مطالعه سطح سرمی Angiopoietin-like protein 4 در زنان مبتلا به سندرم تخمدان پلیکیستیک با توده بدنی نرمال
مقدمه: سندرم تخمدان پلیکیستیک (PCOS) یک بیماری اندوکرین پیچیده در زنان میباشد و تقریباً 75% از زنان PCOS دچار افزایش وزن هستند. Angiopoietin-like protein 4 (ANGPTL4) مهارکننده قوی آنزیم لیپوپروتئین لیپاز (LPL) میباشد. این پروتئین تعیینکننده غلظت و کلیرانس تریگلیسرید پلاسما میباشد. مطالعه حاضر با هدف بررسی سطح سرمی ANGPTL4 در زنان PCOS با شاخص توده بدنی نرمال انجام شد.
روشکار: این مطالعه مورد- شاهدی در سال 95-1394 بر روی 47 زن مراجعه کننده به درمانگاه ابوالفضل وابسته به دانشگاه علوم پزشکی بوشهر انجام شد. در این مطالعه 25 زن PCOSبه روش غیراحتمالی آسان و بر اساس معیارهای تشخیصی روتردام (2003) به عنوان گروه مورد و 22 زن سالم به عنوان گروه شاهد انتخاب شدند. سطح سرمی تمام هورمونها به کمک تکنیک الایزا و پارامترهای بیوشیمیایی نیز به کمک اتوآنالیزر اندازهگیری شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS(نسخه 16) و آزمونهای تی مستقل، من ویتنی و رگرسیون خطی انجام شد. میزان p کمتر از 05/0 معنادار در نظر گرفته شد.
یافتهها: نتایج این مطالعه افزایش معناداری در میزان LH (003/0=p)، LH/FSH(007/0=p)، FBS (03/0=p)، تستوسترون (02/0=p) و پرولاکتین (005/0=p) در گروه مورد نشان داد. تفاوت معناداری در میزان ANGPTL4، انسولین، HOMA-IR، لیپوپروتئینهای با چگالی پایین (LDL)، HDL، HOMA-B و کراتینین در گروه مورد نسبت به گروه شاهد مشاهده نشد. ارتباط معکوس و معناداری بین سطح سرمی ANGPTL4 با لگاریتم انسولین (002/0=p)، FBS (006/0=p)، لگاریتم HOMA-IR (001/0=p) و شاخص توده بدنی (03/0=p) وجود داشت، اما ارتباطی بین ANGPTL4 و پارامترهای لیپیدی مشاهده نشد.
نتیجهگیری: در مطالعه حاضر تفاوت معناداری در سطح سرمی ANGPTL4 در زنان مبتلا به تخمدان پلیکیستیک با توده بدنی نرمال در مقایسه با گروه شاهد مشاهده نشد.

بررسی فراوانی کم خونی در زنان باردار مراجعه کننده به مراکز جامع سلامت شهر یزد (96-1395)
مقدمه: کمخونی در بارداری از شیوع نسبتاً بالایی برخوردار است. هرچند آنمی خفیف منجر به عوارض قابل توجه جنینی نمیگردد، ولی آنمی شدید (هموگلوبین زیر 7)، میتواند منجر به آثار سوء بر مادر و جنین و حتی افزایش مرگومیر آنها شود، مطالعه حاضر با هدف تعیین شیوع کمخونی و عوامل مرتبط با آن در زنان باردار یزدی و برخی عوارض نوزادی آن انجام شد.
روشکار: این مطالعه توصیفی و مقطعی در سال 96-1395 بر روی 550پرونده زن باردار مراجعه کننده به مراکز بهداشتی درمانی شهر یزد انجام شد. دادههای مربوط به مراقبتهای بارداری با استفاده از فرم طراحی شده از پرونده افراد استخراج شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماریSPSS (نسخه 16) و آزمونهای کای دو و تی تست انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: در این مطالعه شیوع آنمی در زنان باردار شهر یزد 2/7% بود. 25 نفر (5/4%) در سه ماهه اول و 22 نفر (4%) در سه ماهه دوم بارداری آنمی داشتند. وجود آنمی با سن، سطح تحصیلات مادر، شغل مادر، جنسیت نوزاد، نوع زایمان، وجود بارداری پرخطر و وزن نوزاد در بدو تولد، ارتباط معناداری نداشت (05/0p>)، ولی شیوع آنمی در مناطق مختلف شهری متفاوت بود و دور سر نوزادان مادران آنمیک به طور معنیداری کمتر از نوزادان مادران بدون آنمی بود (01/0p<).
نتیجهگیری: بهنظر میرسد شیوع آنمی در شهر یزد در مقایسه با اغلب مناطق کشور پایینتر است که یکی از علل آن میتواند گسترش پوشش خدمات نظام سلامت در نقاط شهری و اجرای برنامههای مداخلهای پیشگیرانه باشد.

مقایسه تأثیر میزوپروستول واژینال و سوند سرویکال به تنهایی و مصرف توأم جهت آمادهسازی سرویکس در زنان نولیپار
مقدمه: استفاده از میزوپرستول و سوند سرویکال، روشهای اثبات شده و مؤثری جهت آمادهسازی سرویکس هستند؛ در مطالعات انجام شده نتایج متناقضی از مقایسه این دو روش و روش ترکیبی بر آمادهسازی سرویکس بهدست آمده است، لذا مطالعه حاضر با هدف مقایسه تأثیر میزوپروستول واژینال و سوند سرویکال به تنهایی و مصرف توأم در بیمارستان آیت الله موسوی شهر زنجان انجام شد.
روشکار: این مطالعه کارآزمایی بالینی تصادفی شده در سال 93-1392 بر روی 85 زن باردار ترم مراجعه کننده به بیمارستان آیت الله موسوی شهر زنجان که با اندیکاسیون ختم بارداری بستری شده بودند، انجام شد. بیماران بهطور تصادفی در سه گروه استفاده از میزوپروستول، سوند سرویکال و استفاده همزمان این دو روش قرار گرفتند. در گروه اول 25 میکروگرم میزوپروستول واژینال هر 4 ساعت تا حداکثر 4 دوز (در مجموع 100 میکروگرم)، در گروه دوم از سوند سرویکال شماره 18 با 50 سیسی نرمال سالین در سوراخ داخلی سرویکس و در گروه سوم، از هر دو روش بهطور همزمان استفاده شد. سپس طول مدت شروع روش تا زایمان، طول مدت فاز نهفته و فعال لیبر، نوع زایمان، عوارض جنینی و مادری در سه گروه مقایسه شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 22) و آزمونهای کی دو و آنالیز واریانس یکطرفه انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: در بیماران سه گروه، از نظر نوع زایمان، طول مدت فاز فعال لیبر، مکونیال، دیسترس جنینی، کوریوآمنیونیت و میزان تاکی سیستولی رحم تفاوت معناداری مشاهده نشد (05/0p>). طول فاز نهفته لیبر در روش سوند سرویکال و روش توأم در مقایسه با میزوپروستول به تنهایی بهطور معناداری کمتر بود (به ترتیب 002/0=p، 001/0=p). همچنین طول مرحله اول زایمان در روش توأم نسبت به سوند سرویکال بهطور معناداری کمتر بود (009/0=p).
نتیجهگیری: در موارد نیاز به ختم بارداری با سرویکس ناآماده، روش ترکیبی تأثیر بیشتری نسبت به سوند سرویکال در جهت کوتاهتر شدن طول مدت مرحله اول زایمان دارد.

بررسی تأثیر تماس پوستی بلافاصله و مداوم مادر و نوزاد پس از تولد بر طول مرحله سوم زایمان: کارآزمایی بالینی تصادفی شده
مقدمه: مهمترین مداخله برای پیشگیری از طولانی شدن مرحله سوم زایمان، تجویز داروهای صناعی است که با تمام کارایی، اثرات نامطلوبی را به همراه دارند. بر این اساس نیاز به جایگزینی با روشهای کم خطرتر احساس میشود. مطالعه حاضر با هدف بررسی تأثیر تماس پوستی بلافاصله و مداوم مادر و نوزاد پس از تولد بر طول مرحله سوم زایمان انجام شد.
روشکار: این مطالعه کارآزمایی بالینی تصادفی در سال 1396 بر روی 92 مادر نخست باردار در زایشگاه بیمارستان امالبنین (س) شهر مشهد انجام شد. در گروه تماس پوستی، نوزاد پس از تولد بدون پوشش، در تماس پوستی با مادرش قرار میگرفت. در گروه مراقبت مرسوم، نوزاد به زیر وارمر منتقل میشد و پس از ترمیم پرینه، در داخل پتو تحویل مادر میشد. اطلاعات با استفاده از پرسشنامه جمعآوری گردید و دو گروه جهت ارزیابی طول مرحله سوم زایمان و میزان نیاز به اکسیتوسین صناعی مورد پیگیری قرار گرفتند. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 14) و آزمونهای کایدو و تی مستقل انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: طول مدت مرحله سوم زایمان در 11 نفر (4/23%) از مادران گروه تماس پوستی و 3 نفر (7/6%) از گروه مراقبت مرسوم، کمتر از 5 دقیقه و در 6 نفر (8/12%) از گروه تماس پوستی و 13 نفر (9/28%) از گروه مراقبت مرسوم، بین 20-10 دقیقه بود که بین دو گروه تفاوت آماری معنیداری وجود داشت (02/0=p). همچنین نیاز به اکسیتوسین صناعی پس از زایمان در گروه تماس پوستی بهطور معنیداری کمتر از گروه کنترل بود (03/0=p).
نتیجهگیری: تماس پوستی مادر و نوزاد، طول مرحله سوم پس از زایمان را کاهش میدهد و از نیاز به دوز بالای اکسیتوسین صناعی و عوارض آن میکاهد. میتوان با برقراری تماس پوستی بهعنوان یک روش مؤثر، ساده، ارزان و بدون عارضه جانبی به تسریع مرحله سوم زایمان کمک نمود و در نهایت از خونریزی پس از زایمان بهعنوان یکی از علل شایع مرگ مادر کاست.

مقایسه تأثیر آموزش مبتنی بر شبیهسازی، تلفیقی و سخنرانی بر مهارت شناختی ماماها در اداره پرهاکلامپسی و اکلامپسی
مقدمه: پرهاکلامپسی به عنوان یک مشکل جهانی سلامت زنان مطرح شده است. آموزش ماماها جهت کسب صلاحیت لازم برای مدیریت بحرانهای مامایی از جمله پرهاکلامپسی و اکلامپسی ضروری میباشد، لذا مطالعه حاضر با هدف مقایسه تأثیر آموزش مبتنی بر شبیهسازی، تلفیقی و سخنرانی بر مهارت شناختی ماماها در اداره پرهاکلامپسی و اکلامپسی انجام شد.
روشکار: این مطالعه کارآزمایی بالینی در سال 1395 بر روی 90 ماما در بیمارستانهای منتخب شهر مشهد انجام شد. ماماها با استفاده از جدول اعداد تصادفی به سه گروه آموزش شبیهسازی، تلفیقی و سخنرانی تقسیم شدند. گروه شبیهسازی به مدت 6 ساعت در مرکز مهارتهای بالینی، گروه تلفیقی به مدت 4 ساعت سخنرانی و سپس به مدت یک هفته از طریق وب سایت آموزشی و گروه سخنرانی به مدت 6 ساعت به صورت سخنرانی آموزش دیدند. قبل و 2 هفته بعد از مداخله، آزمون مهارت شناختی گرفته شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 23) و آزمونهای تی زوجی، آنالیز واریانس یکطرفه، ویلکاکسون و کروسکال والیس انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: میانگین نمره مهارت شناختی ماماها در هر سه گروه قبل از آموزش تفاوت آماری معنیداری نداشت (05/0<p). میانگین نمره مهارت شناختی ماماها 2 هفته بعد از آموزش در هر سه گروه افزایش معنیداری داشت (001/0>p) و بر اساس نتایج مقایسه بینگروهی، میانگین نمره مهارت شناختی در گروه تلفیقی و سخنرانی بهطور معنیداری بیشتر از گروه شبیهسازی بود (001/0>p)، اما اختلاف معنیداری در میانگین نمره مهارت شناختی بین دو گروه تلفیقی و سخنرانی وجود نداشت (869/0=p).
نتیجهگیری: آموزش مبتنی بر شبیهسازی، تلفیقی و سخنرانی، باعث افزایش مهارت شناختی ماماها در اداره پرهاکلامپسی و اکلامپسی گردید که در گروه آموزش تلفیقی و سخنرانی بیشتر از گروه شبیهسازی بود و با توجه به اینکه روش تلفیقی، روشی فعال و فراگیر محور است، بنابراین میتوان از روش تلفیقی برای آموزش اورژانسهای مامایی استفاده کرد.

شناسایی جهشهای اگزون ۶، ۱۳ و ۲۰ ژن BRCA1 در بیماران مبتلا به سرطان پستان در استان خراسان جنوبی
مقدمه:سرطان پستان، شایعترین بدخیمی در زنان و بهعنوان اولین علت مرگومیر ناشی از سرطان در زنان شناخته میشود.این بیماری به شدت ناهمگن بوده و تحت تأثیر عوامل ژنتیکی و محیطی بهوجود میآید. جهش در دو ژن بسیار پرنفوذ BRCA1 و BRCA2 که نقش مهمی در ترمیم DNA دارند، علت اصلی 30-20% سرطانهای پستان ارثی شناخته شده است. مطالعه حاضر با هدف بررسی فراوانی جهشهای ژن BRCA1 در افراد مبتلا به سرطان پستان در جمعیت خراسان جنوبی انجام شد. روشکار:این مطالعه توصیفی بین سالهای 96-1394 بر روی ۸۸ بیمار مبتلا به سرطان پستان شهر بیرجند انجام شد. از افراد نمونه خون جمعآوری شد. قطعات مورد نظر ژن BRCA1 با استفاده از DNA استخراج شده از بیماران و تکنیک PCR تکثیر گردید. برای مشخص کردن جهشهای اگزون ۶، ۱۳ و ۲۰ از تکنیک PCR-SSCP، رنگآمیزی نیترات نقره و توالییابی مستقیم DNA استفاده شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه ۲۲) انجام گرفت. یافتهها: بررسی اگزون ۶ و ۲۰ در بیماران هیچ جهشی را نشان نداد. در بررسی اگزون ۱۳ یک پلیمورفیسم مشاهده شد. این واریانت c.4308T>C در ۲۸ نمونه تشخیص داده شد که 4 نفر (2/14%) آنها مبتلا به سرطان پستان ارثی بودند. فراوانی جهش در افراد مبتلا به سرطان پستان وراثتی 3/36% و در افراد مبتلا به سرطان پستان غیر وراثتی 1/31% در جمعیت مورد مطالعه بود. نتیجهگیری: در این مطالعه میزان بروز جهش در دو اگزون ۶ و ۲۰ از ژن BRCA1 صفر و یک جهش از نوع پلیمورفیسم rs1060915در اگزون ۱۳ از ژن BRCA1 با فراوانی 8/31%مشاهده شد.

بررسی تأثیر مشاوره شناختی رفتاری گروهی بر افسردگی حین بارداری: یک کارآزمایی بالینی شاهددار
مقدمه: زنان باردار از لحاظ سلامت روان، حساس و آسیبپذیر میباشد. از این رو ممکن است روان درمانی یک مداخله مؤثر جهت بهبود وضعیت روانی مادر باشد، لذا مطالعه حاضر با هدف بررسی تأثیر درمان شناختی رفتاری گروهی بر افسردگی حین بارداری انجام شد.
روشکار: این مطالعه کارآزمایی بالینی در سال 96-1395 بر روی 60 زن باردار که در سن بارداری 22-12 هفته به مراکز سلامت کننده شهر قزوین مراجعه کرده بودند، انجام گرفت. تمام مادران منتخب بر اساس پرسشنامه بک دارای افسردگی در حد خفیف یا متوسط بودند که بهصورت تصادفی به دو گروه 30 نفری آزمون و شاهد تقسیم شدند. گروه آزمون به مدت 6 هفته تحت درمان شناختی- رفتاری گروهی قرار گرفتند و گروه شاهد هیچ آموزشی دریافت نکردند. بلافاصله بعد از اتمام دوره و 2 ماه بعد، مجدداً آزمون بک بهعنوان پس آزمون در تمام افراد اجرا گردید. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 23) و آزمون ANOVA و کای دو انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: بر اساس نتایج آزمون منویتنی، تفاوت معنیداری بین گروه آزمون و شاهد در مراحل پس آزمون (001/0=p) و پیگیری (001/0p≤) وجود داشت. همچنین بر اساس تحلیل ANOVA در گروه آزمون از لحاظ کاهش علائم افسردگی (001/0p≤) در مرحله پس آزمون و پیگیری، تفاوت معنیداری وجود داشت که این تفاوتها در گروه شاهد مشاهده نشد.
نتیجهگیری: درمان شناختی- رفتاری گروهی، میتواند بهعنوان یک رویکرد درمانی در بهبود افسردگی زنان باردار مؤثر باشد.

تأثیر تمرینات کگل، ثبات دهنده مرکزی و ترکیبی بر استقامت عضلات مرکزی و کیفیت زندگی زنان نخستزا بعد از اپیزیاتومی
مقدمه: فعالیت ورزشی با اولویت تمرینات ترکیبی، ثباتدهنده مرکزی و کگل، به عنوان روشی آسان، کم هزینه، مؤثر و ایمن برای بهبود کیفیت زندگی پس از زایمان زنان نخستزا محسوب میشود. مطالعه حاضر با هدف بررسی تأثیر تمرینات کگل، ثبات دهنده مرکزی و ترکیبی بر استقامت عضلات مرکزی و کیفیت زندگی زنان نخستزا بعد از اپیزیاتومی انجام شد.
روشکار: اینمطالعه کارآزمایی بالینی در سال ۱۳۹۵ بر روی 36 زن نخستزا در شهرستان نجفآباد انجام شد. آزمودنیها بهطور تصادفی به سه گروه تمرینات کگل، گروه تمرینات ثبات مرکزی و گروه ترکیبی تقسیم شدند. سپس آزمودنیهای هر گروه برنامه تمرین مورد نظر را در مدت 6 هفته و هر هفته 3 جلسه انجام دادند. متغیرهای مورد نظر شامل استقامت عضلات تنه و کیفیت زندگی در دو نوبت قبل و بعد از انجام تمرینات اندازهگیری شد. برای ارزیابی کیفیت زندگی مرتبط با تندرستی نیز از پرسشنامه SF-36 استفاده شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 21) و تست لون، آزمون کولموگروف- اسمیرنوف و تحلیل واریانس انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: تمرینات ترکیبی در تمام شاخصها، بیشترین اثرگذاری را داشت. تأثیر تمرینات ترکیبی بر استقامت عضلات مرکزی، بهطور معناداری بیشتر از تمرینات کگل و ثباتدهنده مرکزی بود (05/0>p)، اما در مورد کیفیت زندگی مرتبط با تندرستی، چنین رابطهای وجود نداشت و بین تأثیر هر سه روش تمرینی، اختلاف معناداری وجود نداشت (05/0<p).
نتیجهگیری: با توجه به نتایج این پژوهش، تمرینات ترکیبی بیشترین تأثیر را بر بهبود استقامت عضلانی دارد و پس از آن تمرینات ثبات دهنده مرکزی تأثیر قابل توجهی بر استقامت عضلات مرکزی بدن دارند، اما در ارتباط با کیفیت زندگی، اختلاف معناداری میان سه روش تمرینی وجود ندارد.

ردیابی هرپس سیمپلکس ویروس، کلامیدیا تراکوماتیس و مایکوپلاسما ژنیتالیوم در نمونه منی مردان نابارور در استان کرمان به روش Multiplex-PCR در سال 1395
مقدمه: عفونتهای دستگاه تناسلی - ادراری، یکی از عوامل مهم ایجاد ناباروری در مردان میباشد. این عفونتها با اختلال در پارامترها (مانند تعداد، تحرک، قابلیت حیات و مورفولوژی) و عملکرد اسپرم و همچنین ایجاد التهاب اپیدیدیم موجب ناباروری در مردان میشوند. مطالعه حاضر با هدف تعیین فراوانی عفونت هرپس ویروس، کلامیدیا تراکوماتیس و مایکوپلاسما ژنیتالیوم در مایع منی افراد نابارور با علت نامشخص انجام شد.
روشکار: در این مطالعه توصیفی - تحلیلی، مایع منی 60 مرد نابارور مراجعهکننده به مرکز درمان ناباروری کرمان در طی یک بازه زمانی 6 ماهه (از ابتدای خرداد تا انتهای آبان 1395) مورد بررسی قرار گرفت. سپس multiplex-PCR با استفاده از پرایمرهای اختصاصی جهت شناسایی هرپس ویروس، کلامیدیا تراکوماتیس و مایکوپلاسما ژنیتالیوم انجام شد. تمامی نتایج حاصل از مطالعه با استفاده از نرم افزار SPSS مورد تجزیه و تحلیل قرار گرفتند.
یافتهها: در این مطالعه در مجموع 8 نمونه (3/13%) از 60 نمونه مورد آزمایش از نظر وجود ارگانیسمهای مورد مطالعه مثبت بودند. در بین نمونههای مثبت، 3 مورد (5/37%) HSV-1، 3 مورد (5/37%) کلامیدیا تراکوماتیس (CT)، 1 مورد (5/12%) مایکوپلاسما ژنیتالیوم (Mg) و 1 مورد (5/12%) HSV-2 جدا شد.
نتیجهگیری: بررسی فراوانی ارگانیسمهای مورد مطالعه در نمونه مایع منی مردان میتواند راهنمایی برای غربالگریهای بعدی از نظر عفونتهای بدون علامت و درمان مناسب قبل از شروع درمانهای کمک باروری باشد.

بررسی اثر فرمهای فرموله شده جدید کورکومین بر مهار رشد تومور در سرطان پستان: مقاله مروری
مقدمه: سرطان پستان، شایعترین بیماری بدخیم در بین زنان و دومین علت مرگومیر ناشی از سرطان، پس از سرطان ریه در زنان میباشد. کورکومین، یک ترکیب گیاهی است که اثرات ضد سرطانی آن در مطالعات پیش بالینی و بالینی نشان داده شده است. اثرات ضد سرطانی کورکومین شامل مهار شکلگیری سلول سرطانی، مهار آنژیوژنز و مهار رشد تومور میباشد. به دلیل جذب پایین کورکومین در روده، حلالیت پایین در آب و متابولیسم سریع و دفع زیاد، فراهمی زیستی کورکومین پایین میباشد، بنابراین استفاده از کورکومین فرموله میتواند باعث افزایش فراهمی کورکومین گردد. مقاله مروری حاضر با هدف بررسی اثرات کورکومین و ترکیبات نانو فرموله آن در پیشگیری و درمان سرطان پستان انجام شد.
روشکار: در این مطالعه مروری مقالات انگلیسی و فارسی مناسب و مرتبط از پایگاههای الکترونیکی Pubmed، Scopus ،Google Scholar و Magiran تا پایان سال ۲۰۱۶ با استفاده از کلید واژههای Curcumin، Cancer، Breast و Nanoform Clinical Study و در مقالات فارسی با کلمات سرطان، پستان، زردچوبه، مطالعات بالینی و اشکال نانو بررسی و در مجموع 45 مقاله گردآوری و بررسی شدند.
یافتهها: نتایج مطالعات مختلف نشان داد که استفاده از کورکومین فرموله شده از یک سو باعث افزایش جذب و فراهمی زیستی آن شده و از سوی دیگر باعث افزایش اثربخشی آن بر سلولهای سرطانی از جمله سرطان پستان میشود که این نتایج میتواند نشان دهنده اثرات مفید ترکیبات نانوفرموله کورکومین در پیشگیری و درمان سرطان پستان باشد.
نتیجهگیری: به دلیل فراهمی زیستی بالاتر ترکیبات فرموله شده کورکومین در مقایسه با فرم آزاد کورکومین و با توجه به سمیت پایین این داروی گیاهی، میتوان از آن در کنار سایر داروهای ضد سرطان در درمان سرطان پستان استفاده کرد.

ارتباط متغیرهای مرتبط با بارداری و خطر ابتلاء به سرطان پستان: مرور سیستماتیک
مقدمه: سرطان پستان شایعترین سرطان شناخته شده در جهان است که سبب مرگومیر گسترده در زنان میگردد. در اکثر موارد عامل خطر شناخته شدهای برای مبتلایان به سرطان پستان شناسایی نشده است. به نظر میرسد تغییرات هورمونی وسیعی که در بارداری رخ میدهند، در بروز سرطان پستان نقش دارند. از آنجایی که شناسایی عوامل خطر این بیماری به پیشگیری اولیه و تشخیص زودرس سرطان پستان کمک میکند، مطالعه حاضر با هدف مروری سیستماتیک بر مطالعات انجام شده در مورد متغیرهای مرتبط با بارداری و خطر سرطان پستان در ایران و جهان انجام شد. روش کار: در این مطالعه مرور سیستماتیک، مطالعات مورد نظر از پایگاههای اطلاعاتی Google Scholar، Scopus، Pubmed، Science Direct، Cochrane و SID با کلیدواژههای سرطان پستان و متغیرهای مرتبط بارداری در بازه زمانی 2000 تا 2017 جستجو شدند. بر اساس معیار نیوکاسل اتاوا، مطالعاتی که امتیاز 6 و یا بیشتر گرفتند، وارد مطالعه شدند. نتایج به صورت کیفی گزارش شد. یافته ها: در این مطالعه 50 مقاله شامل 26 مقاله مورد شاهدی و 24 مقاله همگروهی که دارای معیار ورود به مطالعه بودند، بررسی شدند. بررسیها نشان داد در بین متغیرهای مورد بررسی، سقط و پرهاکلامپسی ارتباط قویتری با سرطان پستان دارند. نتیجه گیری: به نظر میرسد که وجود سابقه سقط باعث افزایش خطر سرطان پستان میگردد. همچنین سابقه پرهاکلامپسی نقشی محافظتکننده در سرطان پستان دارد. در مورد ارتباط دیابت بارداری و چندقلویی با سرطان پستان، نیاز به انجام مطالعات بیشتر است.

گزارش یک مورد متاستاز سرطان اپیتلیال تخمدان به سرویکس
مقدمه: سرطان تخمدان، یکی از بدخیمیهای شایع زنان میباشد و اکثر بیماران بهدنبال بروز علائم گوارشی مراجعه میکنند. انتشار این سرطان اغلب در حفره پریتو ئن صورت میگیرد، اما به صورت نادر متاستاز به سرویکس نیز گزارش شده است. در این مطالعه یک موردمتاستاز سرطان اپیتلیال تخمدان به سرویکس گزارش شد.
معرفی بیمار: خانم 58 ساله با سابقه 3 نوبت زایمان طبیعی با شکایت خونریزی بهدنبال تماس جنسی به درمانگاه انکولوژی دانشگاه علوم پزشکی مشهد در سال 1395 مراجعه کرد. بهدلیل نمای غیرطبیعی سرویکس در معاینه و سپس تأیید آن با نمونهبرداری کولپوسکوپی با تشخیص سرطان سرویکس در مراحل اولیه (‖ A) تصمیم به جراحی رادیکال شد، اما در لاپاراتومی بهدلیل وسعت درگیری داخل شکمی مشابه سرطان تخمدان و عدم امکان جراحی، فقط انجام بیوپسی از امنتوم مقدور بود. پاسخ آسیبشناسی نمونه فوق آدنوکارسینوم پاپیلاری سروز تخمدان بود. در بررسی مجدد نمونه آسیبشناسی سرویکس، متاستاز سرطان تخمدان به سرویکس تأیید شد، لذا با تشخیص سرطان تخمدان مرحله پیشرفته، شیمی درمانی (تاکسول-کربوپلاتین) و سپس جراحی مرحلهبندی سرطان تخمدان انجام شد و بیمار تحت پیگیری دورهای سرطان تخمدان قرار گرفت. متأسفانه در مدت کمتر از 6 ماه با عود سرطان تخمدان در شکم و لگن مراجعه کرد و فعلاً تحت شیمی درمانی است.
نتیجهگیری: در سرطان تخمدان وقوع متاستاز به سرویکس در زمان تشخیص و حتی در مرحله پیشرفت بیماری غیر معمول است. خونریزی بعد از تماس جنسی به عنوان اولین علامت سرطان تخمدان، یافته نادری است، لذا در بیماران با سرطان سرویکس احتمال متاستاتیک بودن ضایعه از ساختمانهای مجاور را نیز باید در نظر داشت.
