مقایسه میزان تهوع و استفراغ بهدنبال بیحسی اسپاینال با استفاده از دو ترکیب دارویی بوپیواکائین-فنتانیل و بوپیواکائین- سوفنتانیل در اعمال جراحی سزارین
مقدمه: بیحسی اسپاینال، رایجترین روش بیدردی برای سزارین الکتیو میباشد. تهوع و استفراغ در بیماران میتواند باعث اختلال در انجام جراحی گردد. مطالعه حاضر با هدف بررسی مقایسهای فراوانی تهوع و استفراغ بهدنبال بیحسی اسپاینال با استفاده از دو ترکیب دارویی بوپیواکائین- فنتانیل و بوپیواکائین- سوفنتانیل در اعمال جراحی سزارین انجام شد. روشکار: این مطالعه کارآزمایی بالینی دوسوکور در سال ١٣٩٢ بر روی 60 بیمار با بارداری ترم کاندید سزارین الکتیو و تحت بیحسی اسپاینال انجام گردید. گروه F 25 میکروگرم فنتانیل و گروه S 5/2 میکروگرم سوفنتانیل با 5/12 میلیگرم بوپیواکائین اینتراتکال دریافت کردند. عارضه اصلی تهوع و استفراغ حین و در ساعتهای 1، 2، 6، 12 و 24 بعد از عمل بود. سایر عوارضی مورد بررسی شامل: هیپوتانسیون، احتباس ادراری و زمان بازگشت بلوک حسی به حد T8 بودند. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه ١٦) و آزمونهای کای دو، تی دانشجویی، رگرسیون لوجستیک و لوجستیک دادههای تکراری انجام گردید. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد. یافتهها: دو گروه درمیزان بروز تهوع (395/0=p) و استفراغ حین (5/0=p)، 1، 2، 6، 12 و 24 ساعت پس از جراحی، هیپوتانسیون (589/0=p)، تغییرات ضربان قلب (105/0=p)، احتباس ادراری، زمان ایجاد بلوک حسی تا سطح 8T (556/0=p) و برگشتن بلوک حرکتی (787/0=p) تفاوت معناداری نداشتند. نتیجهگیری: استفاده از دو داروی فنتانیل و سوفنتانیل در ترکیب با بوپیواکائین تأثیری در میزان فراوانی و شیوع تهوع و استفراغ نداشتند.

نقش کمکاری تیروئید بر مورفولوژی تخمدان و رحم در سونوگرافی زنان در سنین باروری
مقدمه: هیپوتیروئیدی یک سندرم بالینی ناشی از کمبود هورمونهای تیروئیدی است که میتواند در تولید مثل تأثیرگذار باشد. مطالعه حاضر با توجه به شیوع هیپوتیروئیدی و نازایی با هدف بررسی نقش هیپوتیروئیدی بر مورفولوژی رحم و تخمدان انجام شد. روشکار: در این مطالعه همگروهی که در سال 1396 در درمانگاه بیمارستان امام رضا (ع) کرمانشاه انجام شد، ۱۶۱ زن مبتلا به هیپوتیروئیدی ساب کلینیکال که بر اساس سطح TSH سرم انتخاب شده بودند، بهعنوان گروه مواجهه و ۱۶۱ زن سالم بهصورت همتا شده بهعنوان گروه شاهد انتخاب شدند. در روز چهاردهم سیکل ماهیانه از رحم و تخمدان هر دو گروه سونوگرافی گرفته شد و اطلاعات بهدست آمده در فرم مخصوص جمعآوری دادهها وارد شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه ۱۶) و آزمونهای آماری فیشر، کولموگروف اسمیرنوف و تی مستقل انجام گرفت.میزان p کمتر از 05/0 معنادار در نظر گرفته شد. یافتهها: در گروه مواجهه نسبت به گروه شاهد بهطور معنیداری بینظمی قاعدگی (01/0=p)، کاهش تعداد بارداری یا عدم آن (017/0=p)، افزایش ضخامت آندومتر (01/0=p)، افزایش حجم و تعداد فولیکولهای تخمدانها و کاهش سایز فولیکولهای تخمدان (01/0=p) مشاهده شد،اما هیپوتیروئیدی نقشی در اندازه رحم نداشت (406/0=p). نتیجهگیری: هیپوتیروئیدی با افزایش حجم تخمدانها، تعداد فولیکولها و کاهش سایز فولیکولها سبب تغییر مورفولوژی تخمدانها، مشابه اختلال تخمدان پلیکیستیک شده و میتواند علتی برای نامنظمی قاعدگی در زنان سنین باروری باشد.

فراوانی فنوتیپهای سندرم تخمدان پلیکیستیک در نوجوانان شهر مشهد براساس معیار روتردام در سال 1397
مقدمه: سندرم تخمدان پلیکیستیک (PCOs) شایعترین اختلال غدد درونریز در زنان بهشمار میرود. با توجه به زمان شروع PCOS از دوران نوجوانی، مطالعه حاضر با هدف بررسی میزان شیوع این سندرم و فنوتیپهای آن براساس معیار روتردام در دختران نوجوان شهر مشهد انجام شد.
روشکار: این مطالعه مقطعی در سال 98-1397 بر روی 650 نفر از دختران 19-14 سال مدارس دولتی مشهد انجام شد. نمونهگیری بهصورت خوشهای احتمالی چند مرحلهای طبقهبندی شده انجام گردید. بعد از پر کردن پرسشنامه اطلاعات فردی و دموگرافیک در صورت داشتن معیارهای ورود اقدام به معاینه بالینی از نظر بررسی علائم بالینی هیپرآندروژنیسم شامل هیرسوتیسم، آکنه و ریزش مو با الگوی مردانه نموده و در صورت مثبت بودن این علائم جهت انجام آزمایشات هورمونی و سونوگرافی ارجاع داده میشدند. تشخیص سندرم تخمدان پلیکیستیک و فنوتیپهای آن بر اساس معیار روتردام صورت گرفت. تجزیه و تحلیل دادهها با استفاده از نرمافزار SPSS (نسخه 22) و آزمونهای کای دو و فیشر انجام گرفت. میزان p کمتر از 05/0 معنادار در نظر گرفته شد.
یافتهها: فراوانی فنوتیپهای چهارگانه سندرم تخمدان پلیکیستیک براساس معیارهای روتردام شامل فنوتیپ A (هیپرآندروژنیسم بالینی/بیوشیمایی+تخمدان پلیکیستیک) 3 نفر (7/7%)، فنوتیپ B (اختلالات تخمک گذاری+هیپرآندروژنیسم بالینی/بیوشیمایی) 11 نفر (2/28%)، فنوتیپ C (اختلالات تخمکگذاری+تخمدان پلیکیستیک) 14 نفر (9/35%) و فنوتیپ D (اختلالات تخمکگذاری+هیپرآندروژنیسم بالینی/بیوشیمایی+تخمدان پلیکیستیک) 11 نفر (2/28%) مشخص گردید. شیوع کلی سندرم تخمدان پلیکیستیک بر اساس معیار روتردام 8/6% بهدست آمد.
نتیجهگیری: با توجه به شیوع فنوتیپهای چهارگانه سندرم تخمدان پلیکیستیک بهدست آمده و مشابهت علائم با تظاهرات بالینی دوران نوجوانی، بهتر است تشخیص و درمان این سندرم از سنین نوجوانی شروع گردد.

بررسی عوامل مرتبط با عملکرد جنسی در زنان شیرده با کمکاری میل جنسی مراجعهکننده به مراکز بهداشتی درمانی شهر سنقر در سال 1397
مقدمه: عوامل مختلفی از جمله زایمان و شیردهی میتوانند عملکرد جنسی زنان را تحت تأثیر قرار دهند. شناخت مشکلات جنسی و عوامل مداخله کننده در دوره پس از زایمان از ضروریات است. مطالعه حاضر با هدف تعیین عوامل مرتبط با عملکرد جنسی زنان شیرده با کمکاری میل جنسی انجام شد.
روشکار: این مطالعه توصیفی در سال 1397 بر روی 104 زن شیرده طی 6-2 ماه پس از زایمان با کمکاری میل جنسی در مراکز جامع سلامت شهر سنقر انجام شد. ابزار گردآوری دادهها شامل پرسشنامه مشخصات دموگرافیک و مامایی، پرسشنامه ارزیابی میل جنسی کمکار و شاخص عملکرد جنسی زنان بود. تجزیه و تحلیل دادهها با استفاده از نرمافزار R (نسخه 2.5.3) و SPSS (نسخه 24) و آزمونهای آماری منویتنی و ضریب همبستگی پیرسون انجام گرفت. میزان p کمتر از 05/0 معنادار در نظر گرفته شد.
یافتهها: بر اساسنتایج آزمون ضریب همبستگی پیرسون، بین سن و تمایل جنسی، تهییج جنسی، ارگاسم، رضایت جنسی و همچنین نمره کل عملکرد جنسی همبستگی معکوس و معنیداری وجود داشت (05/0>p). همچنین همبستگی معکوس و معنیداری بین تعداد بارداری با تمایل جنسی و تهییج جنسی (05/0>p) و بین تعداد فرزند زنده با تمایل جنسی مشاهده شد (05/0>p). همبستگی بین عملکرد جنسی با سن ازدواج، سن نوزاد، فاصله بین بارداریهای اخیر و میزان درآمد از نظر آماری معنیدار نبود (05/0<p).
نتیجهگیری: سن، تعداد بارداری و تعداد فرزند زنده با عملکرد جنسی همبستگی معکوس دارند، بدینصورتکه با افزایش سن، تعداد بارداری و تعداد فرزند زنده، نمره عملکرد جنسی کاهش مییابد. همبستگی بین عملکرد جنسی با سن ازدواج، سن نوزاد، فاصله بین بارداریهای اخیر و میزان درآمد از نظر آماری معنیدار نبودند.

مقایسه زمان تجویز شیاف دیکلوفناک بر روی درد پس از عمل کشیدن اووسیت بهروش ترانس واژینال تحت بیهوشی عمومی: یک مطالعه کارآزمایی بالینی تصادفی شده دوسوکور
مقدمه: با توجه به روحیات حساس بیماران کاندید باروری آزمایشگاهی، در مورد شدت درد تجربه شده پس از کشیدن اووسیت، داروی ضددرد مناسب و زمان تجویز آن در بیماران، اختلاف نظر وجود دارد. مطالعه حاضر با هدف مقایسه زمان تجویز شیاف دیکلوفناک بر روی درد پس از عمل کشیدن اووسیت بهروش ترانس واژینال تحت بیهوشی عمومی انجام شد.
روشکار: این مطالعه کارآزمایی بالینی در سال 1397 بر روی 70 زن کاندید کشیدن تخمک از تخمدان در مرکز ناباروری بیمارستان آموزشی درمانی پژوهشی فاطمیه انجام شد. بیماران بهصورت تصادفی به دو گروه 35 نفره A و B تقسیم شدند. گروه B، 15 دقیقه قبل از کشیدن اووسیت از فولیکول و گروه A بلافاصله بعد از کشیدن اووسیت از فولیکول تخمدان 100 میلیگرم شیاف دیکلوفناک دریافت کردند. شدت درد با استفاده از مقیاس رتبهبندی دیداری درد (VAS) بعد از ورود به ریکاوری در دقیقه 15، 30، 60، 90 و 120 ارزیابی شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 16) و آزمونهای تی مستقل، منویتنی و کای دو (تست دقیق فیشر) انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: هر دو گروه از نظر میانگین سن، سطح تحصیلات، تعداد تخمک کشیده شده، طول مدت بیهوشی، داروی بیهوشی استفاده شده و وضعیت همودینامیک اختلاف معناداری نداشتند (05/0>p). 25 نفر (4/71%) از بیماران گروه B و 32 نفر (4/91%) از گروه A پس از بیهوشی درد شکمی یا واژینال داشتند (03/0=p). میانگین کل شدت درد در گروه B در 120 دقیقه اول پس از بیهوشی کمتر از گروه A بود و در دقیقه 15 و 90پس از بیهوشی اختلاف معنیداری داشتند (032/0=p، 017/0=p). تهوع و استفراغ در 4 نفر (4/11%) از گروه B و 3 نفر (6/8%) از گروه A مشاهده شد (690/0=p). هیچکدام از بیماران خونریزی پس از عمل نداشتند.
نتیجهگیری: تجویز شیاف دیکلوفناک سدیم قبل از کشیدن اووسیت از فولیکول نسبت به تجویز آن پس از کشیدن اووسیت از فولیکول در بیدردی بیماران مؤثرتر است.

مقایسه دوز کم پروپوفول با میدازولام بر روی علائم همودینامیک مادر و آپگار نوزادان در زنان تحت بیحسی اسپاینال در عمل جراحی سزارین: یک مطالعه کارآزمایی بالینی تصادفی شده دوسویهکور
مقدمه: بیحسی ناحیهای، روش انتخابی در جراحی سزارین میباشد. داروهای خوابآور داخل وریدی که در القاء بیهوشی استفاده میشوند، بهسرعت از جفت عبور کرده و ممکن است باعث دپرسیون تنفسی جنین شوند. مطالعه حاضر با هدف مقایسه دوز کم پروپوفول با میدازولام بر روی علائم همودینامیک مادران و آپگار نوزادان در زنان تحت بیحسی اسپاینال در عمل جراحی سزارین انجام شد. روشکار: در این مطالعه کارآزمایی بالینی تصادفی شده دوسویهکور که در سال 1392 در شهرستان جهرم انجام شد، 42 بیمار با کلاس بیهوشی I و II تحت بیحسی اسپاینال به دو گروه مساوی تزریق 20 میلیگرم پروپوفول و 5/1 میلیگرم میدازولام همزمان با انجام بیحسی اسپاینال قرار گرفتند. علائم همودینامیک مادر در دقایق 1، 3، 5، 10، 15، 30 و 60 دقیقه بعد از بیحسی اسپاینال و آپگار نوزادان در دقایق 1 و 5 بعد از بهدنیا آمدن سنجیده شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 21) و آزمون منویتنی انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد. یافتهها: بین گروههای میدازولام و پروپوفول از لحاظ فشارخون سیستول و دیاستول، اشباع اکسیژن و ضربان قلب در زمانهای مختلف و از لحاظ آپگار نوزادان تفاوت آماری معنیداری وجود نداشت (05/0p>). نتیجهگیری: با توجه به نتایج مطالعه و عدم تفاوت دو گروه از نظر آپگار نوزادان و نوسانات متغیرهای همودینامیک مادر، میتوان نتیجه گرفت در شرایط معمولی دوزهای کم پروپوفول و میدازولام برای جراحی سزارین، از نظر بالینی تفاوت بارزی بر آپگار نوزاد و متغیرهای همودینامیک مادر ندارند و میتوان بسته به شرایط و تجربه متخصص بیهوشی از داروهای مورد نظر استفاده کرد.

بررسی تأثیر کپسول خوراکی عصاره برگ ریحان بر افسردگی زنان یائسه: یک کارآزمایی بالینی تصادفیسازی شده
مقدمه: زنان بهدلیل تغییرات جسمانی و روانی دوران یائسگی، عوارض متعددی را تجربه میکنند. یکی از مهمترین این عوارض، بروز افسردگی است. با توجه به اثرات جانبی داروهای شیمیایی، امروزه استفاده از درمانهای گیاهی مورد توجه زنان قرار گرفته است. مطالعه حاضر با هدف تعیین تأثیر کپسول خوراکی عصاره برگ ریحان بر افسردگی زنان یائسه انجام شد.
روشکار: این مطالعه کارآزمایی بالینی تصادفی دوگروهه سهسوکور در سال 99-1398 بر روی 60 زن یائسه مراجعهکننده به مراکز بهداشتی درمانی مشهد انجام شد. زنان واجد شرایط ورود به مطالعه بهمدت یک ماه روزی یک عدد کپسول 500 میلیگرمی عصاره برگ ریحان یا دارونما را دریافت میکردند. بررسی افسردگی قبل و بعد از مداخله با ابزار DASS (نسخه 21) انجام شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار SPSS (نسخه 21) و آزمونهای تی مستقل، منویتنی و ویلکاکسون صورت گرفت. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: قبل از مداخله میانگین نمره افسردگی در گروه ریحان 3/6±8/8 و در گروه دارونما 3/6±6/8 بود که از نظر آماری تفاوت معنیداری نداشت و دو گروه همگن بودند (904/0=p). بعد از مداخله میانگین نمره افسردگی در گروه ریحان (3/5±9/5) بهطور معناداری کمتر از گروه دارونما (2/6±2/9) بود (049/0=p). تفاوت نمره افسردگی بعد نسبت به قبل از مداخله در گروه ریحان 5/4±8/2 نمره کاهش و در گروه دارونما 4/2±6/0 نمره افزایش داشت که این تفاوت از نظر آماری معنیدار بود (001/0>p).
نتیجهگیری: مصرف کپسول عصاره برگ ریحان منجر به کاهش نمره افسردگی در زنان یائسه شد، بنابراین میتواند بهعنوان یک گیاه دارویی برای کاهش افسردگی زنان در دوران یائسگی مورد استفاده قرار گیرد.

اثربخشی بوپروپیون بر عملکرد جنسی زنان مبتلا به مالتیپل اسکلروزیس: یک کارآزمایی بالینی
مقدمه: اختلالات جنسی در زنان مبتلا به بیماری مولتیپل اسکلروزیس (MS) شیوع نسبتاً زیادی دارد و بر کیفیت زندگیشان اثر میگذارد. مطالعه حاضر با هدف بررسی تأثیر بوپروپیون آهسته رهش بر عملکرد جنسی زنان مبتلا به مالتیپل اسکلروزیس انجام شد.
روشکار: این مطالعه کارآزمایی بالینی شاهددار تصادفی شده در سال 1398 بر روی 60 زن مبتلا به MS نوع بهبود یابنده- عودکننده که تحت درمان معمول MS و دارای اختلالات جنسی بودند، انجام شد. افراد به دو گروه 30 نفره دریافتکننده بوپروپیون و پلاسبو تقسیم شدند. پرسشنامههای صمیمیت و جنسی مولتیپل اسکلروزیس، شاخص عملکرد جنسی زنان، و شدت خستگی و همچنین پرسشنامه افسردگی بک II پیش، 4 و 8 هفته پس از شروع مداخلات برای تمامی بیماران تکمیل شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 20) و آزمونهای تحلیل واریانس و اندازهگیریهای مکرر انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: نمره پرسشنامه عملکرد جنسی 8 هفته پس از مداخلات در گروه دریافتکننده بوپروپیون (85/45±96/9) بهطور معنیداری کمتر از گروه پلاسبو (46/11±56/53) بود (007/0=p). همچنین نمره افسردگی در دو مرحله پس از مداخله در گروه بوپروپیون (03/23±26/11) کمتر از گروه پلاسبو (43/13±86/37) بود و در سیر مداخلات کاهش یافته بود (0001/0p<). نمره شاخص عملکرد جنسی FSFI و زیرمقیاسهای آن در طول مطالعه در گروه بوپروپیون (88/25±67/2) نسبت به گروه دریافتکننده پلاسبو (8/2±44/17) بهبود یافت که نشاندهنده تداوم اثربخشی بوپروپیون مستقل از تغییرات نمرات افسردگی و خستگی در طی درمان در این بیماران بود (001/0p<).
نتیجهگیری: نتایج این مطالعه اثربخشی درمان با بوپروپیون در بهبود اختلالات جنسی در بیماران زن مبتلا به MS را نشان داد.

بررسی ارتباط خشونت خانگی از طرف همسر و پرهاکلامپسی در زنان پستپارتوم مراجعهکننده به مرکز آموزشی- درمانی شهر سنندج در سال 1396
مقدمه: پرهاکلامپسی از عوارض دوران بارداری و یک اختلال چندارگانی است که در 5-3% از بارداریها در کشورهای غربی رخ میدهد. با توجه به درگیری سیستم اتونوم و سمپاتیک در پاتوفیزیولوژی پرهاکلامپسی، نقش استرس در تشدید این عارضه پررنگ میگردد. از آنجایی که نقش عواملی چون خشونت خانگی بر بروز و یا تشدید پرهاکلامپسی مطرح میباشد و از طرفی با توجه به رشد و شیوع خشونت خانگی در کشور، مطالعه حاضر با هدف بررسی و تعیین میزان ارتباط بین خشونت خانگی و ابتلاء به پرهاکلامپسی انجام شد.
روشکار: این مطالعه مورد- شاهدی در سال 1396 بر روی 110 مادر پستپارتوم در دو گروه 55 نفره مبتلا به پرهاکلامپسی و غیرمبتلا به پرهاکلامپسی در بیمارستان بعثت شهرستان سنندج انجام شد. ابتدا اطلاعات جمعیتشناختی با استفاده از پرسشنامه محققساخته دموگرافیک- مامایی و سپس اطلاعات مربوط به خشونت خانگی با استفاده از پرسشنامه خشونت خانگی برگرفته از سازمان جهانی بهداشت جمعآوری شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار SPSS (نسخه 22) و آزمونهای آماری تی تست، من ویتنی و کای اسکوئر انجام گرفت. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: بین خشونت خانگیاز طرف همسر با پرهاکلامپسی ارتباط معنیداری وجود نداشت (203/0=p). احتمال ابتلاء به پرهاکلامپسی در صورت مواجهه با خشونت خانگی و خشونت فیزیکی 5/1 برابر گروه شاهد، مواجهه با خشونت جنسی 6/1 برابر گروه شاهد و مواجهه با خشونت روانی 2/1 برابر گروه شاهد بود. بین شدت خشونت فیزیکی (328/0=p)، جنسی (473/0=p) و روانی (606/0=p) با پرهاکلامپسی ارتباط معنیداری وجود نداشت.
نتیجهگیری: بین ابتلاء به پرهاکلامپسی و انواع خشونتها ارتباط معنیداری یافت نشد.

نوروپاتیهای با منشأ ایمنی در دوران حاملگی و نفاس: مطالعه مروری
مقدمه: سندرم گیلنباره (GBS) و پلی رادیکولونوروپاتی التهابی دمیلینیزان مزمن (CIDP) گونهای از اختلالات با منشأ ایمنی اعصاب محیطی هستند که علائم مشخصه آنها شامل: ضعف قرینه پیشرونده عضلات، فقدان رفلکسهای وتری عمقی و نشانههای حسی میباشد. مقاله مروری حاضر با هدف بررسی اثرات بارداری و دوران نفاس بر نوروپاتیهای با منشأ ایمنی و همچنین اثرات این نوروپاتیها بر روی پیشآگهی حاملگی و نحوه درمان این بیماریها در این دوران انجام شد.
سندرم گیلنباره بهصورت حاد طی 4-2 هفته تکامل پیدا میکند و در دو سوم موارد، سابقه ابتلاء به عفونتهای ویروسی و باکتریایی پیش از بروز علائم وجود دارد. پلی رادیکولونوروپاتی التهابی دمیلینیزان مزمن، یک اختلال مزمن اتوایمیون است که در قیاس با سندرم گیلنباره که طی ۴ هفته تکامل مییابد، سیر پیشرفت حداقل ۸ هفتهای دارد.
در این مقاله اثرات بارداری و دوره نفاس بر روی این بیماریها بررسی و همچنین اثر این بیماریها بر روی حاملگی و دوران پس از زایمان در مطالعاتی که در سالهای اخیر انجام گرفته است، مرور شد و درمانهای مناسب این بیماریها در این دوران جمعآوری گردید. این مقاله میتواند مرجع مناسبی جهت متخصصین زنان و مغز و اعصاب برای درمان و تعیین پیشآگهی مادران مبتلا به نوروپاتیهای با منشأ ایمنی قرار بگیرد.
نتیجهگیری: بسیاری از بیماریهای نوروماسکولار با منشأ ایمنی در زنان سنین باروری بروز مییابند و ممکن است بروز آنها در این دوران بر روی روند بارداری و زایمان تأثیرگذار بوده و یا بارداری بر روی سیر و علائم بیماری و همچنین درمان و پیشآگهی این اختلالات مؤثر باشند. از جمله مهمترین بیماریهای سیستم اعصاب محیطی با منشأ ایمنی، سندرم گیلنباره و پلی رادیکولونوروپاتی مزمن دمیلیزان التهابی میباشند. شناخت اثرات این دو اختلال در دوران بارداری و نفاس و نیز اثرات بارداری بر سیر این دو بیماری حائز اهمیت است.

بررسی صحت تشخیصی دو روش برست لایت و ترموگرافی در غربالگری سرطان پستان: یک مطالعه مروری
مقدمه: ماموگرافی، استاندارد طلایی برای تشخیص سرطان پستان است، اما در سالهای اخیر روشهای دیگری مانند دستگاه برست لایت و ترموگرافی در غربالگری سرطان مطرح شده است، اما نتایج ضدونقیضی در مورد صحت تشخیصی این دو روش وجود دارد. مطالعه مروری حاضر با هدف تعیین صحت تشخیص دو روش برست لایت و ترموگرافی در غربالگری سرطان پستان انجام شد.
روشکار: برای دستیابی به مطالعات، جستجو بین سالهای 2019-1900 در پایگاه الکترونیکی SID،Google scholar، Pubmed،Cochran ،Scopus و Web Of Sciences با استفاده از کلیدواژههای انگلیسیBreast Cancer، Thermography، Breast light، Ultrasound و Mammography انجام شد. جمعیت مورد مطالعه تمام زنان مراجعهکننده به مراکز درمانی بودند که با روش ترموگرافی و برست لایت ارزیابی شدند. معیارهای اصلی ورود مطالعات در مطالعه شامل: مقالات منتشر شده در مورد حساسیت، ویژگی، ارزش اخباری مثبت و ارزش اخباری منفی دو روش ترموگرافی و برست لایت بودند. دادههای ناکافی مقالات و محتوای نامناسب و خارج از مطالعه، بهعنوان معیارهای خروج از مطالعه در نظر گرفته شدند. متن کامل مقالات مرتبط با استفاده از چکلیست STARD ارزیابی و در نهایت 9 مقاله که واجد ورود به مطالعه بودند، انتخاب شد.
یافتهها: در بررسی مطالعات مختلف با مقایسه حساسیت، ویژگی، ارزش اخباری مثبت و منفی دو روش برست لایت و ترموگرافی با سایر روشها (ماموگرافی، سونوگرافی و معاینه بالینی پستان)، روش برست لایت دارای حساسیت 93-6/85%، ویژگی 3/73-4/53%، ارزش اخباری مثبت 5/93-4/91% و منفی 8/77-7/29% بود. از طرفی روش ترموگرافی با حساسیت 100%، ویژگی 3/95-68/68%، ارزش اخباری مثبت 1/41-8/15% و منفی 100-1/99% گزارش شد.
نتیجهگیری: روش برست لایت ویژگی پایین دارد و روش ترموگرافی نیز اگرچه حساسیت بالایی دارد، اما ویژگی و ارزش اخباری مثبت روش پایین است، بنابراین میتوان از این دو بهعنوان روش کمکی در کنار ماموگرافی و یا سونوگرافی استفاده کرد.

نقش رادیکالهای آزاد و آنتیاکسیدانها در اختلالات باروری مردان و زنان: مقاله مروری
مقدمه: رادیکالهای آزاد بر اساس سطح فیزیولوژیکی خود در بدن نقش دوگانه ایفا میکنند و میتوانند مفید یا مضر باشند. مطالعه مروری حاضر با هدف بررسی مکانیسمهای سلولی و مولکولی استرس اکسیداتیو بر اسپرماتوژنز و اووژنز و نقش استفاده از آنتیاکسیدانها در بهبود باروری در مردان و زنان انجام شد.
روشکار: در این مقاله مروری مطالب مرتبط با نقش رادیکالهای آزاد و آنتیاکسیدانها در اختلالات باروری مردان و زنان به زبان فارسی و انگلیسی در پایگاههای اطلاعاتی از جمله Elsevier، Scopus، Irandoc، SID،Iran medex، Pubmed، Google scholar، Science direct و Web of Science با استفاده از کلمات کلیدی رادیکال آزاد، ناباروری مردان، ناباروری زنان و آنتیاکسیدان بدون در نظر گرفتن بازه زمانی مورد جستجو قرار گرفتند. در نهایت 63 مقاله وارد مطالعه مروری حاضر شدند.
یافتهها: تولید بیش از حد رادیکالهای آزاد در دستگاه تناسلی مردانه باعث آسیب بهDNA ، RNA و غشای اسپرم و در نهایت اختلال در فرآیند اسپرماتوژنز میگردد و در دستگاه تناسلی زنانه نیز باعث ایجاد اختلالاتی همانند نقص در تکامل و بلوغ اووسیت، رشد فولیکول و عملکرد جسم زرد میشود.
نتیجهگیری: بر اساس یافتههای مطالعات قبلی، ممکن استمصرفآنتیاکسیدانها موجب برطرف کردن آثار مضر رادیکالهای آزاد تولید شده بر بارداری مردان و زنان شود.
