بررسی اثر فروس سولفات در عملکرد تیروئید در زنان باردار مبتلا به کم‌کاری تیروئید اکتسابی در سه ماهه دوم بارداری در بیمارستان کوثر ارومیه

چکیده
مقدمه: زنان باردار در معرض خطر بالای کم‌خونی فقر آهن قرار دارند و کمبود آهن نیز در بیماران مبتلا به کم‌کاری تیروئید شایع است. با توجه به ارتباط متقابل متابولیسم آهن و عملکرد تیروئید، این مطالعه با هدف بررسی تأثیر فروس سولفات بر عملکرد تیروئید در زنان باردار مبتلا به کم‌کاری اکتسابی تیروئید در سه‌ماهه دوم بارداری در بیمارستان کوثر ارومیه انجام شد.
روشکار: این مطالعه کارآزمایی بالینی تصادفی بر روی 48 زن باردار مبتلا به کم‌کاری اکتسابی تیروئید در سه‌ماهه دوم بارداری انجام شد. شرکت‌کنندگان به‌صورت تصادفی در دو گروه 24 نفره مداخله و کنترل قرار گرفتند. گروه مداخله روزانه 2 قرص فروس سولفات حاوی 50 میلی‌گرم آهن عنصری دریافت کردند، در حالی که گروه کنترل 2 قرص دارونما مصرف نمودند. پس از 3 ماه، سطوح سرمی فریتین، آهن و هورمون محرک تیروئید (TSH) در هر دو گروه اندازه‌گیری و مقایسه شد. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار آماری SPSS (نسخه 21) و آزمون‌های تی مستقل و تی زوجی انجام شد. میزان p کمتر از 05/0 معنی‌دار در نظر گرفته شد.
یافته­ ها: میانگین سن شرکت‌کنندگان 24/11±90/30 سال بود. میانگین سطح سرمی فریتین و آهن در گروه مداخله به‌طور معنی‌داری بالاتر از گروه کنترل بود (به‌ترتیب 37/22 در برابر 96/14 نانوگرم بر میلی‌لیتر و 89/35 در برابر 24 میکروگرم بر دسی‌لیتر؛ 001/0>p). همچنین، میانگین سطح TSH بین دو گروه تفاوت آماری معنی‌داری نشان داد (001/0>p).
نتیجه­ گیری: نتایج این مطالعه نشان داد که مصرف فروس سولفات می‌تواند بر شاخص‌های عملکرد تیروئید در زنان باردار مبتلا به کم‌کاری اکتسابی تیروئید تأثیرگذار باشد. با اینحال، انجام مطالعات بیشتر با حجم نمونه بالاتر برای تأیید این یافته‌ها و تعیین اهمیت بالینی آن‌ها ضروری است.

Introduction: Pregnant women are more susceptible to iron deficiency anemia, and iron deficiency is common in patients with hypothyroidism. Given the interrelationship between iron metabolism and thyroid function, this study was conducted with aim to investigate the effect of ferrous sulfate on thyroid function in pregnant women with acquired hypothyroidism during the second trimester at Kosar Women’s Hospital in Urmia.
Methods: This randomized clinical trial study was conducted on 48 pregnant women with acquired hypothyroidism at second trimester. The participants were randomly divided into two groups of 24 subjects. The intervention group daily received two tablets of ferrous sulfate containing 50 mg of elemental iron, while the control group received two placebo tablets. After three months, serum levels of ferritin, iron, and TSH were evaluated in both groups. Data analysis was performed using SPSS statistical software (version 21) and independent and paired t-tests. P<0.05 was considered significant.
Results: The mean age of the participants was 30.90 ± 11.24 years. The mean serum ferritin and iron levels in the intervention group were significantly higher than those in the control group (22.37 vs. 14.96, P < 0.001; 35.89 vs. 24, P < 0.001, respectively). Additionally, the mean TSH levels showed a statistically significant difference between the two groups (P < 0.001).
Conclusion: The findings indicated that the use of ferrous sulfate could influence thyroid function in pregnant women with acquired hypothyroidism. However, further research with high sample size is necessary to confirm these findings and determine clinical importance.
