Methods: This descriptive study was performed on 200 pregnant women with gestational age of 35-37 weeks who had referred to Shahid Beheshti hospital in Isfahan during one year (2015) for monthly examinations. Sampling was done from the vaginal area (a third terminal) and rectum by sterile swabs separately. Collected specimens were transferred to enrichment broth medium and after 4 and 24 hours incubation, PCR method was performed. After 24 h, all specimens were evaluated using phenotypic methods. Also, sensitivity and specificity of molecular method were calculated. Data were analyzed by SPSS software (version 19), and logistic regression. PResults: Among 200 pregnant women, 22 (11%) were colonized using phenotypic and 27 (13.5%) were colonized using PCR methods after 4 and 24 hours; no difference was observed between the results. According to the results of statistical tests, the sensitivity of molecular method after the mentioned times was 100% and the specificity were 97% and 95% for vaginal and rectal specimens, respectively. Conclusion: Using PCR method after 4 hoursincubation of enrichment medium not only increase the speed of the colonization detection in pregnant women with Streptococcus agalactiae, but also provides the opportunity to perform the antibiotic susceptibility tests after bacteria growth and chose the appropriate prophylaxis or treatment if needed.}, keywords = {Colonization,Molecular diagnosis,Streptococcus agalactiae}, title_fa = {بررسی صحت تشخیص مولکولی کلونیزاسیون زنان باردار با استرپتوکوک گروه B به دنبال کشت غنی کننده باکتری}, abstract_fa = {مقدمه: کلونیزاسیون زنان باردار با استرپتوکوک گروه B، علاوه بر ایجاد عفونت در مادر می‌تواند خطر زایمان زودرس و انتقال باکتری در حین زایمان به نوزاد را افزایش­ دهد و این مسئله می­تواند باعث ایجاد سپسیس و مننژیت نوزادی در 6 هفته اول زندگی شود، لذا مطالعه حاضر با هدف تشخیص کلونیزاسیون زنان باردار مراجعه کننده به دو بیمارستان شهر اصفهان با باکتری استرپتوکوکوس آگالاکتیه به‌وسیله روش PCR، 4 و 24 ساعت بعد از کشت غنی کننده و مقایسه آن با نتایج تشخیص فنوتیپی انجام شد. روش‌کار: این مطالعه توصیفی بر روی 200 زن باردار در هفته 37-35 بارداری که در طی یک دوره یک‌ساله (فروردین تا اسفند ماه 1394) جهت معاینات ماهیانه به بیمارستان شهید بهشتی شهر اصفهان مراجعه کرده بودند، انجام شد. نمونه‌گیری از دو ناحیه واژن (یک سوم انتهایی) و رکتوم به‌وسیله سواب استریل و به‌طور جداگانه صورت گرفت. نمونه های تهیه شده به محیط کشت غنی کننده منتقل و پس از گذشت 4 و 24 ساعت از گرمخانه‌گذاری PCR انجام شد. بعد از گذشت 24 ساعت، محیط­های کشت توسط روش‌های فنوتیپی نیز مورد بررسی قرار گرفتند. همچنین حساسیت و اختصاصیت روش مولکولی استفاده شده محاسبه گردید. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار آماری SPSS (نسخه 19) و آزمون رگرسیون لجستیک انجام شد. میزان p کمتر از 05/0 معنی‌دار در نظر گرفته شد. یافته‌ها: از 200 زن باردار مورد بررسی، 22 نفر (11%) به‌وسیله روش فنوتیپی کلونیزه تشخیص داده شدند و 27 نفر (5/13%) به وسیله روش PCR بعد از 4 و 24 ساعت کلونیزه بودند و هیچ تفاوتی بین نتایج مشاهده نشد. بر اساس نتایج آزمون‌های آماری، روش مولکولی بعد از زمان‌های ذکر شده دارای حساسیت 100% و اختصاصیت 97% و 95% به‌ترتیب برای نمونه­های واژینال و رکتال بود. نتیجه‌گیری: استفاده از روش مولکولی PCR، 4 ساعت پس از کشت غنی‌کننده می­تواند علاوه بر اینکه سرعت تشخیص کلونیزاسیون زنان باردار با استرپتوکوکوس آگالاکتیه را افزایش دهد، می­تواند این امکان را فراهم کند که تست‌های حساسیت آنتی‌بیوتیکی نیز بعد از رشد باکتری انجام شود و در صورت نیاز، پیشگیری دارویی و یا درمان مناسب انجام شود.}, keywords_fa = {استرپتوکوکوس آگالاکتیه,تشخیص مولکولی,کلونیزاسیون}, url = {https://ijogi.mums.ac.ir/article_12649.html}, eprint = {https://ijogi.mums.ac.ir/article_12649_5c1ed5b6dc70eef89cfcd49f978e9246.pdf} }
