Introduction: Severe pain of labor not only can lead to stress and physiological complications, but sometimes can make mothers experience unpleasant natural delivery. Also, severity of labor pain can indirectly affect the newborn Apgar score. This study was conducted with aim to investigate the effectiveness of aromatherapy with citrus aurantium and auriculotherapy on labor pain in primiparous women and the newborn Apgar score.
Methods: This randomized clinical trial was conducted in 2024 with the participation of 138 primiparous women in Yazd, Iran. Participants were divided into three groups of ear acupressure, aromatherapy, and control. Aromatherapy with citrus aurantium essential oil and ear acupressure were administered in the active stage of labor for the two intervention groups. Labor pain intensity was assessed before and three times after the intervention with a visual analogue scale. Apgar scores were calculated at the first and fifth minutes after birth using the Apgar table. The data were analyzed using SPSS statistical software (version 26) and chi-square, one-way analysis of variance, and Bonferroni multiple comparisons tests. P<0.05 was considered significant.
Results: In the present study, aromatherapy and ear acupressure significantly reduced the labor pain intensity score compared to the control group (p<0.05). There was no statistically significant difference among the three groups of ear acupressure, aromatherapy, and control regarding the first and fifth minute Apgar scores of the newborn (p>0.05). Auriculotherapy had a greater soothing effect in the middle and late stages of labor, while spring orange aromatherapy showed higher effectiveness in the early stages of labor. Auriculotherapy had a greater soothing effect in the middle and late stages of labor, while aromatherapy with citrus aurantium showed higher effectiveness in the early stages of labor.
Conclusion: This study highlights the potential of aromatherapy and ear acupressure for managing the severity of labor pain. Furthermore, none of the interventions had a negative impact on the Apgar score of neonates. Therefore, these two methods can be suggested as complementary and safe non-pharmacological interventions to make the normal vaginal delivery process more pleasant. Given that the research sample consisted of women with indications for natural childbirth and met the necessary conditions for implementing interventions, the results of this study can be generalized to a population of women with similar conditions.
