This study was performed with aim to evaluate the prevalence of pregnancy complications in women with different phenotypes of polycystic ovary syndrome based on the NIH in addition to Rotterdam criteria.  Methods: In this retrospective study, the pregnancy records of 266 PCOS women were investigated based on their medical file in 2014-2016. These women based on PCOS related clinical symptoms were divided to two groups: The first group, or group A (severe phenotype), included women who had all three clinical signs of oligo-anovulation, hyper-androgenism, and ovarian cysts, or only two signs of oligo-ovulation and hyper-androgenism when the disease was diagnosed. Group B included people who had only hyper-androgenism with ovarian cysts (PCOM) or oligovulation with ovarian cysts (mild phenotype).The maternal and fetal complications during pregnancy was retrospectively evaluated between two groups based on the participants’ medical records information. Data were analyzed by SPSS software (version 17) and Mann-Whitney, Chi-square, T test and Logistic Regression. P<0.05 was considered statistically significant. Results: Among maternal and fetal complications, low birth weight was significantly different between the two groups. After adjusting with age, body mass index and number of pregnancies, the odds ratio of low birth weight baby in group A compared to group B was 2.6 (95% CI; 1.5-2.9, p <0.01). The study found no statistically significant difference in gestational diabetes, preterm delivery, or the chance of spontaneous abortion between different PCOS phenotypes. Conclusion: The chance of LBW in women with sever form of PCOS (group A) is higher than women with moderate form of the disease (group B). In clinical practice, it is recommended that the PCOS phenotype should be considered as an affecting factor on fetal outcomes in pregnancy.}, keywords = {Pregnancy complications,PCOS phenotypes,Polycystic ovary syndrome}, title_fa = {بررسی مقایسه‌ای عوارض مادری و جنینی در زنان زایمان کرده مبتلا به فنوتیپهای مختلف سندرم تخمدان پلی‌کیستیک}, abstract_fa = {مقدمه: شیوع عوارض مادری و جنینی در زنان باردار مبتلا به سندرم تخمدان پلی‌کیستیک (PCOS) نسبت به زنان باردار غیرمبتلا بالاتر است. مطالعه حاضر با هدف بررسی میزان شیوع عوارض دوران بارداری در فنوتیپ­های مختلف زنان باردار مبتلا به PCOS (بر مبنای معیارهای انستیتوی ملی سلامت با فنوتیپ­های اضافه شده برحسب معیار روتردام) انجام شد. روش­‌کار: در این مطالعه گذشته‌نگر اطلاعات مربوط به دوران بارداری 266 زن مبتلا به PCOS بر اساس پرونده پزشکی آنها 95-1393 مورد بررسی قرار گرفت. این زنان بر مبنای علائم بالینی مرتبط با PCOS به دو گروه تقسیم شدند. گروه اول یا گروه A (فنوتیپ شدید) شامل افرادی بودند که هنگام تشخیص بیماری هر سه علامت بالینی الیگو-آن‌اوولاسیون، هیپرآندروژنیسم و کیست­های تخمدانی یا تنها دو علامت الیگواوولاسیون و هیپرآندروژنیسم را داشتند. گروه B شامل افرادی بود که فقط هیپرآندروژنیسم همراه کیست­های تخمدانی (PCOM) یا الیگواوولاسیون همراه با کیست تخمدانی را داشتند (فنوتیپ خفیف). مقایسه عوارض مادری و جنینی دوران بارداری به‌صورت گذشته‌نگر و بر مبنای اطلاعات پرونده در بین دو گروه انجام شد. تجزیه و تحلیل با استفاده از نرم‌افزار آماری SPSS (نسخه 17) و آزمون­های تی تست، من‌ویتنی، خی دو و رگرسیون لوجستیک انجام شد. میزان p کمتر از 05/0 معنی‌دار در نظر گرفته شد. یافته‌ها: از بین عوارض مادری و جنینی، وزن کم هنگام تولد در بین زنان شرکت‌کننده در دو گروه تفاوت معنی‌داری داشت که پس از تعدیل با سن، شاخص توده بدنی و تعداد بارداری نسبت شانس تولد نوزاد با وزن کم هنگام تولد در گروه A نسبت به گروه B، 6/2 (01/0>p، 9/5-2/1؛ 95%:CI) بود. در این مطالعه تفاوت آماری معنی‌داری از ابتلاء به دیابت دوران بارداری، زایمان زودرس و شانس سقط خودبه‌خودی بین فنوتیپ­های مختلف PCOS مشاهده نشد. نتیجه‌گیری: شانس تولد نوزادان با وزن کم در زنان مبتلا به فنوتیپ شدیدتر بیماری (گروه A) PCOSدر مقایسه با زنان مبتلا به فنوتیپ خفیف‌تر بیماری (گروه B) بالاتر می­باشد. بنابراین در ملاحظات بالینی در زنان باردار مبتلا به PCOS، در نظر گرفتن فنوتیپ بیماری به‌عنوان یکی از عوامل تأثیرگذار در پیامدهای جنینی توصیه می­گردد.}, keywords_fa = {سندرم تخمدان پلی‌کیستیک,عوارض حاملگی,فنوتیپ­های سندرم تخمدان پلی‌کیستیک}, url = {https://ijogi.mums.ac.ir/article_15998.html}, eprint = {https://ijogi.mums.ac.ir/article_15998_b37bb96aeb1b65cc8eb69ff0e35cc449.pdf} }
