Aim: The aim of present study is the comparing colposcopy and histological findings in cervical biopsy of patients with abnormal Pap smear
Methods: A cross-sectional study was performed on women who was referred to an academic gynecology oncology clinic from 2017-2018 with indication of colposcopy. Patients with history of previous abnormal Pap smear or biopsy due to suspicious cervical lesions were excluding of this study. Findings during the procedure including abnormal vessels or white lesions and other abnormal lesions were recorded. At the end, every patient’s data were completed after the pathological result of colposcopy biopsy and HPV test.
Results: Overall 200 patients enrolled in this study. 163 patients (81.5%) had abnormal finding in impression of oncologist at the end of the colposcopy. According to the abnormal pathologic results was reported in 120 patients (60%). The correlation between abnormal finding in impression and biopsy were 64% .The most common pathology report was CIN1 (85.8%), CIN2 (9.1%), CIN3 (2.5%), respectively. Sensitivity of colposcopy in predicting the pathology result was 89% and the specificity was 30%. The positive and negative predictive value rate in predicting the pathology result was reported 65% and 64% respectively.
Conclusion: According to present study, colposcopy has acceptable sensitivity in the cervical lesions diagnosis but associated with low specificity and high false positive probability. Therefore this diagnostic test can be used in one step of screening after the initial screening with Pap smear
