیرد.

Introduction: Oligoasthenoteratozoospermia (OAT) is one of the most common causes of male infertility, leading to significant alterations in sperm parameters. Increased production of reactive oxygen species (ROS) can cause cellular damage and reduce fertility. Myo-inositol (MYO) has been suggested to improve certain sperm parameters and mitigate the negative effects of ROS. This study was conducted with aim to evaluate the efficacy of myo-inositol-containing supplements on sperm parameters in men with OAT.
Methods: A systematic search was conducted in Magiran, PubMed, Scopus, Web of Science, Cochrane, SID, and Google Scholar databases using Persian and English keywords (Male Infertility, Myo-Inositol, Sperm Quality, Sperm Motility, Randomized Controlled Trial, Meta-Analysis) and keywords with Boolean operators AND/OR. Data analysis was performed using Egger software. Heterogeneity among studies was assessed using Cochran’s Q test and the I² index.
Results: Out of 23.250 identified articles, 10 studies entered the study, and among them, 4 studies were eligible for meta-analysis. The analysis using fixed-effect model showed that myo-inositol supplementation was associated with significant improvements in sperm parameters: sperm motility (effect size: 2.66; 95% CI: 0.58–4.73), progressive motility (3.20; 1.44–4.96), sperm concentration (3.93; 2.64–5.41), and sperm morphology (6.19; 5.63–6.75).
Conclusion: The findings indicate that myo-inositol supplementation can significantly improve sperm parameters and male fertility in men with OAT, supporting its potential use as an effective adjunct in the management of male infertility associated with OAT.
