Midwives or obstetricians may need to make a surgical incision (episiotomy) to increase the diameter of the vaginal outlet and facilitate the neonate’s birth. The aim of this study was to compare perineal lacerations in routine versus selective episiotomy. Methods: The current study was performed on 986 nulliparous women, admitted to Besat Hospital of Sanandaj in 2010-2011. Patients were randomly allocated to routine and selective episiotomy (only in case of vacuum-assisted delivery or fetal distress) groups. For data analysis, t-test, Chi-square, and Fisher's exact test were performed, using SPSS version 15. P-value less than 0.05 was considered statistically significant. Results: duration of the second stage of labor, first-degree tears, and labia minora lacerations in selective episiotomy was significantly greater than the routine episiotomy group (P=0.0001). However, there was no significant difference between the two groups regarding second-, third-, or fourth-degree lacerations. Conclusion: Routine mediolateral episiotomy could not prevent severe perineal lacerations but failure to do this in all cases (performed only in selected cases) resulted in first-degree lacerations and anterior perineal trauma. So it seems, limited use of episiotomy is a more rational approach for managing perineal outcomes during labor.}, keywords = {Mediolateral Episiotomy,Nulliparous,Perineal Laceration,Selective episiotomy}, title_fa = {مقایسه پارگی های پرینه در اپی زیاتومی های مدیولاترال معمول و انتخابی در زنان مراجعه کننده به بلوک زایمان بیمارستان بعثت سنندج، 1390}, abstract_fa = {مقدمه: پارگی های واژن در حین زایمان شایع بوده و ممکن است به طور خود به خود اتفاق بیفتد و یا ماما و متخصص مامایی جهت افزایش قطر خروجی واژن به منظور تسهیل تولد نوزاد نیاز به برش جراحی (اپی زیاتومی) پیدا کنند. مطالعه حاضر با هدف مقایسه درجات پارگی های پرینه در اپی زیاتومی معمول با انتخابی انجام شد. روش کار: این مطالعه آینده نگر در سال 90-1389 بر روی 986 زن باردار شکم اول مراجعه کننده جهت زایمان به بیمارستان بعثت سنندج انجام شد. افراد به طور تصادفی در دو گروه اپی زیاتومی روتین و اپی زیاتومی انتخابی (در صورت استفاده از واکیوم یا دیسترس جنینی) قرار گرفتند. تجزیه و تحلیل داده ها با استفاده از نرم افزار آماری SPSS (نسخه 15) و آزمون های تی تست، کای اسکوئر و تست دقیق فیشر انجام شد. میزان p کمتر از 05/0 معنی دار در نظر گرفته شد. یافته ها: طول مدت مرحله دوم زایمان، پارگی درجه یک و پارگی لیبیا مینورها در گروه اپی زیاتومی انتخابی به طور معناداری بیشتر از گروه اپی زیاتومی معمول بود (0001/0=p). نتایج مطالعه، اختلاف آماری معنی داری را در میزان پارگی های درجه 2، 3 و 4 در دو گروه اپی زیاتومی معمول و انتخابی نشان نداد. نتیجه گیری: اپی زیاتومی مدیولاترال معمول از پارگی های شدید پرینه جلوگیری نمی کند اما عدم انجام آن در تمام موارد (انجام فقط در موارد انتخابی) منجر به پارگی های درجه یک و قسمت قدامی پرینه می شود. بنابراین به نظر می رسد که محدودیت کاربرد اپی زیاتومی، روش منطقی تری برای کنترل پرینه مادر در حین زایمان باشد.}, keywords_fa = {اپی زیاتومی انتخابی,اپی زیاتومی مدیولاترال,پارگی پرینه,نولی پار}, url = {https://ijogi.mums.ac.ir/article_3430.html}, eprint = {https://ijogi.mums.ac.ir/article_3430_49f8d00b1af856f93b63b4d77ba27723.pdf} }
