Cervical status, determined by Bishop's score, is one of the factors that influence the duration of the latent phase. This study was performed with aim to determine the effect of topical magnesium sulfate in latent phase on Bishop Score, latent phase duration and delivery outcomes in primiparous women. Methods: This double-blind clinical trial study was conducted on 60 nulliparous women in Izadi hospital of Qom in 2018. The participants were randomly divided into two groups of magnesium sulfate and placebo (n=30 in each group). Before the intervention, vaginal examination was performed and Bishop's score was calculated. If Bishop's score was less than 5, the individual would be included in the study. In the intervention group, 10cc of magnesium sulfate 50% was administered during the examination and in the control group, 10 cc distilled water were poured on the cervix. Vaginal examination was done every 4 hours. Finally, the Bishop scores, duration of latent phase, and overall duration of latent phase until delivery were calculated and compared in the two groups. Data were analyzed by SPSS software (version 24) and Chi-square, Fisher, Independent t, and Mann-Whitney tests. P<0.05 was considered statistically significant. Results: Before the intervention, there was not significantly different between the two groups in mean Bishop score (p= 0.554). However, in subsequent examinations which were done 4 (p=0.001) and 8 (p=0.001) hour after the intervention, these differences were significant. Duration of latent phase in the intervention group was 636.87± 455.55 minutes and in the control group was 1209.17±688.52 minutes (p=0.004). Also, the total duration of labor was 1032.54±585.20 minutes in the intervention group and 1524.28±531.99 minutes in the control group (p=0.001). Two groups had no significant difference in terms of other delivery outcomes (p≥0.05). Conclusion: The use of topical magnesium sulfate at the beginning of the latent phase improves Bishop's score, thereby shortening the latent phase duration and subsequently shortening the labor duration.}, keywords = {Latent phase,Bishop Score,Delivery,Primiparous}, title_fa = {بررسی تأثیر سولفات منیزیم موضعی در فاز نهفته بر بیشاپ اسکور و طول مدت فاز نهفته در زنان نخست‌زا}, abstract_fa = {مقدمه: مانند مرحله فعال زایمان، طولانی شدن مرحله نهفته نیز برای مادر و جنین با مخاطراتی همراه است. وضعیت سرویکس که با تعیین نمره بیشاپ مشخص می­شود، یکی از عواملی است که بر طول مدت مرحله نهفته مؤثر است. مطالعه حاضر با هدف تعیین تأثیر سولفات منیزیم موضعی در فاز نهفته، بر بیشاپ اسکور، طول مدت فاز نهفته و پیامدهای زایمان در زنان نخست‌زا انجام شد. روش‌کار: این مطالعه کارآزمایی بالینی دوسوکور در سال 1397 بر روی 60 زن نخست‌زا در بیمارستان ایزدی شهر قم انجام شد. افراد به‌طور تصادفی در دو گروه 30 نفره کنترل و سولفات منیزیم قرار گرفتند. قبل از مداخله، معاینه واژینال انجام و نمره بیشاپ محاسبه می­شد. در صورتی که نمره بیشاپ از 5 کمتر می­بود، فرد وارد مطالعه می­شد. در گروه مداخله حین معاینه 10 سی‌سی سولفات منیزیم 50% و در گروه کنترل 10 سی‌سی آب مقطر روی سرویکس ریخته می­شد. هر 4 ساعت معاینه واژینال انجام می­شد. در نهایت در دو گروه مقادیر نمره بیشاپ، طول مدت فاز نهفته و مدت کلی شروع مرحله نهفته تا انجام زایمان محاسبه و با هم مقایسه شدند. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار آماری SPSS (نسخه 24) و آزمون­های کای دو، فیشر، تی مستقل و من‌ویتنی انجام شد. میزان p کمتر از 05/0 معنی‌دار در نظر گرفته شد. یافته‌ها: قبل از مداخله، میانگین نمره بیشاپ در دو گروه تفاوت معناداری نداشت (547/0=p)، اما طی معاینه­های بعدی که 4 (001/0=p) و 8 ساعت (001/0≥p) بعد از مداخله انجام شد، این تفاوت­ها از نظر آماری معنادار بود. طول مدت مرحله نهفته در گروه مداخله 55/455±87/636 و در گروه کنترل 52/688±17/1209 دقیقه بود (004/0=p). همچنین طول مدت کلی زایمان در گروه مداخله 20/585±54/1032 دقیقه و در گروه کنترل 99/531±28/1524 دقیقه بود (001/0=p). از نظر سایر پیامدهای زایمانی دو گروه تفاوت معناداری نداشتند (05/0p≥). نتیجه‌گیری: استفاده از سولفات منیزیم موضعی در ابتدای فاز نهفته، موجب بهبود نمره بیشاپ شده و بدین‌ترتیب طول مدت فاز نهفته کوتاه‌تر شده و به‌دنبال آن سیر لیبر نیز کوتاه‌تر می­شود.}, keywords_fa = {بیشاپ اسکور,زایمان,فاز نهفته,نخست‌‌زا}, url = {https://ijogi.mums.ac.ir/article_15962.html}, eprint = {https://ijogi.mums.ac.ir/article_15962_bffd5b7f5c9d04f7da4ac19790cfc451.pdf} }
