Preventive analgesia is an effective approach in controlling this pain. This study was performed with aim to evaluate the efficacy of preventive analgesia and side effects of using acetaminophen, meperidine, and their combination after elective cesarean section under spinal anesthesia. Methods: This double-blind randomized study was conducted on 120 full-term pregnant women with ASA class I undergoing elective Cesarean Section under spinal anesthesia at Shiraz Hafez hospital in 2017. The patients were randomly divided into three groups of each containing 40 patients. After delivery, during the clamping of umbilical cord and before the end of surgery, the drugs were injected to the patients. The patients received 15 mg/kg intravenous acetaminophen, 0.5 mg/kg intravenous meperidine, and a combination of them, respectively. After clamping the umbilical cord, the groups were compared in terms of postoperative pain severity and complications. Data were analyzed by SPSS software (version 22) and ANOVA and Chi-square tests. P<0.05 was considered statistically significant. Results: The mean score of pain severity during the first six hours after the operation was significantly lower in the acetaminophen group compared to the meperidine group (P<0.001). The mean score of pain severity, duration of recovery, and time interval between the initial and the next administration of additional analgesics doses was significantly lower in meperidine plus acetaminophen group compared with other groups (P<0.001). Moreover, incidence of postoperative headache was significantly higher in meperidine group and shivering was significantly higher acetaminophen group (P=0.005). Conclusion: Acetaminophen had a higher efficacy in postoperative pain management compared to meperidine, and their combination had greater analgesic effect and lower side effects.}, keywords = {Acetaminophen,Cesarean section,Meperidine,Postoperative pain}, title_fa = {مقایسه اثر استامینوفن، مپریدین و ترکیب آنها در درمان درد محل جراحی پس از عمل جراحی سزارین انتخابی}, abstract_fa = {مقدمه: مدیریت درد بعد از جراحی، از مهم‌ترین چالش‌های بعد از سزارین می­باشد. یک راهکار مؤثر در کنترل درد، پیشگیری از درد می­باشد. مطالعه حاضر با هدف ارزیابی اثربخشی پیشگیری از درد و عوارض جانبی استفاده از استامینوفن، مپریدین و ترکیب آنها بعد از عمل سزارین انتخابی تحت بی‌حسی نخاعی انجام شد. روش‌کار: این مداخله دوسوکور تصادفی در سال 1396 بر روی 120 زن با بارداری کامل با طبقه‌بندی I انجمن بیهوشی آمریکا تحت عمل جراحی سزارین انتخابی با بی‌حسی نخاعی در بیمارستان حافظ شهر شیراز انجام شد. بیماران به‌طور تصادفی به سه گروه 40 نفری تقسیم شدند که پس از زایمان، زمانی که بندناف بسته شد، قبل از پایان عمل جراحی، دارو مورد مطالعه به بیماران تزریق می‌شد. بیماران به‌ترتیب 15 میلی‌گرم بر کیلوگرم استامینوفن وریدی، 5/0 میلی‌گرم/ کیلوگرم مپریدین وریدی و ترکیبی از هر دو را دریافت می‌کردند. پس از بستن بندناف، گروه­ها با توجه به شدت درد و عوارض بعد از عمل مقایسه شدند. تجزیه و تحلیل داده­ها با استفاده از نرم‌افزار آماری SPSS (نسخه 22) و آزمون‌های آنووا و کای اسکوئر انجام شد. میزان p کمتر از 05/0 معنی‌دار در نظر گرفته شد. یافته‌ها: میانگین نمره شدت درد در 6 ساعت اول پس از عمل، در بیماران تحت درمان با استامینوفن داخل وریدی به‌طور معنی‌داری کمتر از گروه مپریدین داخل وریدی بود (001/0p<). در گروه مپریدین به همراه استامینوفن، متوسط نمره شدت درد، طول مدت بهبودی و فاصله زمانی بین تزریق اولیه و دوز اضافی ضددرد در مقایسه با دو گروه دیگر، به‌طور معنی‌داری کمتر بود (001/0p<). به‌علاوه، بروز سردرد پس از عمل در گروه­های مپریدین و لرز پس از عمل در گروه­های استامینوفن به‌طور معنی‌داری بیشتر از گروه ترکیبی بود (005/0=p). نتیجه‌گیری: استامینوفن در کنترل درد پس از عمل نسبت به مپریدین اثر بیشتری داشته و ترکیب آنها دارای اثر ضددرد بسیار بیشتر و عوارض جانبی کمتری می­باشد.}, keywords_fa = {استامینوفن,درد بعد از عمل,سزارین,مپریدین}, url = {https://ijogi.mums.ac.ir/article_13385.html}, eprint = {https://ijogi.mums.ac.ir/article_13385_d3e24558f622a19d0c5371d6c7c2fec4.pdf} }
