بررسی تأثیر پیاده روی با گام شمار بر کیفیت ذهنی خواب زنان یائسه

چکیده

مقدمه: اختلال خواب یکی از آشکارترین علائم دوران یائسگی می باشد. هورمون درمانی علائم ناشی از یائسگی مانند بی خوابی را بهبود می بخشد، اما هورمون درمانی به دلیل اثرات جانبی که به همراه دارد، از طرف زنان انتخاب نمی شود. لذا بسیاری از زنان به راه های درمان جایگزینی دیگر متوسل می شوند. مطالعه حاضر با هدف تعیین تأثیر پیاده روی منظم با گام شمار بر کیفیت ذهنی خواب زنان یائسه در مراکز بهداشتی درمانی اهواز انجام شد.
روش کار: این مطالعه کارآزمایی بالینی در سال 1393 بر روی 112 زن یائسه واجد شرایط در مراکز بهداشتی درمانی اهواز انجام شد. افراد به طور تصادفی در دو گروه آزمایشی مداخله و کنترل قرار گرفتند. گروه مداخله هفته ای حداقل 5 بار و هر بار حداقل یک ساعت پیاده روی داشتند و تعداد قدم ها با گام شمار ثبت شد. در این پژوهش ابزار گردآوری داده ها پرسشنامه شاخص کیفیت خواب پیتزبورگ بود که در چهار مرحله (قبل از مداخله، 4 هفته و 8 هفته و 12 هفته بعد) جمع آوری شد. تجزیه و تحلیل داده ها با استفاده از نرم افزار آماری SPSS (نسخه 18) و آزمون های آماری تی تست، کای اسکوئر و آزمون غیرپارامتری من ویتنی و اندازه گیری تکراری فریدمن انجام شد. میزان p کمتر از 05/0 معنی دار در نظر گرفته شد.
یافته ها: میانگین نمره کیفیت ذهنی خواب در گروه مداخله از 6/0±1/2 به 6/0±2/1 به طور معناداری کاهش پیدا کرد (001/0=p) که نشانه بهبود وضعیت خواب می باشد و در گروه کنترل از 5/0±1/2 به 6/0±0/2 با اختلاف آمار بی معنایی تغییر پیدا کرد (45/0=p).
نتیجه گیری: پیاده روی منظم در زنان یائسه کیفیت ذهنی خواب را حفظ می کند و می تواند باعث ارتقاء کیفیت ذهنی خواب زنان شود. لذا با توجه به نتایج مطالعه در دسترس بودن منابع حمایتی در سیستم های بهداشتی درمانی، لزوم برگزاری جلسات آموزشی در مورد فعالیت فیزیکی و پیاده روی در مراکز مختلف بهداشتی درمانی هرچه بیشتر آشکار می شود.

چکیده [English]

Introduction: Sleep disturbance is one of the most obvious symptoms in menopause. Hormone replacement treatment improves menopausal symptoms such as insomnia. However, women do not select hormone therapy because of its adverse effects. This study was performed with aim to determine the effect of regular walking with pedometer on sleep subjective quality in postmenopausal women at medical-health centers of Ahvaz.
Methods: This randomized clinical trial was performed on 112 qualified postmenopausal women in medical-health centers of Ahvaz in 2014. The subjects were randomly placed in two groups of intervention and control. The intervention group had walking at least five times a week, each time for one hour, and their steps were recorded with a pedometer. Data collection tool was Pittsburgh Sleep Quality Index questionnaire that were collected in four stages (before intervention, four weeks, eight weeks, and twelve weeks later). Data was analyzed using SPSS software (version18) and t-test, Chi-square test,   non-parametric Mann-Whitney test, and Friedman repeated measurement. PResult: Mean score of sleep subjective quality in intervention group was significantly reduced from 2.1±0.6 to 1.2±0.6 (P=0.001) Which indicate improvement of sleeping. and in control group from 2.1±0.5 to 2±0.6 which was not significant (P=0.08).
Conclusion: Regular walking in postmenopausal women maintains sleep subjective quality and promotes their sleep subjective quality. Therefore, according to the results of the study and the access to supportive sources in health system, the necessity of holding the training sessions about physical activity and walking at health centers has been much more evident.
