نیازهای زنان مراجعه کننده به مراکز بهداشتی- درمانی برای انجام آزمایش پاپ اسمیر
مقدمه: سرطان رحم چهارمین سرطان شایع در بین زنان ایرانی است. اکثر زنان آزمایش پاپ اسمیر که یک ابزار غربالگری جهت پیشگیری از سرطان رحم است را به عنوان یک روش معمول انجام نمیدهند. مطالعه حاضر با هدف تبیین نیازهای زنان مراجعهکننده به مراکز بهداشتی- درمانی شهر ورامین برای انجام آزمایش پاپ اسمیر انجام شد. روش کار: مطالعه حاضر بر پایه رویکرد کیفی در سال 1394 در شهر ورامین انجام گرفت. 15 زن مراجعهکننده به مراکز بهداشتی- درمانی از طریق نمونهگیری هدفمند وارد مطالعه شدند. دادهها از طریق مصاحبههای عمیق نیمه ساختار شامل 7 مصاحبه فردی و دو مصاحبه گروهی از گروه زنان دارای تجربه آزمایش پاپ اسمیر و نیز مشاهده و یاداشتبرداری جمعآوری شدند. تمام مصاحبهها ضبط و تایپ شد و از نرم افزار MAXQDA (نسخه 2010) جهت فرایند کدگذاری و طبقهبندی دادهها استفاده شد. تجزیه و تحلیل دادهها با استفاده از روش تحلیل محتوای کیفی قراردادی انجام گرفت. جهت صحت داده ها از معیارهای تأییدپذیری، اعتبارپذیری و انتقال پذیری استفاده شد. یافتهها: از تجزیه و تحلیل دادهها چهار طبقه اصلی شامل "تهدید درک شده"، "رفع موانع فرهنگی- محیطی"، "ایجاد انگیزه" و "تقویت ویژگیهای فردی" استخراج گردید. نتیجه گیری: انجام آزمایش پاپ اسمیر جهت پیشگیری از سرطان منوط به تعامل مناسب و صمیمی با پرسنل بهداشتی و درمانی، حمایت خانواده، سیاستها و قوانین تشویق کننده و اطلاع رسانی و هماهنگی سازمانها است

بررسی ارتباط نمره بیشاپ حین پذیرش با پیامدهای زایمانی در زنان نخست زا در بیمارستان 29 بهمن تبریز
مقدمه: هدف از مراقبتهای زمان تولد، داشتن مادر و نوزاد سالم و دستیابی به یک زایمان طبیعی موفق است. از آنجایی که عدم آمادگی سرویکس منجر به افزایش سزارین و ایجاد عوارض مادری و نوزادی می شود، مطالعه حاضر با هدف بررسی ارتباط بین نمره بیشاپ حین پذیرش با پیامدهای زایمانی در زنان نخستزا انجام شد. روش کار: این مطالعه توصیفی تحلیلی در سال 1394 بر روی 300 نفر از زنان باردار نخستزا مراجعهکننده به بخش زایمان بیمارستان 29 بهمن تبریز انجام شد. ابزار گردآوری دادهها شامل چک لیست مشخصات فردی، تاریخچه مامایی، اطلاعات نمره بیشاپ حین پذیرش واحدهای پژوهش و پیامدهای زایمانی بود. اطلاعات مورد نیاز از طریق مشاهده سیر زایمان و مطالعه مندرجات پرونده بیمار گردآوری شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS(نسخه 21) و آزمون همبستگی انجام شد. میزان p کمتر از 05/0 معنی دار در نظر گرفته شد. یافتهها: در این مطالعه بین نمره بیشاپ و طول مدت مرحله اول و دوم زایمان (0001/0=p)، طول انقباضات رحمی در مرحله اول زایمان (001/0=p)، الگوی ضربان غیر طبیعی قلب جنین در مرحله اول و دوم زایمان (0001/0=p)، استفاده از اکسیتوسین (0001/0=p)، دفعات استفاده از اکسیتوسین (0001/0=p)، فاصله بین آمنیوتومی یا پارگی خودبخودی پردهها تا زایمان(0001/0=p)، انجام اپیزیاتومی (01/0=p)، فشار بر فوندوس هنگام زایمان (001/0=p)، خونریزی بیش از حد بعد از زایمان (014/0=p)، آپگار دقیقه اول نوزاد (014/0=p) و اقدامات انجام شده بر روی نوزاد (009/0=p) ارتباط آماری معنیداری وجود داشت. روش زایمان (0001/0=p) با نمره بیشاپ حین بستری در بیمارستان ارتباط آماری معنیداری داشت؛ به طوری که 41 نفر (1/31%) از افراد با نمره بیشاپ کمتر یا مساوی 4 و 6 نفر (6/3%) از افراد با نمره بیشاپ بیشتر از 4 نیاز به انجام سزارین داشتند. نتیجهگیری: نمره بیشاپ پایین و عدم آمادگی سرویکس منجر به افزایش پیامدهای نامطلوب زایمانی شده و با توجه به نتایج پژوهش باید اقداماتی جهت آماده کردن سرویکس به منظور ارتقاء امتیاز بیشاپ انجام داد.

مرور سیستماتیک کارآزمایی های بالینی در درمان شقاق و درد نوک پستان زنان شیرده
مقدمه: شقاق نوک پستان یکی از مشکلات شایع زنان شیرده و عمده ترین دلیل توقف زودهنگام شیردهی محسوب میشود. به دلیل اهمیت تغذیه با شیر مادر، مطالعه حاضر با هدف مروری بر مطالعات انجام شده در زمینه کارآزماییهای بالینی در درمان شقاق و درد نوک پستان زنان شیرده انجام شد. روش کار: در این مطالعه مرور سیستماتیک تمام پایگاههای اطلاعاتی داخلی با کلید واژه "شقاق نوک پستان" و "کارآزمایی بالینی" و پایگاههای GoogleScholar، PubMed، Proquest، Scopus، Science Directو Medline با کلیدواژه های "clinical trial"and "sore nipple" بدون محدودیت زمانی و زبانی تا نوامبر 2015 جستجو شدند. تمام مقالات بر اساس معیار جداد بررسی و مطالعاتی که نمره 3 یا بیشتر گرفتند، انتخاب شدند. نتایج به صورت کیفی گزارش شد. یافتهها: در نهایت 14 مقاله که دارای معیار ورود به مطالعه بودند، مورد بررسی قرار گرفتند. از میان درمانهای بررسی شده، لانولین همراه با محافظ پستان، ژل آلوئورا، اسانس منتول و کمپرس آب گرم باعث کاهش بیشتر شدت درد و بهبود سریعتر شقاق نوک پستان شده بودند. نتیجه گیری: لانولین همراه با محافظ پستان، ژل آلوئورا، اسانس منتول و کمپرس آب گرم مؤثرترین درمان شقاق نوک پستان میباشند. قضاوت در مورد اثربخشی قطعی این مداخلات، مطالعات بیشتر و دقیقتر و با حجم نمونه کافی را میطلبد. مطالعات در زمینه درمان شقاق و درد نوک پستان پراکنده هستند و نتیجهگیری در مورد هر مداخله با توجه به محدود بودن مطالعات امکان پذیر نیست.
